From 1995 to 1997, we prospectively evaluated the prevalence of hepatitis C virus (HCV) RNA in 124 patients with porphyria cutanea tarda (PCT) from Northern France (83 sporadic and 41 familial PCT). Serum samples were analyzed for ferritin, transaminases, HCV antibodies, and HCV RNA. In addition, genotyping of HCV and searches for HCV infection risk factors (blood transfusion, iv drug abuse, and surgical intervention) were performed. We report two new cases of HIV-1 seropositive subjects who developed porphyria cutanea tarda (PCT). Laboratory examinations revealed a rise of uroporphyrin in the 24-hour urine and a fall of CD4 lymphocytes below 200/mm3 reflecting severe immuno-depression. Treatment with iterative phlebotomy resulted in regression of PCT. In both patients HIV seropositivity was discovered before PCT was diagnosed. Up to now, none of the hypotheses put forward concerning the exact role played by HIV in the genesis of PCT has been confirmed.

BACKGROUND: Porphyria cutanea tarda (PCT) and similar vesiculobullous disorders have been described with increased frequency in patients with chronic renal failure undergoing long-term hemodialysis. Haemodialysis-related porphyria cutanea tarda is a rare, but serious and mutilating skin condition, resulting from extremely high plasma porphyrin levels because of their inadequate clearance by haemodialysis. The treatment is very difficult as chloroquine is ineffective and venesection, the conventional treatment of this disease, is not always an option because of anaemia of end-stage renal disease. We report a case of haemodialysis-related porphyria cutanea tarda and her successful management by recombinant human erythropoietin treatment.

Porphyria cutanea tarda in human beings is believed to be due to reduced hepatic uroporphyrinogen decarboxylase activity. However, extrinsic factors such as alcohol abuse and drug intake are required for clinical manifestation of the disease. A case is reported of acquired porphyria cutanea tarda (PCT) due to administered valproate acid in an epileptic patient. Sonography (US) and computed tomography (CT) showed multiple nodular, well-defined lesions of the liver. Biopsy confirmed the diagnosis of focal fatty infiltration and revealed additionally the signs of hepatic involvement in PCT. Focal fatty infiltration should be considered in patients with multiple liver lesions if there is a predisposing condition. PCT can be a further cause of this type of infiltration.

The clinical value of a very rapid spectrofluorometric method for the determination of plasma uroporphyrin levels was studied during follow-up of 122 patients with porphyria cutanea tarda (PCT). Four-hundred and eight measurements were carried out within a 3-year period. In active PCT plasma uroporphyrin varied between 15 and 448 nmol/l (normal 0-1.4). A high correlation was seen between elevated levels of uroporphyrin in the plasma and urine in new cases (r = 0.72, n = 23) and relapses (r = 0.92, n = 37). A parallel course between these variables was noted during the treatment of 31 patients.

The HLA-A3 alloantigen, usually associated with the hemochromatosis gene, was detected in 12 of 18 patients (67%) with sporadic porphyria cutanea tarda but in only 23 per cent of 328 normal subjects (p = 0.0006). This difference remained significant after correcting for the number of HLA-A locus antigens tested (p = 0.025). These results suggest that an HLA-linked hemochromatosis allele is present and may account for the iron abnormalities in many patients with sporadic porphyria cutanea tarda.
