Mechanisms of Change in Population-Based Parenting Interventions for Children and Adolescents.

OBJECTIVE: Developing an effective population-level system of evidence-based parenting support capable of shifting (at a population level) rates of child maltreatment and social, emotional, and behavioral problems in children requires an integrated theory of change. This paper presents a systems-contextual model of change and identifies modifiable mechanisms that can potentially explain population-level changes in parenting and child outcomes. METHOD: Using the Triple P-Positive Parenting Program as an exemplar of a tiered, multi-level system of evidence-based parenting interventions, we disc

Culture Change in Nursing Homes: What Is the Role of Nursing Home Resources?

Quality of care has been a long-standing issue in US nursing homes. The culture change movement attempts to transition nursing homes from health care institutions to person-centered homes. While the adoption of culture change has been spreading across nursing homes, barriers to adoption persist. Nursing homes that disproportionately serve minority residents may have additional challenges implementing culture change compared with other facilities due to limited financial and staffing resources. The objective of this study was to examine how nursing home characteristics are associated with cultu

Contextual factors associated with uptake of breast and cervical cancer screening: A systematic review of the literature.

Existing research on barriers to breast and cervical cancer screening uptake has focused primarily on socio-demographic characteristics of individuals. However, contextual factors, such as service organization, as well as healthcare providers' training and practices, are more feasibly altered to increase health service use. The objective of the authors in this study was to perform a critical systematic review of the literature to identify contextual factors at the provider- and system-level that were associated with breast and cervical cancer screening uptake. Studies published from 2000 to 20

Development of an integral assessment approach of health status in patients with obstructive airway diseases: the CORONA study.

BACKGROUND: Traditional assessment of patients with obstructive lung diseases (asthma and chronic obstructive pulmonary disease; COPD) relies on physiological tests. The COPD and Asthma Rotterdam Integrated Care Approach (CORONA) study aims to develop a diagnostic pathway with a more comprehensive approach to the assessment of patients with asthma and COPD in secondary care. METHODS: An eight-step method was used to develop and implement the pathway for patients with asthma or COPD referred to an outpatient hospital setting. RESULTS: The diagnostic pathway consists of an evidence-based set o

Satisfaction, commitment, and psychological well-being among HMO physicians.

OBJECTIVE: To identify the factors that predict professional satisfaction, organizational commitment, and burnout among physicians working for health maintenance organizations (HMOs). METHODS AND PARTICIPANTS: Data came from mail surveys of Kaiser Permanente physicians in the Northwest and Ohio regions. The average response rate was 80% (n = 608). RESULTS: The single most important predictor for all 3 outcomes was a sense of control over the practice environment. Other significant predictors included perceived work demands, social support from colleagues, and satisfaction with resources. The

"Do you guys tell anybody?": Exploring allocation and implementation of opioid settlement funds and reduced access to health vending machines for people who use drugs.

OBJECTIVE: This study examined how county-level decision-making shaped implementation and accessibility of an opioid-settlement-funded health vending machine (HVM) in California. METHOD: We conducted approximately 250 hours of ethnographic fieldwork and qualitative interviews with 17 HVM users. Using constructivist grounded theory, we analyzed the data to explore how structural and political dynamics influenced program awareness and engagement. RESULTS: Three themes emerged: (1) county leadership placed the HVM in a low-visibility location to minimize anticipated political and organizational

In later life health-related changes, the loss of social relationships and institutional transitions can substantially impair communication. OBJECTIVE: Based on group interviews with professionals this paper examines the significance of communication partners, the professional and institutional prerequisites for successful communication in later

Signs of safety: An investigation of how OHS professionals interpret injury metrics.

INTRODUCTION: Injury metrics are traditional indicators of safety performance widely used by organizations, industry bodies, and regulatory agencies. In this paper we contend that they are semiotic signs leading to a variety of interpretations beyond their traditionally held meaning as a representation of an organization's safety performance. We argue that injury metrics are semiotic signs where different organizational uses and signals lead to a variety of interpretations amongst internal and external stakeholders. METHOD: This exploratory study reveals these different uses and meanings thro

Co-Designing Exploratory Paths for the Smart and Sustainable Transformation of Healthcare.

Healthcare systems are complex ecosystems facing increasing pressure to balance quality, affordability, and sustainability. Rising costs, aging populations, and climate change pose significant challenges, prompting a shift toward sustainability transformation and interdisciplinary collaboration. To address these challenges, digital technologies, including AI and smart health systems, are being leveraged to enhance efficiency, optimize resource allocation, and support sustainable healthcare models. Nevertheless, healthcare remains constrained by siloed organizational structures that hinder cros
