Experience with photocoagulation in Behçet's disease.

Between 1973 and 1987 we examined both eyes of 300 patients with the uveoretinitis-type lesions characteristic of Behçet's disease. Of the 556 eyes whose fundus could be examined, 38 eyes (6.8%) in 33 patients (11%) had developed retinal capillary nonperfusion, branch retinal vein occlusion, or retinal or disc neovascularization. These eyes were treated by photocoagulation, primarily to forestall vitreous hemorrhage and the development of neovascular glaucoma, as well as to decrease the macular edema resulting from vein occlusion. The treatment, which was well tolerated, was successful in clos…

Ophthalmodynamometry: a reliable method for measuring intracranial pressure.

Under physiological conditions, the pressure in the central retinal vein is equal to or higher than the intracranial pressure (ICP) because the cerebrospinal fluid (CSF) passes the sheath of the optic nerve before draining into the cavernous sinus. The optic nerve sheath is where the ICP affects the retinal venous pressure. Ophthalmodynamometry (ODM) is a useful method for determining the central retinal artery pressure. While papilledema and a lack of venous pulsations are commonly used as a vague indication of the ICP, ODM may be advantageous for determining the pressure in the central retin…

Bilateral CRAO and CRVO from thrombotic thrombocytopenic purpura: OCT findings and treatment with triamcinolone acetonide and bevacizumab.

A patient with thrombotic thrombocytopenic purpura secondary to adult-onset Still's disease presented with bilateral combined central retinal artery occlusion and central retinal vein occlusion, a rare complication reported only once before. Fundus appearance and fluorescein angiography were similar to the previous case. Optical coherence tomography findings demonstrated aspects consistent with both central retinal artery occlusion and central retinal vein occlusion. Treatment of one eye with intravitreal triamcinolone acetonide (4 mg) was not effective in improving visual acuity. Treatment of…

Improved interpretation of flow maps obtained by scanning laser Doppler flowmetry using a rat model of retinal artery occlusion.

PURPOSE: To improve the interpretation of Heidelberg Retina Flowmeter (HRF; Heidelberg Engineering GmBH, Dosselheim, Germany) flow maps by e … d to blood flow. Strong flow signals can be obtained from the retinal arteries, retinal arterioles, retinal veins, and choroidal vessels. Current HRF flow maps in the rat therefore reflect blood flow in the larger elements of the microvasculature rather than the capillary network.

Vitreous haemorrhage caused by unusual giant macular tear: a case report.

Macular tears rarely occur without trauma. Here, we describe a patient with vitreous haemorrhage, which was caused by an unusual giant macular tear secondary to existing branch retinal vein occlusion. A 60-year-old woman presented with vision loss in the right eye because of vitreous haemorrhage. She had a history of branch retinal vein occlusion and had been treated with retinal photocoagulation 3 years prior. As treatment for vitreous haemorrhage, the patient underwent 23-gauge pars plana vitrectomy combined with silicone oil tamponade. During the operation, a large jagged tear was observed …

An eye disorder caused by chronic cardiovascular disease.

Retinal vein occlusion is a serious ophthalmologic condition that requires prompt diagnosis and treatment to avoid permanent vision loss. With acute onset of central vein occlusion, vision loss is sudden, unilateral, and painless. Vision loss from branch vein occlusion is typically less severe. Many chronic medical conditions put the patient at high risk for this problem, among them hypertension, diabetes mellitus, arteriosclerosis, and other types of cardiovascular disease. Primary care practitioners, who ordinarily see many patients with such chronic illnesses, are in an excellent position t…

Opticociliary veins in a primary optic nerve sheath meningioma.

A 59-year-old man developed painless visual loss in his left eye over a one-year period, with rapid progression to blindness over three months. Ocular examination of the left eye revealed proptosis, marked optic disk pallor, dilated retinal veins, and congeries of vessels at the disk margin (opticociliary veins). His right eye was unremarkable. Fluorescein angiography and histopathologic examination confirmed the presence of opticociliary veins communicating between branches of the central retinal vein and the adjacent choroidal vasculature. Primary orbital optic nerve meningioma associated wi…

Retinal venous occlusions: diagnosis and choice of treatments.

Retinal vascular occlusive disorders constitute one of the major causes of blindness and impaired vision. There is marked controversy on their pathogeneses, clinical features and particularly their management. Recently, advances in clinical research added antivascular endothelial growth factor, corticosteroids and sustained-release implants to our armamentarium in the management of retinal vein occlusions. The purpose of our paper is to provide an update and a brief review on the current treatment options of retinal vein occlusions.

Computer-Aided Detection of Retinopathy of Prematurity Severity in Preterm Infants via Measurement of Temporal Vessel Width and Angle.

Retinopathy of prematurity (ROP) is a retinal disorder that occurs in preterm infants with low birth weight and is the leading cause of preventable blindness in children. Early identification of high-risk patients and early diagnosis and timely treatment of ROP can substantially improve patients' visual outcomes. However, manual screening consumes both time and resources. Telescreening using retinal fundus images has the potential to reduce the burden engendered by the necessity of on-site screening. Recently, substantial progress has been made in using computer-aided diagnosis with retinal fu
