Observational study of surgical resection in small non-functional pancreatic neuroendocrine tumors: AS SEER-based study.

The potential benefits of surgical resection for small non-functional pancreatic neuroendocrine tumors (NF-PNETs) in terms of survival remain uncertain. This study aimed to evaluate the impact of surgical treatment on patients with NF-PNETs. Using SEER data, we identified 1102 patients from 2004 to 2015 with well and moderately differentiated pancreatic neuroendocrine tumors (PNETs). The associations between continuous variables and receipt of surgery were assessed using Wilcoxon rank-sum tests. Kaplan-Meier survival curves for OS were compared using the log-rank test. We compared outcomes in

Static and dynamic quantitative sensory testing in chronic pancreatitis: Relation of pain sensitization to patient-reported outcomes and psychiatric comorbidities.

BACKGROUND: The etiology of pain in chronic pancreatitis is multi-factorial and includes altered central pain processing. Pancreatic Quantitative Sensory Testing (P-QST) indirectly assesses pain sensitization, yet its clinical significance in relation to psychological comorbidities and quality of life (QoL) remains unclear in Indian populations. The primary objective was to apply an unbiased, data-driven clustering approach using P-QST parameters to stratify patients into centrally sensitized and non-sensitized phenotypes and evaluate their associations with demographic, clinical and patient-r

Long-term efficacy and safety of lomitapide in patients with familial chylomicronemia syndrome: Data from an expanded access program.

BACKGROUND: Familial chylomicronemia syndrome (FCS) is a rare, severe, autosomal recessive disorder characterized by extremely high triglyceride (TG) levels and an increased risk of acute and/or recurrent pancreatitis. Lomitapide, a microsomal triglyceride transfer protein (MTP) inhibitor, is approved for the treatment of homozygous familial hypercholesterolemia. The open-label, single-arm LOCHNES study (EudraCT 2018-002911-80) investigated lomitapide in adult patients with genetically confirmed FCS and a history of pancreatitis, demonstrating its efficacy and tolerability. METHODS: Fourteen

Attenuated FOLFIRINOX in the salvage treatment of gemcitabine-refractory advanced pancreatic cancer: a phase II study.

BACKGROUND: Combination therapy with oxaliplatin, irinotecan, fluorouracil, and leucovorin (FOLFIRINOX) chemotherapy drastically improves survival of advanced pancreatic cancer patients. However, the efficacy of FOLFIRINOX as a second-line treatment after gemcitabine failure has not been tested prospectively. We investigated the feasibility and safety of attenuated FOLFIRINOX in patients with gemcitabine-refractory advanced pancreatic cancer. METHODS: A multicenter phase II prospective open-label, single-arm study was conducted at 14 hospitals. Patients with histologically proven invasive duc

Simultaneous endoscopic and laparoscopic management of acute biliary pancreatitis in the second trimester of pregnancy: A case report.

INTRODUCTION AND IMPORTANCE: Acute biliary pancreatitis during pregnancy is a rare but significant clinical challenge that requires a delicate balance between maternal and fetal safety. Gallstone disease remains among the leading causes of acute pancreatitis in pregnancy due to hormonal and physiological changes that predispose pregnant women to biliary stasis and sludge formation. While conservative management is often preferred in mild cases, persistent biliary obstruction or recurrent symptoms necessitate definitive intervention. ERCP and laparoscopic cholecystectomy have emerged as safe an

Neutrophil-lymphocyte ratio, gamma-glutamyl transpeptidase, lipase, high-density lipoprotein as a panel of factors to predict acute pancreatitis in pregnancy.

Acute pancreatitis in pregnancy (APIP) is a rare but dangerous complication. APIP has common symptoms with acute abdomen. Assessment of an acute abdomen is more complicated during pregnancy because the gravid uterus could mask most of symptomatic signs. It has been a challenge to diagnose APIP by physical examination or diagnostic imaging. Case studies on APIP are also limited for analysis on the risk factors associated with the disease. This retrospective study evaluated a series of risk factors from a relatively substantial number of APIP cases to determine early predictors or prognosis mark

Unusual presentation of primary sclerosing cholangitis.

A 23-year-old man presenting with acute pancreatitis and autoimmune hemolytic anemia was diagnosed with primary sclerosing cholangitis (PSC) without evidence of ulcerative colitis. This constellation of rare associations constitutes a unique mode of presentation of PSC. Within two years he also developed ankylosing spondylitis with sacroiliitis. Disordered immune regulation as a major factor in the mechanism of injury in PSC is supported by its increased association with other immunologically mediated disorders, most notably ulcerative colitis. Autoimmune hemolytic anemia, however, has been re

Can semaglutide influence tumor biomarkers? Insight from a clinical case.

To our knowledge, this is the first case reported in literature, regarding an increase in tumoral biomarkers during a treatment with Semaglutide. A 61-year-old patient was referred to our gynecological consultation due to suspected adnexal cysts following an MRI performed for a functional defecation disorder. The patient was on Semaglutide treatment. Gynecological examination and transvaginal ultrasound were normal, with no evidence of adnexal cysts. Tumor biomarker testing revealed a significant increase in CA 19-9 and a mild elevation in CA-125. In an asymptomatic patient, these findings rai

Functional Inactivation of PAX4 Results in Disrupted Endocrine Pancreas Development and Neonatal Diabetes in Pigs.

