Outcomes of referrals from endodontic to orofacial pain specialists: A retrospective cohort study.

OBJECTIVES: Diagnosis and treatment of non-odontogenic pain is challenging for endodontists. The purpose of the study was to investigate the outcomes of referrals to orofacial pain specialists made for patients with suspected non-odontogenic pain, after evaluation and/or treatment by an endodontist. MATERIALS AND METHODS: A retrospective review of dental records was conducted for 60 patients referred from a postgraduate endodontic clinic to an orofacial pain clinic. Patient demographics, pain history, endodontic, and orofacial pain diagnoses were collected. Number of visits, length of treatme

[Range of symptoms after contact with the clinging jellyfish (Gonionemus vertens)].

Ten patients showed to the general practitioner with complaints of dyspnea and/or heavy generalised pain after contact with the clinging jellyfish (Gonionemus vertens) after swimming in a local lake. Four patients were referred to the emergency department for additional treatment. Available literature describes local treatment with seawater of a compress with lukewarm water. In the cases described above, additional treatment was needed.Because of the lack of available literature we decided to aim on treating the symptoms. One patient was seen afterward with neuropathic pain. Taking in consider

Familial Mediterranean fever: a differential diagnosis for the surgical abdomen.

FMF is characterised by dysregulation of the inflammatory process in the body, presenting as recurrent episodes of serositis. Patients with FMF commonly present with episodes of fever, peritonitis, synovitis, pleuritis, arthritis, and occasionally pericarditis. We present a case of a young 19-year old female, who presented to hospital four times over several months with intermittent fevers, abdominal pain and pleuritic chest pain. After being initially admitted under the surgical team, she was reviewed by the medical team who referred her for genetic testing, which subsequently confirmed the d

Intraosseous Medullary Fat Necrosis on 99mTc-MDP Bone Scan of Patient With Acute Pancreatitis.

Pancreatic disease can be associated with polyarthritis and panniculitis in a small number of patients, and this triad constitutes PPP syndrome. Early diagnosis is critical as it has high morbidity and mortality. Panniculitis can occur in fat present anywhere in the body. Involvement of fat in bone marrow is relatively uncommon, and radiologic imaging shows osteolytic lesions involving long bones. Here we present a case of acute pancreatitis, referred to our department for evaluation of severe joint pain and multiple bone pain. Tc-MDP scan with SPECT/CT has been done, which showed medullary ex

Eagle's syndrome: signs and symptoms.

Eagle's syndrome, an uncommon sequela of an elongated styloid process, can manifest itself as a sensation of a foreign body in the throat and a retrogoniac or anterolateral neck pain often referred to the TMJ and the ear. When treating patients affected by temporomandibular disorders (TMD), complaining of atypical orofacial pain, and with a concomitant radiographic finding of a bilaterally elongated styloid, the possible coexistence of Eagle's syndrome should always be considered. The aim of this review is to present an overview of the epidemiology, pathophysiology, diagnosis, and therapy for

Sodium tetradecyl sulphate direct intralesional sclerotherapy of venous malformations of the vulva and vagina: report of five cases.

We report five cases of female patients affected by symptomatic focal external genital venous malformations treated with percutaneous direct intralesional injection of sodium tetradecyl sulphate (STS). All patients were referred because of discomfort and pain when sexual intercourse was attempted. Direct sclerotherapy with 3% STS was performed on a day-hospital basis with the patient under local anesthesia. Complete resolution of the symptoms was achieved in all cases. No major adverse effects were reported. Direct intralesional sclerotherapy with STS may be considered a safe and effective met

Clues to pathogenesis of fibromyalgia in patients with sickle cell disease.

OBJECTIVE: To investigate the association between sickle cell disease (SCD) and fibromyalgia (FM). METHODS: Nine patients with SCD for whom a rheumatology consult was requested were assessed for FM by retrospective chart review. Eleven inpatients with other forms of anemia referred for rheumatology consult were also assessed for FM. RESULTS: Eight of 9 patients with SCD fulfilled classification criteria for FM compared to one of 11 patients without SCD (p < 0.001). CONCLUSION: Awareness of the high frequency of FM in SCD can improve treatment of sickle cell crisis. Some pain that is labeled

Common carotid artery dissection: a rare cause for cervical pain.

