Common and Less Well-known Upper-limb Injuries in Elite Tennis Players.

A recent increase in epidemiology studies on injuries in elite tennis players has helped to shed light on the types of injuries these athletes sustain. This article reviews the common upper-limb injuries in elite players and includes less well known, but important, injuries. A search was conducted to identify current relevant studies involving elite tennis players. Injury frequency rates, injuries by regions, and types of injuries together with a list of commonly reported injuries in the upper limb were established. This list was then reviewed and refined by a sports medicine physician who car

New endoscopic classification for subscapularis lesions.

BACKGROUND: The absence of a coherent classification system has hampered communication about the treatment and outcomes of the various types of subscapularis tendon lesions. In addition, a reliable classification system allows comparisons of epidemiological and therapeutic data. The classification systems used until now fail to incorporate the radiological and intraoperative abnormalities of the bicipital sling, and they do not consider the degree of subscapularis tendon cleavage. Here, we describe a new arthroscopy-based classification system intended for therapeutic and prognostic purposes.

Preoperative diagnosis and rotator cuff status impact functional internal rotation following reverse shoulder arthroplasty.

PURPOSE: This study aimed to evaluate whether functional internal rotation (fIR) following reverse shoulder arthroplasty (RSA) differs based on diagnosis of either: primary osteoarthritis (OA) with intact rotator cuff, massive irreparable rotator cuff tear (MICT) or cuff tear arthropathy (CTA). METHODS: A retrospective review was carried out on RSAs performed by a single surgeon with the same implant over a 5-year period. Minimum 2-year follow-up was available in 235 patients; 139 (59.1%) were female, and the mean patient age was 72 ± 8 years. Additional clinical evaluation included the Subje

Clinical outcomes following surgical fixation of acromion fractures.

BACKGROUND: Acromial fractures are rare in the traumatic setting; however, they have recently gained attention due to the increase in incidence as a result of a postoperative complication of reverse total shoulder arthroplasty. While historically these fractures were routinely treated nonoperatively, there is evidence that surgery can improve outcomes. The study aims to evaluate clinical outcomes following surgical treatment of acromion fractures and compare outcomes among patients with an intact rotator cuff against those with a deficient rotator cuff or reverse shoulder arthroplasty. METHOD

Postoperative Rehabilitation Following Subscapularis Repair and Biceps Tenodesis in an Adolescent Overhead Athlete: A Resident's Case Report.

BACKGROUND: The incidence of rotator cuff pathology in adolescent athletes is incredibly rare, consisting of only 1% of upper extremity injuries in those under the age of 20. Most rotator cuff injuries in this population are of the supraspinatus with few being of the subscapularis. The subscapularis aids with internal rotation and stabilization of the glenohumeral joint. Surgical management of these injuries in an adolescent athlete presents unique challenges due to periods of rapid growth in adolescence. Additional challenges exist in guiding post-operative physical therapy due to limited inf…

Shoulder Osteoarthritis.

Glenohumeral osteoarthritis has proven to be a major contributor to shoulder joint pain and dysfunction in the elderly. There are several conditions about the shoulder that contribute to the development of glenohumeral osteoarthritis, which includes traumatic injuries, rotator cuff pathology, glenohumeral instability, glenoid dysplasia, and osteonecrosis. When glenohumeral osteoarthritis pain is refractory to conservative treatment, intra-articular injections and surgery can be performed. The radiologist should be aware of the varying types of shoulder arthroplasties, what preoperative finding

Anterior dislocation of the shoulder in the older patient.

A significant number of rotator cuff defects are brought to light following a first-time dislocation in older patients. Recurrence in this group of patients is low; however, standard anterior repairs may be employed effectively in such situations. Neurologic and vascular injuries, although infrequent, should be carefully ruled out as accompanying the initial injury. Knowledge of these facts will allow a better appreciation of the pathology associated with the anterior dislocation in the older patient. From this, more confident recommendations may be made regarding management of these difficult

Sex Is Associated with the Success or Failure of Manipulation Alone for Joint Stiffness Associated with Rotator Cuff Repair.

Purpose: One-stage arthroscopic rotator cuff repair with manipulation has been recently performed for rotator cuff tears with shoulder stiffness, whereas some patients require capsular release due to severe stiffness that is difficult to treat with manipulation. The purpose of this study was to analyze patient backgrounds and related factors of success or failure of manipulation alone for the treatment of shoulder stiffness associated with rotator cuff tears. Methods: This study included 64 patients with rotator cuff tears and shoulder stiffness who underwent arthroscopic rotator cuff repair w

Muscle work during manual wheelchair propulsion in people with a spinal cord injury: a model-based approach.