Variants in the human PAX4 gene are associated with both monogenic and complex forms of diabetes, yet their pathogenic effects remain difficult to define in models that accurately mimic human islet architecture and neonatal metabolic transitions. Here, we created a porcine PAX4 loss-of-function model using CRISPR/Cas9 cytidine deaminase base editing to introduce a premature stop codon in the PAX4 coding sequence. PAX4 knockout piglets developed severe hyperglycemia within 24 h of birth, followed by rapid postnatal clinical deterioration and uniform death by day 3. Biochemical analysis showed s

[Review of Multiple Endocrine Neoplasia Type 1 Based on a Clinical Case].

Multiple endocrine neoplasia type 1 (MEN1) is a rare autosomal dominant disease with an estimated prevalence of 2 per 100,000. This disease is caused by a mutation in the tumor suppressor gene MEN1, which is located on chromosome 11 and codifies the menin protein. It is characterized by a predisposition of parathyroids, enteropancreatic, and anterior pituitary tumors, affecting the quality of life and lifespan of those who have the disease. Clinical case: We present a case of a 24-year-old woman with recurrent episodes of symptomatic hypoglycemia and first-degree relatives who were diagnosed w

Genetic determinants of circulating GIP and GLP-1 concentrations.

The secretion of insulin and glucagon from the pancreas and the incretin hormones glucagon-like peptide-1 (GLP-1) and glucose-dependent insulinotropic peptide (GIP) from the gastrointestinal tract is essential for glucose homeostasis. Several novel treatment strategies for type 2 diabetes (T2D) mimic GLP-1 actions or inhibit incretin degradation (DPP4 inhibitors), but none is thus far aimed at increasing the secretion of endogenous incretins. In order to identify new potential therapeutic targets for treatment of T2D, we performed a meta-analysis of a GWAS and an exome-wide association study o

Centroacinar Cells Are Progenitors That Contribute to Endocrine Pancreas Regeneration.

Diabetes is associated with a paucity of insulin-producing β-cells. With the goal of finding therapeutic routes to treat diabetes, we aim to find molecular and cellular mechanisms involved in β-cell neogenesis and regeneration. To facilitate discovery of such mechanisms, we use a vertebrate organism where pancreatic cells readily regenerate. The larval zebrafish pancreas contains Notch-responsive progenitors that during development give rise to adult ductal, endocrine, and centroacinar cells (CACs). Adult CACs are also Notch responsive and are morphologically similar to their larval predecesso

No improvement of pancreas transplant endocrine function by exogenous insulin infusion (islet rest) in the postoperative period.

The concept of islet exhaustion maintains that exposure of pancreatic islets to hyperglycemia and other stresses leads to islet dysfunction and irreparable damage. The process of pancreatic transplantation places many stresses on islets (e.g., counter-regulatory hormones, steroids, cyclosporine toxicity). As practiced by some centers, it may be important to administer exogenous insulin in the postoperative period to provide islet rest. Using a porcine pancreas transplant model that simulates clinical transplantation, we studied 2 groups: 1 group (n = 8) received constant insulin infusion for 7

Preconditioning with associated blocking of Ca2+ inflow alleviates hypoxia-induced damage to pancreatic β-cells.

OBJECTIVE: Beta cells of pancreatic islets are susceptible to functional deficits and damage by hypoxia. Here we aimed to characterize such effects and to test for and pharmacological means to alleviate a negative impact of hypoxia. METHODS AND DESIGN: Rat and human pancreatic islets were subjected to 5.5 h of hypoxia after which functional and viability parameters were measured subsequent to the hypoxic period and/or following a 22 h re-oxygenation period. Preconditioning with diazoxide or other agents was usually done during a 22 h period prior to hypoxia. RESULTS: Insulin contents decreas

A Five-Year Journey to Diagnosis: Resolving Persistent Hypoglycemia Through Successful Insulinoma Resection-A Case Report.

Insulinoma is a rare functional pancreatic neuroendocrine tumor with an annual prevalence of 0.5-5 cases per million. It is characterized by excessive insulin secretion, leading to recurrent hypoglycemia, often diagnosed through Whipple's triad: hypoglycemic symptoms, documented low plasma glucose, and symptom resolution after glucose administration. Approximately 90% of insulinomas are sporadic, while 10% are associated with multiple endocrine neoplasia type 1. Diagnosis is frequently delayed due to nonspecific symptoms and misattributions to neurological or psychiatric conditions. Biochemica

Pancreatic neuroendocrine tumor producing vasopressin: A case report.

RATIONALE: Functional pancreatic neuroendocrine tumors (pNETs) rarely produce vasopressin. Here, we reported a case of pNET producing vasopressin in a 78-year-old man with hyponatremia. PATIENT CONCERNS: The patient presented with anorexia approximately 4 years ago, and the laboratory test results indicated hyponatremia. He was hospitalized 3 times subsequently due to anorexia in the past 4 years, during which laboratory tests consistently indicated severe hyponatremia. DIAGNOSIS: Upon admission, his serum osmolarity, urine osmolarity, urine sodium level, and 24-hour urine sodium level was 2

[Endocrine consequences of corticotherapy. Weaning from long-term corticotherapy].

Long term corticosteroid therapy has multiple effects on the endocrine system. These include adrenocortical suppression in high doses, alteration of gonadotropic and gonadal secretion, slowing down of long bone growth and bone maturation in children, osteoporosis in adults, alteration of the thyrotropic function and changes in thyroxine peripheral metabolism, diabetogenic effect. The list is not exhaustive, and there are more complex effects on the secretion of prolactin, pancreatic peptides, VIP, etc. Because of these various effects, the pros and cons of long term corticosteroid therapy must
