Effectiveness of peer-group therapy using community based participatory research model on medication adherence among patients of diabetes and/or hypertension: study protocol for a multi-centre cluster randomised trial in rural settings across India (PARTICIPATE study).

BACKGROUND: According to the WHO, more than 50% of patients do not take their medications as prescribed. Nonadherence is caused by several variables, including social and economic factors, issues with the healthcare team and system, problems related to specific diseases, characteristics of those diseases, and their treatments. OBJECTIVE: To study the effectiveness of the PARTICIPATE intervention at the community level regarding medication adherence among adults with diabetes and/or hypertension in rural India. STUDY DESIGN: This is a multicentre cluster randomised trial implemented across 4

Patient-reported outcomes from the phase 3, randomized, double-blind, placebo-controlled ENGOT-cx11/GOG-3047/KEYNOTE-A18 study of pembrolizumab plus concurrent chemoradiotherapy in participants with high-risk locally advanced cervical cancer.

OBJECTIVE: In ENGOT-cx11/GOG-3047/KEYNOTE-A18 (NCT04221945), pembrolizumab (vs placebo) + concurrent chemoradiotherapy (CCRT) followed by pembrolizumab (vs placebo) significantly improved progression-free survival and overall survival in participants with newly diagnosed, high-risk locally advanced cervical cancer (LACC). We report patient-reported outcomes (PROs) from the study. METHODS: Participants (≥18 years) with high-risk LACC (FIGO 2014 stage IB2-IIB with node-positive disease or stage III-IVA) were randomized 1:1 to 5 cycles of pembrolizumab 200 mg or placebo Q3W plus CCRT, followed b

Shared decision making and patients satisfaction with strabismus care-a pilot study.

BACKGROUND: Strabismus is a complex disease that has various treatment approaches each with its own advantages and drawbacks. In this context, shared decisions making (SDM) is a communication process with the provider sharing all the relevant treatment alternatives, all the benefits, and risks of each procedure, while the patient shares all the preferences and values regarding his/her choices. In that way, SDM is a bidirectional process that goes beyond the typical informed consent. Therefore, it is known a little of the extent to which SDM influences the satisfaction with the treatment outcom

Interventions to improve safe and effective medicines use by consumers: an overview of systematic reviews.

BACKGROUND: Many systematic reviews exist on interventions to improve safe and effective medicines use by consumers, but research is distributed across diseases, populations and settings. The scope and focus of such reviews also vary widely, creating challenges for decision-makers seeking to inform decisions by using the evidence on consumers' medicines use. This is an update of a 2011 overview of systematic reviews, which synthesises the evidence, irrespective of disease, medicine type, population or setting, on the effectiveness of interventions to improve consumers' medicines use. Patient satisfaction with treatment of breast cancer: does surgeon specialization matter?

PURPOSE: Experience and practice setting vary greatly among surgeons who treat breast cancer patients. However, less is known about how these factors influence patient satisfaction with their care. PATIENTS AND METHODS: We surveyed all ductal carcinoma in situ patients and a 20% random sample of invasive breast cancer patients diagnosed in 2002 reported to the Detroit, MI, and Los Angeles, CA, Surveillance, Epidemiology, and End Results registries. Attending surgeons were surveyed, yielding dyad information for 64.6% of patients (n = 1,539) and 69.7% of surgeons (n = 318). Logistic regression

Living without labels: the interactional management of diagnostic uncertainty in the genetic counselling clinic.

In the genetic counselling setting it is not uncommon for a client and genetic counsellor to be faced with uncertainty surrounding the exact diagnostic label to be assigned to the client's symptoms. The huge range of possible conditions/syndromes and the lack of definitive evidence available often combine to create diagnostic uncertainty or non-diagnosis (the assessment given where a genetic diagnosis of the client's symptoms cannot be confirmed at that point). This paper aims to explore the interactional management and negotiation of diagnostic uncertainty in the genetic counselling clinic in

The decision-making process for the treatment of abnormal uterine bleeding.

