Understanding the experience of reconstructive treatments from the perspective of people who suffer from facial lipoatrophy: a qualitative study.

BACKGROUND: Facial lipoatrophy has been described as the most distressing and stigmatizing expression of the lipodystrophy syndrome, a syndrome that is caused by antiretroviral combination therapy. In recent years, reconstructive treatments (such as poly-l-lactic acid and polyalkylimide) have been increasingly considered for this condition. These treatments allow for facial contours and facial fullness to be restored while being minimally invasive. OBJECTIVE: The main objective of this qualitative research was to explore and describe the experience of people who suffer from facial lipoatrophy

[Problem cases of metatarsalgia].

Metatarsalgia is characterized by pain under the metatarsal heads and is a very common cause of foot pain among adults. Symptoms can be isolated or in combination with accompanying deformities occurring in the forefoot and/or hindfoot. In the foreground of the treatment is the exhaustion of conservative forms of therapy to minimize the symptoms of local pressure increase and callus under the metatarsal heads. In addition, various surgical methods are available, such as corrective osteotomy of the metatarsal bone, soft tissue interventions and the correction of associated deformities. The indic

Use of hair grafting in scar camouflage.

Loss of hair-bearing tissue in the head and neck area can result from surgery, trauma, burns, tumors, and infection, as well as a diversity of inflammatory conditions, and the resulting defect can present a challenging problem for the reconstructive surgeon. Hair transplantation can be used as a reconstructive method alone or in conjunction with other techniques. The current method of using follicular unit grafts has led to natural restorations for a variety of areas including not only the scalp but also eyebrows, eyelashes, and beard areas. Camouflage provided by hair grafts can provide resto

Reconstruction of the midface and maxilla.

The maxilla and midface have an important aesthetic and functional role. Their reconstruction after surgical ablation is a complex problem. The historical development of reconstruction of the midface, classification systems of maxillary defects, and descriptions of reconstructive options are presented.

[Lacrimal system lacerations and their surgical repair].

In lacrimal system lacerations the canaliculi are involved in 70% of cases and the lacrimal sac and/or nasolacrimal duct in 30%. Lacrimal system lacerations can be the result of sharp or blunt trauma. Nasolacrimal ducts may become obstructed by indirect trauma as an aftereffect of naso-orbital fractures. The epidemiology, main principles of surgical repair of canaliculi and of lacrimal sacs and/or nasolacrimal ducts, special techniques of surgical reconstruction and approximate success rates are reviewed with respect to the outcome of our patient collective compared to the literature. Canalicu

Strategies in lip reconstruction.

Injury or surgical trauma can result in significant alterations of normal lip appearance and function that can profoundly impact the patient's self-image and quality of life. Neuromuscular injury can lead to asymmetry at rest and during facial animation, and distressing functional disabilities are common. Loss of labial competence may interfere with the ability to articulate, whistle, suck, kiss, and contain salivary secretions. For smaller defects, reconstruction can be very effective. Reconstructing an aesthetically pleasing and functional lip is more difficult with larger defects.

Surgical correction of gynecomastia.

Gynecomastia, or persistent enlargement of the male breast, is a common occurrence during early sexual development. Typically, two types of tissue can be identified during this period of growth. Initially, a dense fibrous growth of supporting stroma and breast tissue forms directly under the areola, creating a prominent mass effect. A supporting fibrofatty stroma variably develops around the periphery of the breast, particularly as the body mass index of the patient increases. Treatment is directed at both of these tissue types and includes liposuction contouring along with direct excision of

Reconstruction of microtia.

The reconstruction of microtia continues to represent one of the more challenging plastic surgery procedures. The limitations of the reconstruction are partly inherent in the soft-tissue deficiencies present in increasing degrees from large conchal remnant microtia, to lobular microtia, to the displaced remnant in auricular dystopia, and partly because of the high level of technical expertise required. After a brief review of the history of ear reconstruction in general and microtia specifically, issues related to the transition in the popularity from techniques involving three or more stages …

[Anatomic and histological characteristics of vagina reconstructed by McIndoe method].

BACKGROUND/AIM: Congenital absence of vagina is known from ancient times of Greek. According to the literature data, incidence is 1/4 000 to 1/20 000. Treatment of this anomaly includes non-operative and operative procedures. McIndoe procedure uses split skin graft by Thiersch. The aim of this study was to establish anatomic and histological characteristics of vagina reconstructed by McIndoe method in Mayer Küster-Rockitansky Hauser (MKRH) syndrome and compare them with normal vagina. METHODS: The study included 21 patients of 18 and more years with congenital anomaly known as aplasio vaginae

[Reconstructive operations for scarry contractures of the shoulder joint after burns].

The article presents two methods of formation of the axilla cupola with saved skin from the area of hair growth, and when there are scarry changes. The first method allows exclusion of displacement of the axilla skin into not proper places. The second method gives better engraftment and better outcomes of operation. A method of two-stage treatment was worked out for severe contractures of the shoulder joint in order to avoid a transection of the tendons and muscles and overdistention of the vascular-nervous bundles. During the preoperative period a gradual liberation of the shoulder is to be f

[Fournier's gangrene and reconstruction of its defects].

Fournier's gangrene is remarkebly rare but severe, life threatening condition. Thanks to better earlz recongnition of disease in past few years, the percentage of patients which survive this condition is considerable, but consequence of this fact is large number of patients with defects of cutis and subcutis in perineal and scrotal reigions. Reconstruction of this defects is cimplicate and require helathy, well vascularised tissue from another anatomical region which is not involved in gangreneous process. Authors present two cases of reconstruciton of defects in perineal and scrotal regions.

[The surgical treatment of severely injured vagina].

OBJECTIVE: To investigate the surgical methods of treating severely injured vagina. METHODS: A retrospective study was carried out on the clinical materials and results of 7 patients suffered from severe vaginal injury. RESULTS: Different surgical approaches such as trans-suprapubic, transperineal or both approaches were applied according to the position of the atresia, meanwhile, different surgical methods such as skin graft, free flap transplantation, vaginal mucous flap advancement, direct anastomosis of the two ends, "Z" plasty was used to repair the vagina according to the different def

A technique for cleft palate repair.

The author has developed a technique of palate repair that combines minimal hard palate dissection with radical retropositioning of the velar musculature and tensor tenotomy. The repair is performed under the operating microscope. Results are reported for 442 primary palate repairs performed between 1978 and 1992 inclusive, with follow-up of at least 10 years. In 80 percent of these palate repairs, repair was carried out through incisions at the margins of the cleft and without any mucoperiosteal flap elevation or lateral incisions. Secondary velopharyngeal rates have decreased from 10.2 to 4.
