Expert consensus on the diagnosis and treatment of cemental tear.

Cemental tear is a rare and indetectable condition unless obvious clinical signs present with the involvement of surrounding periodontal and periapical tissues. Due to its clinical manifestations similar to common dental issues, such as vertical root fracture, primary endodontic diseases, and periodontal diseases, as well as the low awareness of cemental tear for clinicians, misdiagnosis often occurs. The critical principle for cemental tear treatment is to remove torn fragments, and overlooking fragments leads to futile therapy, which could deteriorate the conditions of the affected teeth. Th

Analysis and Assessment of Pulp Vitality of 102 Intrinsically Stained Teeth in Dogs.

Our prospective study analyzed clinical, radiographic, and histological characteristics of 102 intrinsically stained teeth. Sixty-nine dogs ranging from one to fifteen years of age were included in this study. Little more than half of the intrinsically stained teeth had no evidence of coronal injury (53.9%, 55/102). We found that most intrinsically stained teeth were histologically nonvital (87.6%, 85/97) and approximately 2/3 of these (57.7%, 56/97) had no histological endodontic or periodontal inflammation at the time of evaluation. Radiographic evidence of endodontic disease was present in

Management of traumatically intruded young permanent tooth with 40-month follow-up.

BACKGROUND: Intrusive luxation injuries often result in severe damage to the tooth, periodontal ligament and pulpal tissue. Furthermore, treatment outcome is often unpredictable because of the large number of injury related variables which influence choice of treatment and prognosis. This report presents the case of a 9-year-old boy with a 6 mm intruded permanent maxillary central incisor with an open apex. METHODS: The treatment option chosen was to wait and watch for 'spontaneous re-eruption'. At the end of two weeks, the tooth showed signs of pulp necrosis and external root resorption. A p

Subepithelial connective tissue graft associated with apicoectomy and root-end fillings in the treatment of deep localized gingival recession with apex root exposure: case report.

Periodontal reconstructive surgery procedures seek to correct mucogingival defects, including gingival recession. This case report describes the use of a subepithelial connective tissue graft (SCTG) associated with root-end fillings using mineral trioxide aggregate (MTA) for the treatment of Miller Class II recession with root apex exposure. A partial-thickness double pedicle flap was made, followed by root preparation with curette and bur finishing. The exposed root apex was removed and the canal was filled with MTA. An SCTG taken from the palate was placed over the root surface and covered w

Peripheral odontogenic keratocyst.

BACKGROUND: An odontogenic keratocyst can develop at virtually any site in the jaws and is of concern because of its aggressive clinical behavior. It represents 3% to 12% of all odontogenic cysts. This paper describes the rare peripheral presentation of an odontogenic keratocyst localized to the maxillary anterior gingiva and its differential diagnosis. METHODS: A patient presented with a round yellow nodule on the maxillary gingiva between the left canine and first premolar. Clinical examination ruled out periapical abscess, periodontal abscess, and lateral periodontal cyst. A differential d

Saving Single-rooted Teeth with Combined Endodontic-periodontal Lesions.

INTRODUCTION: Teeth affected by combined endodontic-periodontal lesions are usually considered by all prognosis classifications as hopeless teeth. The development of new biomaterials combined with modern endodontic and periodontal regeneration techniques may improve dental prognosis and maintain the affected teeth. Moreover, 1 of the replacement options for those teeth, dental implants, has shown an increasing number of biological and technical complications. METHODS: Five patients were included in this case series study. Full periodontal and radiographic examination revealed generalized chro

Association between Apical Periodontitis and the Severity of Marginal Periodontitis in Root Canal-treated Teeth: A Retrospective Cohort Study.

INTRODUCTION: To evaluate whether the severity of marginal periodontitis is correlated with the development of apical periodontitis (AP) in root canal-treated teeth. The secondary aim was to investigate the variables associated with the development of AP in patients with different levels of bone loss. METHODS: This longitudinal, observational, retrospective cohort study entailed radiographic evaluation of root-canal-filled teeth in patients with and without periodontal involvement after a minimum period of 2 years. Independent, calibrated specialists evaluated the baseline periapical radiogra

Imaging the past: Dental pathologies and cardiovascular disease in Egyptian mummified remains.

