A 43-year-old male with a diagnosis of severe acute pancreatitis (SAP) was admitted to our hospital. After receiving the step-up approach for three weeks, the patient had a sudden hematemesis (nearly 400 ml). Gastroscopy and computed tomography (CT) were performed on the same day, revealing a gastric ulcer with a hemorrhage and the portal vein was extremely compressed by peripancreatic fluid collections in the retroperitoneal space.

We report a case of a 66-year-old man with an excessive wound fluid discharge during negative pressure wound therapy (NPWT) in the retroperitoneal space. Creatinine concentration of the unclear fluid and a subsequent late-phase computed tomography confirmed the diagnosis of a urinary fistula. NPWT was terminated, and the patient was successfully treated with a nephrostomy combined with a retrograde double J stent and a Foley catheter.

A case of lithiasic pyonephrosis evolving with intrapyelic gas formation, with positive urinary culture for Escherichia coli, is presented. Original clinical presentation was a picture of acute peritonitis with pneumoperitoneum. Clinical and radiological findings were indicative of exploratory laparotomy. The picture was resolved surgically, the procedure adopted being nephrectomy with retroperitoneal space drainage and empirically chosen antibiotics. The paper establishes the uncommon nature of this complication, and includes a literature review.

A method for percutaneous endoscopic study of the renal pelvis has been developed in our clinic. A thin endoscope has been manufactured which is introduced into the dilated renal pelvis via a puncture under ultrasonic real-time guidance to allow inspection. This technique has been named percutaneous pyeloscopy. The method can also be applied to the inspection of the renal cyst, the retroperitoneal space, and the urinary bladder.

BACKGROUND: Laparoscopic surgery is the standard of care for many colon cancer procedures but in patients with high BMI, extensive adhesions, or significant comorbidities, the conventional approach can be technically challenging. The retroperitoneal approach (BRIDGE-technique)* which provides direct access to the retroperitoneal space offers an alternative that enables early vascular control and lymph node dissection without entering the abdominal cavity.
METHODS: We demonstrate two different techniques for port placement in the retroperitoneal approach for right side colon cancer. Three sepa…

Severe forms of acute pancreatitis were treated in 53 patients (25 with a destructive form, 28 with an edematous form) with using permanent infusions of a novocaine cocktail into the retroperitoneal space. General lethality was reduced to 5.7%. In the group of patients with a destructive form of acute pancreatitis lethality was 12%. Nobody died in the group of patients with the edematous form. No complications were noted resulting from the method.

BACKGROUND/AIM: In rare cases, patients undergoing renal transplantation (RT) may develop renal allograft compartment syndrome (RACS) if the renal volume is larger than the retroperitoneal space prepared in the iliac fossa. In such cases, the optimal treatment is early decompression surgery. The primary aim of the present descriptive and comparative study is not to identify the optimal surgical technique, but rather to evaluate the role of early Doppler ultrasonography (DUSG) in the recognition of renal allograft compartment syndrome and to assess outcomes following timely decompressive interv…

Ganglioneuroma (GN) is the most differentiated and benign variant of neuroblastic tumors, most commonly located in the posterior mediastinum, followed by the retroperitoneum and adrenal gland. Children of <10 years of age are more commonly affected than adults. Though benign, GNs can very rarely metastasize to regional lymph nodes or distant sites like liver, bone, spleen, and soft tissues. Metastatic lesions are assumed to represent neuroblastomas in which the metastasis and the primary tumor, both have matured. This differentiation can occur spontaneously or after treatment. We present a pri

Differential diagnosis of retroperitoneal masses may become complex and requires careful anamnesis, physical examination and several complementary tests. We present the clinical case of a male patient aged 45 years who was diagnosed with a 4cm paraaortic lesion compatible with neuroendocrine tumor in the abdominal computed tomography (CT) exam. The workup performed with SPECT-CT, somatostatin receptors scintigraphy, MIBG scintigraphy, 24-hour urine total and fractionated catecholamines and 24-hour urine 5-OH indoleacetic did not confirm the first diagnostic impression. Finally, the lesion was