Manual wheelchair users are at high risk of developing shoulder pathologies due to the repetitive demands of propulsion. Understanding individual muscle contributions is essential to improve rehabilitation strategies and prevent overuse injuries. This study applied a modeling approach to evaluate the individual muscle contributions, particularly the rotator cuff muscles, to mechanical work during the push and recovery phases of wheelchair propulsion in individuals with a spinal cord injury. We used a thoracoscapular shoulder model in OpenSim, which incorporated accurate scapular kinematics and

Shoulder Impingement Pain Syndrome: Pathophysiology, Diagnosis, and a Review of Current Treatment Strategies.

The shoulder is an intricate joint, capable of a wide range of movements for both daily activities and physical exercise. This intricate joint is susceptible to various injuries and conditions due to its design. Shoulder impingement syndrome (SIS), also clinically recognized as subacromial impingement syndrome (SAIS), is a prevalent source of shoulder discomfort, which is a spectrum of disorders such as rotator cuff tendinopathy, partial or complete tears, and inflammation of the subacromial bursa. SAIS can be caused by intrinsic causes, such as tendon degeneration, and extrinsic factors, such

Optimizing Physiotherapeutic Effects With the Maitland Mobilization Technique to Boost the Functional Capacities of a Rotator Cuff Syndrome Patient: A Case Report.

Rotator cuff (RC) muscles give the shoulder joint stability in addition to movement. The case report outlines the physical rehabilitation therapy, condition evaluation, and diagnostic testing that was given to a 62-year-old female field worker who had been complaining of pain in her left shoulder. To improve functional mobility and lessen discomfort, the patient underwent physiotherapy. In this instance, a physical therapy program was put in place to treat rotator cuff syndrome (RCS), enhance range of motion (ROM), and promote long-term recovery. Part of the assessment included a detailed exam

Tissue-engineered rotator cuff tendon using porcine small intestine submucosa. Histologic and mechanical evaluation in dogs.

To determine its efficacy in stimulating the regeneration of a rotator cuff tendon, an implant of 10-ply porcine small intestinal submucosa was used to replace a completely resected infraspinatus tendon in 21 adult mongrel dogs. The contralateral infraspinatus tendon was elevated and then reattached to the greater tubercle with sutures to mimic conventional repair (sham operation). Mechanical evaluations were performed at 0, 3, and 6 months (five specimens at each time period). Histologic comparisons were made at 3 and 6 months (three specimens). At both times, the gross appearance, histologic

Arthroscopically assisted mini-deltopectoral rotator cuff repair.

This study examined the technique and outcome of arthroscopically assisted mini-deltopectoral rotator cuff repair. The mini-deltopectoral approach enables the repair of large or massive rotator cuff tears while preventing injury to the deltoid muscle and axillary nerve often caused by splitting and excessive traction during conventional mini-open repair. This technique also is useful in the repair of subscapularis tears and thus could extend the indications for mini-open repair.

Massive rotator cuff tears without arthropathy: when to consider reverse shoulder arthroplasty.

Massive rotator cuff tears often present a challenge for the treating orthopaedic surgeon. A multitude of surgical approaches have been described to manage this condition, ranging from biceps tenotomy to complex muscle transfers to reverse shoulder arthroplasty. Among these procedures, reverse shoulder arthroplasty is increasingly advocated to relieve pain and restore function; however, the exact role of this arthroplasty procedure continues to be defined, particularly in patients without any evidence of associated glenohumeral arthritis. In this patient population, the reverse shoulder prosth

Guidelines for the Diagnosis and Treatment of Rotator Cuff Tear with Integrated Traditional Chinese and Western Medicine.

Rotator cuff tear is a common injury among middle-aged and elderly people, and it has a great impact on patients' physical and mental health and quality of life. Integrative medicine based on Traditional Chinese Medicine (TCM) has certain advantages in the diagnosis and treatment of rotator cuff tears. TCM, which mainly involves the use of plant-based products, has relatively stable and reliable curative effects. It is of great significance to formulate a combined diagnosis and treatment plan for rotator cuff tear based on evidence-based medicine, which can help to standardize the clinical dia