Spontaneous dissections of extracranial carotid arteries occur most frequently in the internal carotid artery. In contrast, common carotid artery dissection (CCA) is a rare cause of cerebral ischaemia with only a few cases having been reported. We present the case of a 59-year-old male patient who was referred to our clinic with left cervical pain. The patient was otherwise asymptomatic. Magnetic resonance imaging (MRI) showed enhancement and double lumen in the left CCA without affecting the carotid bulb, internal carotid artery or aortic arch. We discuss the diagnostic and therapeutic manage

Vertebral fractures as initial signs for acute lymphoblastic leukemia.

Common presenting symptoms of acute lymphoblastic leukemia in children are well known and include pallor, fatigue, and loss of appetite. Limb pain is sometimes described and can be misleading. We describe two recent cases seen in our emergency department, where vertebral fractures, a much rarer finding, were the only presenting symptoms that led to the diagnosis. One case had been thoroughly evaluated only 5 weeks prior to the diagnosis and included magnetic resonance imaging. The second patient was rapidly referred to our center with a history of acute lumbar pain. Emergency physicians caring

Cervicogenic headache: anatomic basis and pathophysiologic mechanisms.

Cervicogenic headache is pain perceived in the head but referred from a primary source in the cervical spine. The physiologic basis for this pain is convergence between trigeminal afferents and afferents from the upper three cervical spinal nerves. The possible sources of cervicogenic headache lie in the structures innervated by the C1 to C3 spinal nerves, and include the upper cervical synovial joints, the upper cervical muscles, the C2-3 disc, the vertebral and internal carotid arteries, and the dura mater of the upper spinal cord and posterior cranial fossa. Experiments in normal volunteers

Glossopharyngeal neuralgia referred from a pontine lesion.

Paroxysmal pain in the form of glossopharyngeal neuralgia is less frequent and less well understood than that of trigeminal neuralgia. Diagnostic confusion can arise especially when both conditions occur in the one patient. We report a patient with a 20-year history of left-sided glossopharyngeal neuralgia with trigger zones in both the trigeminal and glossopharyngeal dermatomal distributions. Magnetic resonance imaging revealed a single T2-weighted hyperintense signal in the left pons with no other abnormality. It is postulated that ephaptic transmission between central pain fibers and the tr

Cervical angina.

Cervical angina is defined as chest pain that resembles true cardiac angina but originates from a cervical discopathy with nerve root compression. This condition, which is also referred to as pseudoangina, most commonly results from compression of the C7 nerve root. Several simple findings from the history and the physical examination help make the diagnosis, which can then be confirmed with magnetic resonance imaging and/or discography. Coexisting coronary artery disease must always be ruled out. Treatment includes intermittent cervical traction, physical therapy, nonsteroidal anti-inflammato

Focal headache during balloon inflation in the vertebral and basilar arteries.

BACKGROUND AND PURPOSE: Headache may be seen in acute cerebrovascular disease. The significance of localized headache in association with vertebrobasilar disease has not been recognized. SUMMARY OF REPORT: We describe a patient with a cerebellar arteriovenous malformation who underwent intravascular balloon occlusion of the vertebral and basilar artery. He developed reproducible patterns of referred pain with balloon inflation at specific sites. CONCLUSIONS: Well localized head pain in the setting of acute stroke should alert physicians to the possibility of localized arterial injury. The pa…

The straight leg raising test: its relevance to possible disc pathology.

The records of 40 patients who had suffered from low back pain, and who had a marked restriction of straight leg raising (SLR) have been studied. Thirty-four of these cases had referred pain down the lower limb. They were all treated, at least in the first place, by manipulation. The response to manipulation demonstrates that, in the absence of evidence of interference with nerve root conduction, limitation of SLR is an unreliable sign of possible disc pathology. Furthermore, the immediate response to manipulation, particularly in the known disc protrusions, strongly suggests that muscle spasm…

Chronic Pain and Distress in the Primary Care Setting: Implementing a Digital Pain and Distress Screener to Optimize Integrated Psychological Treatment.

Chronic pain is common, complex, undertreated, and linked with lower quality of life. The primary care setting provides an opportunity to standardize screening for chronic pain and distress and facilitate early intervention via integrated psychological treatment. This study describes the use of a digital pain and distress screener to improve identification of those who could benefit from integrated psychological treatment to manage chronic pain in primary care. An electronic pain and distress screening tool was implemented in an academic internal medicine clinic at an urban hospital. Data rega
