[Surgical complications and their treatment options in total knee replacement].

INTRODUCTION: One of the most important achievements of modern orthopedics is the treatment of degenerative disorders of the knee joint with endoprostheses. The most often used type of knee prosthesis implanted in our times is the total condylar one. With increasing numbers of prosthesis operations the number of complications is also increasing. AIM: Knowledge of diagnostic and treatment principles of complications is important for all institutions applying knee prostheses. The aim of present study is to report the authors' experience with the most important surgical problems following knee a

Reconstruction of extensor mechanism after trauma and infection by transposition of the Achilles tendon: report of technique and four cases.

High-energy injuries to the knee often involve extensive soft tissue loss as well as fractures. In certain situations, the patellar tendon may be lost due to initial injury or post-injury complications such as infection. Use of the gastrocnemius flap is a popular method of covering the defect as well as controlling infection and promoting healing by increasing local vascularity. We present a method of reconstructing the patellar tendon by extending the use of the gastrocnemius flaps using its Achilles tendon and calcaneal insertion. The distal insertion of the Achilles tendon is fixed to the t

Arthroscopic lysis of adhesions improves knee range of motion after fixation of intra-articular fractures about the knee.

BACKGROUND: Post-traumatic stiffness after open reduction and internal fixation of fractures about the knee can have dramatic effects on function. Traditionally, open quadricepsplasty has been the treatment of choice, but is associated with significant morbidity. PURPOSE: The purpose of this study is to examine the immediate and sustainable range of motion (ROM) changes after surgical arthroscopic lysis of knee adhesions (SALKA) for post-traumatic knee stiffness after open reduction internal fixation (ORIF). METHODS: We retrospectively reviewed a consecutive series of patients at a single in

[Analysis of the results of femoral lengthening in 34 children and adolescents].

The authors have evaluated a complex of 34 femurs prolongated in the period 1978-1988 by means of Wagner (26), Ilizarov (5) or bisegmental external fixation device of their own design (3). The whole group comprised 13 boys and 21 girls in the age group ranging from 6.5 to 17.25 years of age. In 13 cases the shortening of the extremity was congenital, in 21 cases it was of secondary nature. All usual data were observed and the quality of healing in the distraction gap was assessed by means of x-ray check up. The average shortening in the above mentioned group of patients ranged from 40 to 50 mm

Catastrophic osteolysis in total knee replacement. A report of 17 cases.

One hundred eighty-five total knee revisions were done by the 2 senior authors from 1988 to 1994. Of these, 17 patients were identified as having severe osteolysis. The average age was 62.7 years, and the average weight was 90.8 g. All patients had osteoarthritis. Preoperative aspiration was negative in all patients. Eight patients had a metal-backed patella, 8 had a cementless implant, 4 had a cemented implant, and 5 had a hybrid implant. The average time interval from the index surgery to radiographic evidence of lysis was 56 months. Radiographic review showed gross polyethylene wear in 10 p

[Acute arthroscopy in clinically stable knees with hemarthrosis].

From 2006 arthroscopies of the knee, carried out between 1978 and 1983, 196 cases (9.8%) presented with traumatic haemarthroses and clinically stable capsular ligaments. A comparison was made between the standard clinical examination including routine X-ray and arthroscopy carried out under anaesthesia. Arthroscopy, as a diagnostic procedure, was found to be clearly superior with the same minimal complication rate. The standard clinical examination was inferior and inadequate particularly in cases with isolated torn anterior cruciate ligaments, in chondral injuries, and luxation of the patella

Patellar Cut and Composite Thickness: The Influence on Postoperative Motion and Complications in Total Knee Arthroplasty.

BACKGROUND: Little data exist on the influence of patellar thickness on postoperative motion or complications after total knee arthroplasty (TKA). This study addresses the following questions: Is postoperative motion influenced by change in composite patellar thickness? Is change in patellar thickness associated with more complications? And do more complications occur in the knees with a patellar bone remnant (<12 mm) and a native patellar thickness <18 mm? METHODS: In total, 3655 TKAs were performed by 3 surgeons over a 28-year interval. All knees had caliper measurement of patellar thicknes

Periprosthetic patellar fractures.

UNLABELLED: Patellar fracture after total knee arthroplasty is a rare yet challenging complication. Patellar fracture can occur as a result of trauma or it may be atraumatic. A multitude of factors can lead to periprosthetic patellar fracture including patient related factors, surgical technique related factors, and implant specific factors. Understanding the etiologic factors leading to atraumatic patellar fractures could result in minimizing complications. We present the results of peri-prosthetic patellar fractures in 12 patients. All type I non-displaced fractures (7 cases) were treated no

Surgical repair of massive rotator cuff tendon tears: Autologous quadriceps tendon graft versus arthroscopic repair.

INTRODUCTION: Massive rotator cuff tear repair results are variable. The main purpose of this study was to compare functional outcome between two procedures: open repair by autologous quadriceps-patella tendon patch, and arthroscopic suture. The study hypothesis was that there is no significant difference in results between the two techniques. MATERIAL AND METHODS: A retrospective study included all patients younger than 70 years operated on from 1995 to 2013 for massive rotator cuff tear. Exclusion criteria comprised history of dislocation, fracture or surgery or osteoarthritis in the affect
