Central retinal artery occlusion without underlying chronic ocular ischemic syndrome may lead to neovascular glaucoma.

INTRODUCTION: The prevalence and etiology of neovascular glaucoma (NVG) as a complication of central retinal artery occlusion (CRAO) is a debated issue. According to some authors, NVG associated with CRAO always involves underlying chronic ocular ischemic syndrome (COIS) as a primum movens for CRAO. However, we describe 5 cases of NVG following CRAO with no underlying COIS, confirmed by carotid Doppler studies and ultrasound color Doppler imaging (USCDI) of the ophthalmic artery (OA). MATERIAL AND METHODS: We conducted a single-center retrospective analysis of the medical records of 5 consecu…

Asymptomatic carotid artery stenosis and retinal nerve fiber layer thickness. A community-based, observational study.

PURPOSE: To examine whether an abnormally thin retinal nerve fiber layer (RNFL) is associated with cerebrovascular insufficiency. DESIGN: Community-based study. METHODS: The Asymptomatic Polyvascular Abnormalities in Community Study included Chinese aged 40+ years and without histories of cerebrovascular incidents or coronary heart disease. Using transcranial Doppler and carotid duplex ultrasound examination, we assessed presence and degree of an intracranial arterial stenosis (ICAS) and extracranial carotid arterial stenosis (ECAS) and we measured the RNFL thickness by spectral-domain optic

Embolic Central Retinal Artery Occlusion Detected with Point-of-care Ultrasonography in the Emergency Department.

BACKGROUND: Ocular emergencies account for 2-3% of all emergency department (ED) visits. Sonographic evaluation of the eye offers a very use … rmed, and it showed a retrobulbar hyperechoic material, suggestive of an embolus within the central retinal artery. Fluorescein angiography showed limited and sluggish filling of the retinal arteries after injection of fluorescein, and optical coherence tomography demonstrated a decrease in the reflectivity and thickness of the inner retinal layers. The final di …

Central retinal artery occlusion associated with persistent truncus arteriosus and single atrium: a case report.

BACKGROUND: Central retinal artery occlusion (CRAO) is an ocular emergency and most of the cases present with painless sudden persistent loss of vision in the range of counting fingers to perception of light. The presentation of CRAO is associated with a variety of medical conditions. We report a rare case of CRAO associated with persistent truncus arteriosus (PTA) and single atrium in a female patient. CASE PRESENTATION: A 23-year-old woman was admitted due to sudden painless visual loss in the left eye. On examination visual acuity of light-perception was noted in the left eye with a left r…

High correlation of scotopic and photopic electroretinogram components with severity of central retinal artery occlusion.

PURPOSE: The aim of this study was to determine whether a significant correlation exists between the scotopic and photopic components of electroretinograms (ERGs) and the degree of circulation disturbances caused by a central retinal artery occlusion (CRAO). DESIGN: Observational clinical study. PARTICIPANTS: Sixteen patients with a CRAO (16 eyes). METHODS: The circulatory disturbance in eyes with CRAO was graded as mild (group 1) when the arm-to-retina transmission time was <30 sec and severe (group 2) when the arm-to-retina transmission time was ≥30 sec. Scotopic and photopic ERG componen…

[Optical coherence tomography (OCT) in central retinal occlusion with sparing cilioretinal artery--a case report].

The authors present a case of central retinal artery occlusion with sparing cilioretinal artery in 62 year old woman with sudden decrease of visual field in the right eye. The diagnosis of central retinal artery occlusion (ophthalmological examination, fluorescein angiography, field of vision and optical coherent tomography) was performed at the begining and after 1 month. In acute phase, optical coherence tomography reveals increased thickness and reflectivity of the inner retinal layer and decreased reflectivity of the outer retinal layer. After one, three and six months OCT demonstrates a d…

Characteristics of patients presenting with central retinal artery occlusion with and without giant cell arteritis.

BACKGROUND: In patients with concurrent central retinal artery occlusion (CRAO) and giant cell arteritis (GCA), prompt recognition and early treatment are crucial to prevent further visual loss in the contralateral eye. The objective of this study was to describe a series of patients with CRAO resulting from GCA and to compare them with patients with isolated CRAO. METHODS: The patient database for a retina service in Philadelphia was searched to identify patients admitted between 1983 and 1993 with a diagnosis of biopsyproven GCA and CRAO. For every patient with CRAO caused by GCA, the next …

[X-shaped macular hemorrhage disclosing mixed occlusion of the cilioretinal artery and central retinal vein].

A case of a stellate hemorrhage of the macula occurring in a young man is reported. Analysis of this rare clinical entity allowed us to link it to combined occlusion of the central retinal vein and cilioretinal artery. With regard to this case and to a review of the literature, clinical and angiofluorographic features of this syndrome are recalled. The usually good prognosis of this disease was not observed in our case. Finally, the controversial pathophysiological data are discussed.

Central retinal artery occlusion in a patient with Fabry's disease documented by scanning laser ophthalmoscopy.

Fabry's disease, angiokeratoma corporis diffusum, is an X-linked inborn error of glycosphingolipid metabolism due to lack of activity of the lysosomal enzyme, alpha-galactosidase A, resulting in progressive intracellular deposition of neutral glycosphingolipids in various tissues, including vascular endothelial - and smooth muscle cells. Occlusions of the retinal vessels are rare. We present a case of central retinal artery occlusion in a 25-year-old male with Fabry's disease, documented by fluorescein- and indocyanine green angiography performed by scanning laser opththalmoscopy.

Early remodeling and loss of light-induced dilation of retinal small arteries in CADASIL.

A major hurdle to therapeutic development in cerebral small vessel diseases is the lack of in-vivo method that can be used repeatedly for ev … r, wall thickness and wall cross-sectional area were measured in a parapapillary and/or paramacular retinal artery. The ratio of vessel diameters before and after light flicker stimulations was also calculated to measure vasoreactivity (VR). Multivariate mixed-model analysis showed that WLR was increased and associated with a larger wall thickness and smaller in …
