To determine splenic pathology in thrombotic thrombocytopenic purpura (TTP), 10 spleens and two accessory spleens were studied. The eight women and two men ranged from 20 to 66 years of age (mean age, 39 years). Three spleens were enlarged. Thrombi were noted in arteries and arterioles in nine specimens: no associated inflammation was seen. A principal goal of mammographic screening is the early detection of breast cancer. We reviewed records of 125 women who were referred because of nonpalpable, suspicious abnormalities on mammogram, which subsequently proved to be cancer, requiring mammographic localization biopsy and subsequent surgery for therapy. We found that 72 (57.6%) had invasive tumors, 15 (12%) showed evidence of microinvasion and 38 (30.4%) were noninvasive. We describe the results of 43 bronchoscopic examinations carried out on 35 patients who were HIV positive. Of these, 13 were intravenous drug abusers, 13 were homosexual or bisexual, six were haemophiliacs, two were homosexual intravenous drug abusers and one was a heterosexual contact of a drug abuser. Pituitary morphology was studied in 49 autopsied patients with acquired immunodeficiency syndrome. Direct infectious involvement was noted in six adenohypophyses (12%), including five cases by cytomegalovirus and one by Pneumocystis carinii. Two cases with neurohypophysial lesions presumably caused by cytomegalovirus and one questionable case of Toxoplasma gondii were also observed. Murine myelogenous leukemias can be classified into several distinct subgroups based on morphology, cytochemical staining, and immunoreactivity. Tonsillectomy is one of the most controversial topics in otolaryngology. Today the immunologic role of the lymphoid tissue allows to clarify the surgical indications. Chronic bacterial tonsillitis, obstructive hypertrophic tonsils, peritonsillar abscess, suspicious of malignancy and fever convulsions produced by tonsillitis are the absolute indications for tonsillectomy. We review the literature on this topic.

Nonspecific aortoarteritis is a systemic autoimmune disease eventuating in gradual stenosis of the aorta and the main vessels with ischemia of the respective organs. Ophthalmologic symptoms have been examined in 54 patients with nonspecific aortoarteritis. Thyroglobulin, a marker for thyroid vesicles, is a normal constituent of serum. For the last ten years, thyroglobulin has been routinely assayed using a radioimmunologic double antibody technique. In children, normal values are usually under or equal to 30 ng/ml. Neonates have higher levels. The main indications of thyroglobulin assays in pediatrics include diagnosis of metastases of differentiated thyroid epitheliomas and etiologic diagnosis of congenital hypothyroidism. Thyroglobulin assays are less helpful in the other childhood thyroid diseases (goiter, hyperthyroidism).

Lesions of the sacrum are a possible, albeit infrequent, cause of low back pain and sciatica. The purpose of this paper is to draw attention to this possibility which, perhaps due to the rarity of sacral pathologies, is often overlooked in the investigation of lumbosciatic pain. This may influence the prognosis because of the consequent delay in diagnosis. Several cases are reported which were initially operated on for radicular disc syndromes with negative results, and only later correctly diagnosed as sacral lesions.

Some conditions of the tonsils and adenoids often present problems in diagnosis for the surgical pathologist. These include variations of normal histologic features, atypical reactive and metaplastic lesions, hamartomas, papillomas, carcinomas, and some types of lymphomas. The reasons for errors in histopathologic diagnosis have been emphasized, and the clinicopathologic features that can help to prevent misdiagnoses are discussed.

After underlining the frequency of organic lesions observed in functional diseases of vocal cords, emphasis is placed on the importance of the phonetogram in their evaluation and video-laryngo-stroboscopic investigation in their diagnosis. When surgery is necessary, stainless steel instruments are preferred to the laser. Respective isolated or associated vocal reeducational and surgical indications are discussed, and the establishment of a team including laryngologist, phoniatrist and orthophonist recommended to ensure most effective results.

The dexamethasone suppression test (DST) has been used in psychiatric pathology for about 10 years. Carroll et al. consider this test to be specific of endogenous depression.
