Cyst fenestration and Ommaya reservoir placement in endoscopic transcortical transventricular approach for recurrent suprasellar cystic craniopharyngioma without ventriculomegaly.

The efficacy of the endoscopic transcortical transventricular approach (ETTA) for craniopharyngioma in the third ventricle with hydrocephalus has been reported focusing on its reduced invasiveness. On the other hand, suprasellar craniopharyngioma without ventriculomegaly is generally surgically managed by craniotomy or the endoscopic endonasal approach (EEA). Here, we report an elderly patient who received cyst fenestration and Ommaya reservoir placement in ETTA for recurrent suprasellar cystic craniopharyngioma without ventriculomegaly. The ETTA as a less invasive procedure is feasible in pat

Transsphenoidal and infralabyrinthine approach of the petrous apex cholesterol granuloma.

Space-demanding or destructive changes in the petrous bone are often challenging differential diagnosis. Cholesterol granulomas of the petrous apex can clinically present in a combination of hearing loss, vertigo, tinnitus, chronic cephalgia, impairment of facial nerve function, neuralgic pain of the nervus trigeminus, or manifest diplopia by the nerve palsy of the nervus abducens. CT-morphologically cholesterol granulomas appear as soft-tissue density masses, which may display a discrete rim after intravenous administration of a contrast agent. The MRI, T1 as well as T2-weighted images show a

Sympathetic hearing loss: A review of current understanding and report of 2 cases.

Sympathetic hearing loss (SHL) is a rare complication that appears to occur when the immune system is primed against sequestered inner ear antigens. SHL has been implicated in delayed hearing loss after acoustic tumor removal, revision stapedectomy, and temporal bone fractures. We present 2 cases of suspected SHL after excision of skull base tumors during which the surgical approach had violated the otic capsule. Both patients experienced delayed contralateral sensorineural hearing loss. In comparing our cases with those previously reported, we hope to inspire others to do likewise and to deve

Paper patching of the tympanic membrane as a symptomatic treatment for patulous eustachian tube syndrome.

BACKGROUND: Application of one or several paper patches on the tympanic membrane can improve autophony and other symptoms in some patients with a patulous eustachian tube. METHOD: The charts of 21 patients treated for patulous eustachian tube syndrome between 2006 and 2011 were studied for subjective improvement of symptoms following the application of paper patches. RESULTS: Paper patching of the tympanic membrane provided symptomatic relief in 76.2 per cent of patients; relief was permanent in 50 per cent of these patients. CONCLUSION: Paper patching of the tympanic membrane is a simple a

Useful surgical techniques for facial nerve preservation in tumorous intra-temporal lesions.

The management of the facial nerve in tumorous temporal lesions is particularly challenging due to its complex anatomic location and potential postoperative complications, including permanent facial paralysis. The most important concern regarding surgical treatment of a tumorous temporal lesion is the inevitable facial paralysis caused by nerve injury during the tumor removal, especially in patients with minimal to no preoperative facial nerve dysfunction. We describe successful four cases in which various surgical techniques were developed for the preservation of the facial nerve in treatment

[Peculiarities of sanitizing middle ear reoperations].

This paper is devoted to the problem of surgical rehabilitation of patients with the so-called <<operated ear disease>>. Twenty such patients underwent second operation following sanitization ear surgery for the treatment of recurrent (more than once a year) otorrhoea in the previous period. The operation was performed in two (sanitizing and reconstructive) steps. All the patients were allocated to 3 groups based on the results of preoperative examination and the cause of purulent discharge. Group 1 comprised patients with the <<open>> Eustachian tube, group 2 included patients with the cariou

A case of ceruminous adenocarcinoma of the external auditory canal presenting as an aural polyp.

Ceruminous adenocarcinoma is a rare malignant tumor arising from the ceruminous gland of the external auditory canal ceruminous gland. Usually it displays no specific clinical symptoms or signs, so most reported cases have involved an advanced tumor and the treatment results have been disappointing. It is important to make an early diagnosis to improve the treatment's outcome. We present the case of a 54-year-old female patient with a 1-year history of feeling blockage in her left ear. Otoscopic examination revealed an aural polyp arising from the cartilaginous ear canal's anterosuperior wall.

Conductive deafness with normal eardrum: absence of the long process of the incus.

CONCLUSIONS: The most likely cause of lesions with an absence of the long process of the incus was congenital origin because many cases were associated with the stapedial anomaly. However, the possibility of a potential inflammation of this lesion could not be completely excluded, especially in cases with only an absence of the long process of the incus. OBJECTIVE: To clarify whether the absence of the long process of the incus is caused by congenital anomaly or a potential inflammation. SUBJECTS AND METHODS: A total of 21 patients (5 bilateral cases, 16 unilateral cases) with absence of the

The chondroid syringoma: report of a case arising from the external auditory canal.

Chondroid syringoma is an uncommon, benign skin adnexal tumour. It usually presents as a slowly growing intradermal or subcutaneous nodule on the face. Treatment involves local excision with a cuff of normal tissue to prevent recurrence. A rare case of chondroid syringoma arising from the external auditory canal is reported. The clinical features, histology, treatment and follow-up are discussed.

The role of micro-noise trauma in the etiology of aging-related changes in the inner ear.

Eleven chinchillas between 1 and 2.4 years of age had the malleus/incus complex removed from one middle ear and then lived in the Washington University animal facilities for 4 years post-surgery. Each animal had one ear (termed ambient-noise) in which the conductive apparatus was intact; the other ear (termed noise-protected) had a 50-60 dB conductive hearing loss. The background sound level in the animal facility was 59 dBA with periodic brief sounds up to 102 dBA. After the 4-year experimental period, both ears were fixed, embedded in plastic and dissected for microscopic examination as flat

A Case of Bilateral Congenital Aural Stenosis with Down Syndrome Underwent Canalplasty with Underwater Transcanal Endoscopic Ear Surgery.

Congenital aural stenosis (CAS) sometimes can be a risk for external auditory canal (EAC) cholesteatoma and may require surgical treatment. We report a child of Down syndrome with bilateral CAS who underwent bilateral canalplasty with underwater transcanal endoscopic ear surgery (TEES). Case Report: A 12-year-old boy with bilateral EAC stenosis, in whom tympanostomy tube insertion for secretory otitis media (SOM) had failed at 5 and 8 years of age, was referred to our department for surgical treatment. The bilateral EACs were so narrow and curved that the tym-panic membranes could not be entir

Investigating the learning curve in endoscopic compared to microscopic myringotomy and ventilation tube insertion.

OBJECTIVE: Rate of learning is often cited as a deterrent in the use of endoscopic ear surgery. This study investigated the learning curves of novice surgeons performing simulated ear surgery using either an endoscope or a microscope. METHODS: A prospective multi-site clinical research study was conducted. Seventy-two medical students were randomly allocated to the endoscope or microscope group, and performed 10 myringotomy and ventilation tube insertions. Trial times were used to produce learning curves. From these, slope (learning rate) and asymptote (optimal proficiency) were ascertained.