BACKGROUND: Imaging advancements markedly enhance the assessment of skeletal and dental pathologies in clinical and archaeological contexts. Among the most frequently observed dental conditions in bioarcheological studies are periodontal disease and caries, both of which can provide valuable insights into systemic conditions (eg, cardiovascular disease). When identified in mummified remains, dental pathologies offer a unique perspective on the health status and disease burden of past populations and the interplay between oral and systemic health across human history. METHODS: Using a database

Evaluation of two low-level laser techniques as an adjunct to basic periodontal therapy: a randomized clinical trial.

To assess and compare two techniques of low-level laser application-transgingival (TLLLT) and intrasulcular (ILLLT)-used in photobiomodulation as an adjunct to basic periodontal therapy (BPT) in patients with periodontitis. A randomized, split-mouth, double-blind clinical trial was conducted, selecting three diseased periodontal sites from different quadrants in each patient. These sites were assigned to one of three treatment groups: SRP (control), SRP + TLLLT (test 1), and SRP + ILLLT (test 2). Low-level laser therapy in the test groups was applied at 48 h, 7 days, and 14 days after full-mou

Intentional Replantation - Small procedure, big benefits: A report of five cases.

Intentional replantation (IR) is a conservative endodontic procedure that may offer predictable results when conventional non-surgical endodontic treatment (NSET) or retreatment fails. The aim of the present report was to describe the management and outcomes of five cases of persistent apical periodontitis treated successfully with IR after failed NSET or retreatment. All the cases underwent IR involving atraumatic extraction, 3 mm root-end resection, retrograde cavity preparation, and sealing with intermediate restorative material (IRM), followed by replantation and stabilization with flexibl

[Historical evolution and current research of the odontogenic focal theory].

Proposed in the early 20th century, the focal infection theory hypothesizes that teeth with dental caries, endodontic-periapical/periodontal diseases, plus impacted teeth and residual roots, might act as infection foci, inducing pathologies in distant organs including respiratory tract, gastrointestinal tract and brain, etc. During the prevalence of this theory, tooth extraction was well received as a key adjuvant therapy for systemic diseases, profoundly influencing paradigms of dentistry and clinical medicine. In the mid-20th century, advances in evidence-based medicine and diagnostic/therap

Radiographic features of vertically fractured endodontically treated mesial roots of mandibular molars.

OBJECTIVE: The purpose of this study was to evaluate the most frequent radiographic appearance of bony lesions around endodontically treated vertically fractured mesial roots of mandibular molars. STUDY DESIGN: For the study, 49 extracted mesial roots with vertical fractures (study group) were evaluated and compared to 52 extracted roots without fractures (control). RESULTS: The "halo" (36.7%) and "periodontal" (28.6%) type radiolucencies were the most typical appearances of periradicular areas around the mesial roots of mandibular molars with vertical root fractures. By itself, bifurcation

Endodontic-periodontal locally delivered antibiotics.

Endodontic pathology is a bacterial disease. It is well established that periapical disease is the result of bacteria, their product, and the host response to them. Periradicular disease will occur after microorganisms and their metabolic products affect the periradicular tissue. Aim of using antibiotics as part of a treatment regimen is to achieve, within the periodontal environment, a concentration of the drug that is sufficient either to kill (bactericidal) or arrest the growth (bacteriostatic) of pathogenic microorganisms. There are two possible approaches to improve the drug action: susta

Natural killer cells and alterations in collagen density: signs of periradicular herpesvirus infection?

This study evaluated the presence and density of natural killer (NK) cells as well as collagen density in chronic apical periodontitis lesions and tried to find any correlations with concomitant herpesvirus infection or histopathological status of the lesion. Surgical specimens of chronic apical periodontitis lesions were surveyed for the presence and density of NK cells by immunohistochemical analysis. Collagen density in these lesions was quantified by means of histochemistry. All specimens were positive for the presence of CD57-positive cells. Topographically, CD57-positive cells were found
