Comparing adverse events of tenecteplase and alteplase: a real-world analysis of the FDA adverse event reporting system (FAERS).

OBJECTIVES: The aim of this study is to monitor, identify, and compare the adverse events (AEs) related to tenecteplase and alteplase, with the objective of exploring the potential safety of tenecteplase for acute ischemic stroke (AIS) and guiding its use to enhance patient safety. METHODS: In order to evaluate the disproportionality of AEs associated with tenecteplase and alteplase in real-world data, four algorithms (ROR, PRR, BCPNN, EBGM) were utilized as measures to detect signals of AEs related to both drugs. Subsequently, Breslow-Day statistical analysis was applied to compare the RORs

Multicomponent Support Program for Secondary Prevention of Stroke Using Digital Health Technology: Co-Design Study With People Living With Stroke or Transient Ischemic Attack.

BACKGROUND: Few individuals (<2%) who experience a stroke or transient ischemic attack (TIA) participate in secondary prevention lifestyle programs. Novel approaches that leverage digital health technology may provide a viable alternative to traditional interventions that support secondary prevention in people living with stroke or TIA. To be successful, these strategies should focus on user needs and preferences and be acceptable to clinicians and people living with stroke or TIA. OBJECTIVE: This study aims to co-design, with people with lived experience of stroke or TIA (referred to as cons

Follow-Up Care after Adult Lung Transplantation: Summary of an Evidence-Based Clinical Practice Guideline for German-Speaking Countries.

Lung transplantation (LTx) is a life-saving intervention for end-stage lung disease, yet long-term outcomes remain inferior compared to other solid organ transplants. No evidence-based national or international guidelines for post-transplant follow-up care existed prior to this work. This guideline aims to provide treating physicians with structured, evidence-based recommendations for follow-up care after LTx in adult patients. The guideline was developed with a multidisciplinary guideline group according to the guidance of the Association of the Scientific Medical Societies in Germany manual

Practice Variation in Urine Collection Among Emergency Department Providers in Pre-toilet-trained Children With Suspected Urinary Tract Infection.

BACKGROUND: Urinary tract infections are a common cause of acute illness among children presenting to the emergency department (ED). Many techniques exist to collect urine specimens in pre-toilet-trained children. There is wide practice variation regarding the most appropriate collection method. This variation also appears to exist across national health organizations and societies. To date, little is known about the extent of practice variation in urine collection methods or the influence of patient and health care provider characteristics on the choice of collection method. MATERIALS AND ME

From losing control to regaining control: Patient experiences of care for Staphylococcus aureus bacteremia.

OBJECTIVES: Recommendations for appropriate Staphylococcus aureus bacteremia (SAB) care almost exclusively focus on preventing disease-specific outcomes like metastatic infection, relapse and mortality, rather than providing practical information on how to achieve person-centered care (PCC). We described characteristics and features of SAB care from a patient perspective and explored experiences regarding this care to formulate concrete recommendations for improving PCC in SAB patients. METHODS: One-time, in-depth, semi-structured, qualitative interviews with SAB patients ∼12 weeks post-SAB-d

GeneXpert or chest-X-ray or tuberculin skin testing for household contact assessment (GXT): protocol for a cluster-randomized trial.

BACKGROUND: The World Health Organization recommends tuberculosis (TB) preventive treatment (TPT) for all people living with HIV (PLH) and household contacts (HHC) of index TB patients. Tests for TB infection (TBI) or to rule out TB disease (TBD) are preferred, but if not available, this should not be a barrier if access to these tests is limited for high-risk people, such as PLH and HHC under 5 years old. There is equipoise on the need for these tests in different risk populations, especially HHC aged over 5. METHODS: This superiority cluster-randomized multicenter trial with three arms of e

Thresholds for decision-making: informing the cost-effectiveness and affordability of rotavirus vaccines in Malaysia.

Cost-effectiveness thresholds (CETs) based on the Commission on Macroeconomics and Health (CMH) are extensively used in low- and middle-income countries (LMICs) lacking locally defined CETs. These thresholds were originally intended for global and regional prioritization, and do not reflect local context or affordability at the national level, so their value for informing resource allocation decisions has been questioned. Using these thresholds, rotavirus vaccines are widely regarded as cost-effective interventions in LMICs. However, high vaccine prices remain a barrier towards vaccine introdu

Evidence and the Politics of Deimplementation: The Rise and Decline of the "Counseling and Testing" Paradigm for HIV Prevention at the US Centers for Disease Control and Prevention.

POLICY POINTS: In situations of scientific uncertainty, public health interventions, such as counseling for HIV infection, sometimes must be implemented before obtaining evidence of efficacy. The history of HIV counseling and testing, which served as the cornerstone of HIV prevention efforts at the US Centers for Disease Control and Prevention (CDC) for a quarter of a century, illustrates the influence of institutional resistance on public health decision making and the challenge of de-implementing well-established programs. CONTEXT: In 1985, amid uncertainty about the accuracy of the new tes

Whole-Brain Neural Connectivity to Cholinergic Neurons in the Lower Thoracic Intermediolateral Column.

OBJECTIVE: The intermediolateral column (IML) serves as a crucial hub for sympathetic information processing between the brain and peripheral organs, with its defining hallmark being the presence of cholinergic neurons expressing choline acetyltransferase (ChAT). Specifically, the IML of the lower thoracic cord plays a pivotal role in regulating abdominal metabolic and digestive viscera. However, little is known about the whole-brain neural connectivity targeting these lower thoracic IML cholinergic neurons, necessitating further investigation. METHODS: Specific retrograde tracing virus was i