BACKGROUND: Retroperitoneal tumors are an uncommon disease known to consist of a diverse group of benign and malignant neoplasms. Treatment of unresectable retroperitoneal lesions requires pathological diagnosis. Here, we report the utility and safety of retroperitoneoscopic biopsy for unresectable retroperitoneal lesions excluding urogenital cancers.
METHODS: We analyzed 47 patients consisting of 23 (49%) and 24 (51%) cases that underwent retroperitoneoscopic tissue biopsy and open biopsy, respectively. The clinicopathological features, including postoperative complications, were compared be

BACKGROUND: Leiomyomas are benign tumors observed mainly in adult women. The retroperitoneum is a rare location for leiomyomas; almost 100 cases have been reported. Because retroperitoneal leiomyomas are paucisymptomatic and the tumor size at diagnosis is relatively large, surgical management is challenging. Regular follow-up is required because recurrence and malignant sarcomatous transformation have been described in a few cases.
CASE PRESENTATION: We report a case of a 52-year-old North African woman with a 22-cm retroperitoneal leiomyoma. A preoperative embolization was performed 2 days b

We present a case of retroperitoneal well-differentiated inflammatory liposarcoma that was extremely difficult to diagnose preoperatively. Computed tomography and magnetic resonance images showed a 5-cm homogeneous soft-tissue mass with a decreased apparent diffusion coefficient and without fat component in the retroperitoneum. Minimal fat stranding was detected around the mass. The preoperative working diagnosis was malignant lymphoma or inflammatory pseudotumor, whereas the final diagnosis after surgery was well-differentiated inflammatory liposarcoma. As a result, only a large component of

Heterotopic pancreatic tissue is found in several locations of the body. However, to the best of our knowledge, there have been no reports on heterotopic pancreas in the retroperitoneum. A case of retroperitoneal lipoma-like large tumor mass probably arising in the heterotopic pancreas is reported. A 45-year-old Japanese woman was admitted to hospital because of back pain. Imaging modalities showed an abnormal mass in the retroperitoneum separate from the surrounding organs, including the pancreas and kidney. Histologically, the mass consisted of mature adipose tissue and scattered ductal and

The case of a 12-year-old boy with hemorrhage into a previously unknown retroperitoneal lesion following blunt abdominal trauma is reported. Diagnostic work-up of a post-traumatic surgical acute abdomen revealed a giant multicystic tumor in the retroperitoneum, which could be completely removed. Histological examination confirmed the suspected diagnosis of congenital lymphangioma. Attention should be drawn to the possible coincidence of trauma and a preexisting asymptomatic lesion. Knowledge of the characteristic ultrasonographic and computer tomographic features is essential in order to make

We report on our experience in fine-needle aspiration (FNA) biopsy of the retroperitoneum: 111 FNA biopsies performed on 99 patients. Cytologic diagnoses were divided into four groups: nondiagnostic (unsatisfactory samples because of a low cellularity and/or improperly prepared smears) aspirates (20%), benign (16%), suspicious for malignancy (13%), and malignant (50%). There were no known false-positive samples. We had two false-negative diagnoses due to sampling errors. Among diagnostic smears, the procedure showed a sensitivity of 97% and a specificity of 100%. Evaluation of the retroperitoneum is important to assess the extent of retroperitoneal malignancies and because the retroperitoneum is a route of nodal spread for other malignancies. Positron emission tomography (PET) with 2-[fluorine-18]fluoro-2-deoxy-D-glucose (FDG) allows detection of small malignant nodes not identified or not meeting size criteria for malignancy with computed tomography (CT) and tumor recurrences in surgical beds that are otherwise difficult to assess. However, evaluation of retroperitoneal malignancies or adenopathy with FDG PET can be complicated by urinary and colonic

A case of retroperitoneal fibrosis associated with scirrhous gastric cancer is reported. A sixty two-year-old Japanese female was admitted because of acute renal failure. The patient's serum creatinine level showed 3.2 mg/dl while the blood urea nitrogen level was 23 mg/dl. An ultrasound study of the upper abdomen revealed bilateral hydronephrosis. Drip infusion pyelography revealed a dilated right renal pelvis without ureteral obstruction. The left kidney was not opacified, suggesting a functional disorder. Gastrography and gastrofiberscopy revealed scirrhous gastric cancer. Signet ring cell