This pilot study was conducted to investigate the treatment decision-making process of patients and physicians for abnormal uterine bleeding … rnative treatments produced significant improvement in symptom intensity and functioning. Increased patient participation in decision making enhanced patient satisfaction. These findings suggest that hysterectomy rates may be decreased by offering women alternative treatments and by finding ways to increase women's participation in their treatment decisions.

Laying the Foundations for Co-Design: A Micro-Community Engagement Approach to Patient and Public Involvement in Rural Health Research.

BACKGROUND: Rural communities face barriers to accessing, sharing and trusting health information, which can limit the relevance and impact of health education initiatives. Early engagement to understand local communication networks is critical for designing effective, community-grounded co-design processes. OBJECTIVE: To explore, with community members, how health information is accessed, shared and trusted within rural towns to inform a subsequent co-design process. DESIGN: Formative participatory engagement study using qualitative methods, including participatory mapping, semi-structured

Who are we missing? Area deprivation and survey participation.

This study investigates selective participation by area deprivation in two population-based, nationally-representative surveys of children aged 5-16 years in England. We analysed the English participants (N = 22,903) in the British Child and Adolescent Mental Health Surveys, conducted in 1999 and 2004. The surveys recruited parents, teachers and children, allowing us to calculate participation rates for all three types of informant, with particularly detailed analyses possible for parents. Area-deprivation was assigned using Indices of Multiple Deprivation, 2004. We found that parents, teacher

The patient's voice: an exploratory study of the impact of a group self-management support program.

BACKGROUND: Given the potential value of self-management support programs for people with chronic diseases, it is vital to understand how they influence participants' health attitudes and behaviours. The Stanford Chronic Disease Self-Management Program (CDSMP), the most well-known and widely studied such program, is funded in many provinces and jurisdictions throughout Canada. However, there is little published evidence on its impact in the Canadian health-care system. We studied participants' reactions and perceived impacts of attending the Stanford program in one Ontario health region so we

The impact of learning about financial compensation on enrollment in Alzheimer's disease longitudinal research.

INTRODUCTION: Recruitment and retention remains a major challenge in Alzheimer's disease research. This study examined the impact of describing potential financial compensation during recruitment on participant enrollment to a longitudinal cohort. METHODS: Participant recruitment calls (N = 337) were randomized to either a compensation-mentioned group (n = 170) or a control group (n = 167). An intention-to-treat logistic regression assessed the effect of compensation on enrollment. RESULTS: Of 320 analyzed, 124 (38.75%) enrolled. The intervention group's consent rate was lower than the contr

"Helpful in Thinking Through the Question": Redesigning the Patient Health Questionnaire-8 for Depression Assessment to Enhance Patient Engagement and Understanding.

BACKGROUND: Major depressive disorder (MDD) is a prevalent and impairing mental health condition characterized by persistent low mood and diminished energy. Self-report instruments, such as the Patient Health Questionnaire-8 (PHQ-8), are frequently used in clinical and research settings for depression assessment. OBJECTIVE: We developed, redesigned, and tested 2 alternative versions of the PHQ-8 that incorporate visual elements and reframed statements aimed at improving acceptability and comprehension. METHODS: In a mixed methods study, 20 participants with high subclinical to moderately sev

Minimizing nocebo side effects in health care: The unrecognized role of patient involvement.

OBJECTIVE: Nocebo side effects occur when patient expectations worsen negative psychological and physiological responses to medical treatments. This phenomenon can significantly impact patient experiences and outcomes across a broad range of health conditions and treatment types. Interventions are necessary to help patients avoid and minimize nocebo side effects, and in this paper, we explore a new direction for such interventions. DISCUSSION: Current nocebo reduction interventions, such as positive side effect framing, primarily rely on healthcare providers intervening before or during a par
