Esophageal hamartoma is a rare entity, as only 7 cases have been previously reported in the English-language literature. Common symptoms include dysphagia, weight loss, and vomiting. Life-threatening airway obstruction can also occur. Because of the nonspecific nature of the symptoms, patients with these dangerous polyps are often misdiagnosed as having more common entities such as gastroesophageal reflux, peptic ulcer disease, or achalasia. The case of a 6-year-old female who developed hypovolemic shock from mushroom poisoning is discussed. The mushroom ingested, Chlorophyllum molybdites, is considered to be a benign gastrointestinal irritant. This case emphasizes that the gastrointestinal irritant mushrooms can produce severe toxicity and that pediatric patients have a limited capacity for fluid loss before becoming hypovolemic.

Several histamine H2 receptor antagonists and the H+,K(+)-ATPase inhibitor, omeprazole, have been shown to be capable of controlling gastric acid secretion safely and effectively in patients with Zollinger-Ellison syndrome. The relative merits of these agents are discussed, and their use in the acute and long-term control of acid hypersecretion and in special circumstances that require particular care are described. The surgical approaches to the control of acid secretion are described, and the current place of surgery in the management of acid hypersecretion is discussed.

While alkali ingestion nearly always injures the esophagus, acid usually spares the esophagus and damages the gastric outlet. Most reported cases of acid gastric injury have required resection. We report a case of acid ingestion managed with parenteral nutrition and histamine blockers in which healing is documented without operation.

Bismuth compounds remain useful for Helicobacter pylori eradication therapy. These include colloidal bismuth subcitrate (CBS), bismuth subsalicylate (BSS) and, most recently, ranitidine bismuth citrate (RBC).
