Tennis elbow tendinosis (epicondylitis).

Tennis elbow tendinosis (epicondylitis) is most commonly caused by tendon overuse and failed tendon healing. The pathoanatomy of overuse tendinopathy is noninflammatory "angiofibroblastic tendinosis." The specific areas of elbow abnormality include the extensor carpi radialis brevis-extensor digitorum communis complex laterally, the pronator teres, flexor carpi radialis medially, and triceps posteriorly. The primary goal of nonsurgical treatment is to revitalize the unhealthy tissue that produces pain. Successful nonsurgical treatment comprises rehabilitative resistance exercise and progressio

Adult spinal deformity in the osteoporotic spine: options and pitfalls.

Osteoporosis has received heightened attention over the past 2 decades because of its overwhelming cost to society. It is one of the most common diseases affecting both men and women. The key to treatment is early prevention accompanied by modification of risk factors and impact-oriented exercise, optimal medical management with antiresorptive medications, and addressing the complications of this disease such as compression fractures and spinal deformities. Most osteoporotic vertebral compression fractures can be treated nonsurgically, but new techniques such as vertebroplasty and kyphoplasty

Electrothermal chondroplasty--monopolar.

Despite the lack of clinical studies, monopolar RF energy is currently used for the treatment of grade 2, 3, and 4 chondromalacia. Basic science studies have confirmed that monopolar RF can mechanically debride the articular surface by smoothing, potentially reducing further delamination. However, a zone of thermal injury in the areas of treatment has been observed with loss of chondrocyte viability. Therefore, further use of this procedure lies in the surgeon's understanding of the potential deleterious effects of temperature on chondrocyte viability and the limitations of alternate treatment

The hand in recessive dystrophic epidermolysis bullosa.

Recessive dystrophic epidermolysis bullosa is still a difficult challenge for surgeons and doctors. Hand retractions are different from burns. The medical team must have a good knowledge of the disease. Surgery must be performed with the collaboration of every member of the team, as well as the family. In young children, complete correction of the retractions is possible. On the other hand, when retractions are present for a long time in children and adolescents, surgery is purely functional. Static and dynamic splints delay recurrences, but their prolonged use presents psychological obstacles

Osteonecrosis of the femoral head after traumatic hip dislocation in the adult.

Osteonecrosis of the femoral head is most commonly classified according to the system of Ficat and Arlet, and the choice of treatment is based directly on the corresponding stage. Therefore it is important to accurately determine the stage of the disease because it has important bearings on the patient's clinical course. Patients with Stage I lesions (preradiologic stage of the disease) usually can be treated with pulsed electromagnetic fields with or without a core decompression, bone grafting and decompression, and rotational intertrochanteric osteotomy. These procedures are equally applicab

Practice habits in board-eligible shoulder and elbow orthopedic surgeons.

BACKGROUND AND PURPOSE: There is little data regarding newly practicing orthopedic surgeons and their surgical preferences during the 6-month case collection for the American Board of Orthopaedic Surgery Part-II. The purpose of this study was to conduct a comprehensive survey analysis regarding the practice habits of board-eligible orthopedic surgeons specializing in shoulder and elbow surgery. We hypothesize that surgeons would opt out of most complex surgical cases and controversial treatment options during the time of boards case collection. METHODS: An anonymous 27-question survey was cre

Enhanced recovery after surgery (ERAS) in elective intertrochanteric fracture patients result in reduced length of hospital stay (LOS) without compromising functional outcome.

BACKGROUND: Enhanced recovery after surgery (ERAS) has rapidly gained popularity among hip or knee arthroplasty area which can decrease hospital length of stay (LOS). However, limited data exist regarding its safety and efficacy among intertrochanteric fracture patients. The purpose of this study was to determine if LOS associated with intertrochanteric fracture patients can be improved following an existing orthopedic ERAS procedure. METHODS: We reviewed the outcomes of all patients who had been treated with the PFNA intramedullary fixation at our institution. Open fractures, metastatic path

Review of Intravenous Acetaminophen for Analgesia in the Postoperative Setting.

BACKGROUND: Acetaminophen is a non-opioid analgesic commonly utilized for pain control after several types of surgical procedures. METHODS: This scoping primary literature review provides recommendations for intravenous (IV) acetaminophen use based on type of surgery. RESULTS: Intravenous acetaminophen has been widely studied for postoperative pain control and has been compared to other agents such as NSAIDs, opioids, oral/rectal acetaminophen, and placebo. Some of the procedures studied include abdominal, gynecologic, orthopedic, neurosurgical, cardiac, renal, and genitourinary surgeries. R
