Primary orbital liposarcoma.

A 45-year-old-man presented a slightly painful proptosis and diplopia for 7 months. He had been kept elsewhere on oral steroids without evidence of any clinical response over an 8-week period with suspected diagnosis of an inflammatory pseudotumor upon referral to our clinic. An intraoperative biopsy positive for primary liposarcoma was followed by debulking surgery. Exenteration and radiotherapy were performed after pathologic confirmation of this diagnosis. No recurrence has been observed after 2 years of follow-up. We underline the importance of an accurate an early diagnosis in the managem

[Imaging of orbital masses].

A wide spectrum of tumors and pseudotumors may involve the orbit. Their clinical presentation is often nonspecific, consisting of exophthalmos, diplopia, visual loss, orbital pain or signs of orbital inflammation. Imaging plays a major role in the etiological diagnosis, providing a precise analysis of the location of the lesion, its components, and its effects on adjacent and nearby structures. It studies tumor extension and often provides a good evaluation of its composition. Imaging is mainly performed with MRI. CT is needed when the lesion affects the orbital walls. Ultrasound is proposed i…

Management of orbital reconstruction.

One of the concerns after a major trauma to the orbit is to be rebuilt. This can be achieved with care in management of orbital reconstruction, techniques, types of materials, and osseointegrated implants; it allows satisfactory aesthetic recuperation and well-being, whereas psychological therapy allows for patients' reinsertion in social and family environment to make them feel happier and safer.

Update on orbital reconstruction.

PURPOSE OF REVIEW: Orbital trauma is common and frequently complicated by ocular injuries. The recent literature on orbital fracture is analyzed with emphasis on epidemiological data assessment, surgical timing, method of approach and reconstruction materials. RECENT FINDINGS: Computed tomographic (CT) scan has become a routine evaluation tool for orbital trauma, and mobile CT can be applied intraoperatively if necessary. Concomitant serious ocular injury should be carefully evaluated preoperatively. Patients presenting with nonresolving oculocardiac reflex, 'white-eyed' blowout fracture, or …

Odontogenic orbital cellulitis.

PURPOSE: To describe a small series of patients with odontogenic orbital cellulitis and review the visual outcomes of such patients reported in the scientific literature. METHODS: Review of medical records and the scientific literature. Measured parameters included gender, age, days to presentation and surgery, the need for surgical intervention(s), sinus and orbital involvement on CT imaging, and pathogens involved. RESULTS: Current cases and review of the scientific literature resulted in 24 patients with odontogenic orbital cellulitis and adequate examinations for statistical analysis. Tw…

[Orbital hydatid cyst].

CASE: A 72-year-old man presented with left exophthalmia, impaired visual acuity and ocular pain. CT revealed a posterolateral homogeneous unilocular cyst in the left orbital cavity. Ultrasound showed a 38 x 17 mm purely cystic retro-ocular thin lined mass. Hydatid serologic tests were positive. The cyst was completely enucleated, after irrigation with hypertonic saline solution. The diagnosis was confirmed histologically. DISCUSSION: The orbital hydatic cyst is a rare pathology in Western countries but remains relatively frequent in North-African countries. It can be observed in urban center…

Orbital arteriovenous malformations.

OBJECTIVE: To present the clinical features, management, and outcomes in a series of patients with orbital arteriovenous malformations (AVMs). METHODS: Clinical records of patients with orbital AVMs confirmed using angiography were reviewed as a retrospective, noncomparative, interventional case series. RESULTS: Eight patients (3 women and 5 men) with unilateral AVMs and a mean age of 39 years (median, 36.5 years; range, 26-70 years) were reviewed. Findings existed for an average of 11.2 years before diagnosis and included periocular mass (7 patients, 88%); periocular edema, pulsation/bruit,…

Orbital inflammatory disease.

OBJECTIVES: To familiarize rheumatologists and internists with the signs, symptoms, and management of orbital inflammatory disease (OID). METHODS: A comprehensive literature review related to OID was performed and reported from the perspectives of rheumatology, ophthalmology, and radiology. RESULTS: OID is a general term encompassing inflammatory diseases that affect some or all of the structures contained within the orbit external to the globe. Orbital involvement as a part of the initial symptom complex is not uncommon for systemic diseases such as Graves' disease, Wegener's granulomatosis…

Orbital cellulitis in children.

BACKGROUND: To review the epidemiology and management of orbital cellulitis in children. METHODS: The medical records of children < or = 18 years old and hospitalized from June 1, 1992, through May 31, 2002, at the Brenner Children's Hospital, with a discharge ICD-9 code indicating a diagnosis of orbital cellulitis and confirmed by computed tomography scan were reviewed. A literature search for additional studies for systematic review was also conducted. RESULTS: Forty-one children with orbital cellulitis were identified. The mean age was 7.5 years (range, 10 months to 16 years), and 30 (73%…

Update on orbital anatomy.

OBJECTIVES: Portal hypertensive gastropathy (PHG) is a diagnosis made based on endoscopic findings in the appropriate clinical setting. Biopsy may be taken during endoscopy for correlation, but the pathologist may encounter a myriad of nonspecific histologic findings. We undertook this study to evaluate contexts where a histologic diagnosis of PHG might be rendered on biopsy. METHODS: Two cohorts were established: stomach biopsy specimens from patients with cirrhosis or undergoing varices screening (n = 188) and stomach biopsy specimens with findings interpreted as PHG in the pathology report
