Rupture of rotator cuff in the elderly people is mostly degenerative. Due to the rupture, function of the arm may be disturbed up to pseudo-paralysis. Apart from the functional disturbances the patient is suffering from tormenting nightly pain. The therapy for the elderly people will almost always be a conservative one. Apart from symptomatic measures, physical therapy to prevent a frozen shoulder is essential. With old age operational indication should be particularly restrictive and only where conservative treatment has failed so far.

From 1964 to 1984, 10 male and 5 female patients with ruptured aneurysms of the sinus of Valsalva (ASVs) underwent surgical correction. These procedures constituted 0.23% of the 6,350 surgical procedures that used cardiopulmonary bypass during this period. Five patients had an inflammatory condition (bacterial endocarditis, 4; syphilis, 1), and 1 had an ASV relapse 5 years after her first operation. Fifteen cases of extrahepatic rupture of amebic liver abscess have been reviewed. Five patients had thoracic rupture and ten had intra-abdominal rupture. Celiotomies were performed in five patients, with a preoperative diagnosis of acute appendicitis with perforation in four patients and generalized peritonitis of unknown origin in one patient. Ruptured aneurysms of the aortic sinuses of Valsalva have been a surgical rarity at the Karolinska Hospital. Only nine such cases were operated on over a 13-year period (1968-1971). Traumatic rupture of the thoracic aorta is a common injury in automobile accidents. It is the result of a crushing chest injury, most frequently associated with sudden deceleration. Approximately 10 to 20% of the individuals with traumatic rupture of the aorta survive temporarily. During the past 10 years, 15 traumatic ruptures of the aorta isthmus have been diagnosed. The most likely mechanism is that of a brutal deceleration. Three patients developed a complete mid-thoracic flaccid paraplegia. One died in a few hours of a complete rupture, the second after a few days from renal insufficiency, the third 12 days after, having been operated upon on the same day of the accident. A patient had delayed spontaneous rupture of the spleen complicating infective endocarditis. In 20 other cases reported through 1973, the most common presentation was found to be left upper quadrant pain followed by signs of peritoneal irritation and cardiovascular collapse.
