The post-Q-fever fatigue syndrome (QFS) (inappropriate fatigue, myalgia and arthralgia, night sweats, changes in mood and sleep patterns) follows about 20% of laboratory-proven, acute primary Q-fever cases. We report a case of Coxiella burnetii endocarditis in a 42-year old man presenting with a long-known cardiac murmur and an infectious syndrome of several months duration. The aetiological diagnosis, delayed by the lack of knowledge of a primary Q fever, was established by serology. Неврологическое участие в Q Fever является необычным. Мы представляем случай энцефалита из-за Coxiella Burnetii с нейрорадиологическими результатами на КТ, которые не были описаны ранее и которые состоят из районов с пониженным коэффициентом поглощения в субкортическом белом веществе обоих полушарий, преимущественно справа. Clinical, immunological and epidemiological features of 14 human cases of Q fever diagnosed at Aurora Hospital are presented. All patients had an acute febrile disease and 9 (64%) had respiratory symptoms, 4 (29%) verified pneumonia, and 9 (64%) hepatitis, which in 4 biopsied cases proved to be granulomatous. Twenty-three patients with Q fever who were diagnosed over a 3 year period are described. The majority came from the Madrid urban area and less than half had epidemiological antecedents. Nine patients presented with pulmonary infiltrations, 12 with suppressed fever and in 2 criteria for fever of unknown origin were met. A 6-year-old boy with a congenital bicuspid aortic valve presented with finger clubbing and hypertrophic osteoarthopathy, and subsequently he developed severe hypertension. The hypertension was successfully treated by nephrectomy, at which a thrombosed mycotic aneurysm of the renal artey was found. Echocardiography showed the presence of aortic valve vegetations. Blood cultures were sterile, but high antibody titres to the phase 1 and 2 antigens of Coxiella burneti strongly suggested Q fever infection. We believe this is the first reported case of Q fever endocarditis in early childhood.

The IgM globulin response has been measured following the onset of primary Q fever infections. A complement fixation test method was used to measure the IgM after separation by ultracentrifugation in a sucrose density gradient. Specific IgM was demonstrable between 2 and 10 wk after onset of symptoms. The test can be used to confirm a recent infection when only a 'convalescent' blood specimen has been collected and will be useful in the assessment of workers' compensation claims.
