Injuries to the glenoid labrum: a diagnostic and treatment challenge.

The glenoid labrum contributes to shoulder stability by increasing the glenoid surface and providing attachments for the shoulder capsule and various ligaments and tendons. Overhead arm activities, especially throwing, can stress the shoulder excessively and cause labral injury. Symptoms include poorly localized pain, exacerbated by overhead and behind-the-back motions, and popping, catching, or grinding. Imaging includes AP, outlet, and axillary radiographs, but CT arthrogram or MR arthrogram is also recommended. Fractures and dislocations should be referred. In other labral injuries, 2 to 4

Erb's palsy without shoulder dystocia.

OBJECTIVES: The purpose of this commentary is to review certain articles which have provided evidence that Erb's palsy can occur without associated shoulder dystocia. The mechanism of the specific cause of the injury will be described. METHODS: Prior to the last 10-12 years it was assumed that Erb's palsy occurred exclusively with and was the result of shoulder dystocia. Gonik et al. [Am J Perinat 1991;8:31-34], reported on a research study based on the premise that when Erb's palsies occurred there must have been shoulder dystocia present but it went undetected by the delivering physician. Causes and associated factors of mountain bike injuries in bike parks in western Austria: a two-season descriptive epidemiological study.

Mountain bike terrain parks have become increasingly popular, particularly at ski resorts where chairlifts transport riders uphill to enjoy downhill cycling. These parks feature trails of varying difficulty with artificial obstacles designed for jumps and stunts, which contribute to a unique pattern of injuries. This retrospective epidemiological study analyzed mountain biking injuries in two bike parks in western Austria during the summer seasons of 2023 and 2024. The bike parks are located within two major ski resorts, where lifts transport riders and their bikes uphill. Injury data were sys

Shoulder pain in hemiplegia.

Shoulder pain is a common complication in poststroke hemiplegia that reduces functional recovery. Many types of shoulder pathology have been suggested as causes of shoulder pain in hemiplegia, including shoulder subluxation, capsulitis, tendinitis, rotator cuff injury, bursitis, impingement syndrome, spasticity, complex regional pain syndrome, brachial plexus injury, and proximal mononeuropathies. More than one type of pathology may exist in a given patient. Shoulder pain improves in many cases with prompt diagnosis and appropriate management. Although the relationship between subluxation and p

Electromyographic analysis of an ergonomic risk factor: overhead work.

[Purpose] Repetitive upper extremity exercises result in overuse injuries. However, it is challenging to identify the specific causative movements. This study evaluated the effects of different shoulder positions on grip and electrophysiological activity of upper extremity muscles. [Subjects and Methods] Forty subjects ranging from 18 and 30 years of age were analyzed. Surface electromyography and hand grip strength were measured during a range of shoulder exercises and numerous signal processing methods were applied. [Results] The maximum electromyographic activity intensity was observed in t

Factors affecting shoulder pain in adolescents and young adults with spina bifida.

PURPOSE: This study was designed to determine whether factors affecting shoulder pain in adults with spinal cord injury also affected adolescent and young adult manual wheelchair users with spina bifida. METHODS: Forty-one participants with spina bifida rated their pain using the Wheelchair User's Shoulder Pain Index. RESULTS: Results showed no significant relationship between shoulder pain and age or duration of wheelchair use but significant differences in pain between age groups. Participation in a sports program and level of lesion were not factors in pain intensity. Propelling up an inc

[Sports pathology of the shoulder: its imaging diagnosis].

Success and diffusion of various sports have produced more frequent capsular-ligamentous lesions of the shoulder. Diagnosis up to now has relied on traditional radiology using many projections to demonstrate extremity lesions and their congruences; however for capsular-ligamentous lesions, arthrography in used successfully. Since the last few years new investigating methods are used (U.S., C.T., M.N.R.). After examining benefits of each diagnosing method compared to traditional radiology, the authors conclude that traditional radiology still has a pre-eminent role in diagnosing sports patholog

Common Upper-Extremity Injuries.

Primary care and sports medicine physicians will undoubtedly encounter upper-extremity injuries on a regular basis in their practice. Athletes have injuries most commonly to the shoulder, elbow, wrist, and hand as a result of a fall onto an outstretched arm. This article aims to educate physicians about sports-related upper-extremity injuries. Common mechanisms of injury, classic physical examination, and radiographic findings are reviewed. General guidelines for treatment as well as indications for referral to a sports medicine or orthopedic specialist are included in the discussion.

Luxatio erecta (inferior dislocation of the shoulder): a report of 5 cases and a review of the literature.

Luxatio erecta (inferior dislocation of the glenohumeral joint) is an unusual type of shoulder dislocation. The authors present 5 new cases of luxatio erecta. All cases were hyperabduction injuries. In all cases, closed reduction was accomplished by a traction-countertraction maneuver. Three of the 5 subjects sustained concomitant fractures of the greater tuberosity. One underwent open reduction and internal fixation of the greater tuberosity. One suffered axillary nerve injuries. No vascular injuries were noted in our series. The mechanism of the injury is discussed.

Facial injuries in automobile crashes.

In automobile accidents, the facial area is the most frequently injured body region in passenger car occupants. Laboratory studies have indicated that the tolerance of facial bones to impact is relatively low. Most of these facial injuries are rated as minor. The windshield, steering wheel, and instrument panel are the major points of contact. Restraints, lap belts, and lap-shoulder belts reduce the frequency of facial injuries at all levels of severity and also reduce the more severe and serious injuries to other body regions.

Miniopen coracohumeral ligament release and manipulation for idiopathic frozen shoulder.

In the management of idiopathic frozen shoulder, manipulation under anaesthesia is known to have serious potential complications including fractures and intra-articular injuries. Arthroscopy is a safer treatment modality but requires special instruments, experience, and involves added cost. The aim of this work was to study the use of miniopen Coracohumeral ligament release and manipulation of the shoulder as a safe and simple method of treating idiopathic frozen shoulder that could be performed as a quick procedure under short duration anaesthesia obtaining a significant improvement of should

[Prediction, prevention and management of shoulder dystocia].

Shoulder dystocia is one of the most tragic, fatal and unexpected obstetrical events, which is mostly unpredictable and unpreventable. This clinical picture is defined as a delivery that requires additional obstetric maneuvers to release the shoulders after gentle downward traction has failed. Shoulder dystocia occurs when the fetal shoulder impacts on the maternal symphysis or sacral promontory. The incidence of shoulder dystocia is 0.2-0.6%. High perinatal mortality and morbidity is associated with the condition, even when it is managed appropriately. Obstetricians should be aware of the exi

[Surgical treatment of shoulder instability].

Most commonly, shoulder instability results from a traumatic dislocation of the shoulder joint, usually happening to men under the age of 30. The first uncomplicated dislocation is treated conservatively. Instability refers to symptomatic instability of the shoulder joint, i.e. recurrent dislocations or fear of dislocation. Clinical examination and X-ray are usually sufficient for diagnosis. While mechanical instability of the shoulder joint is an indication for surgery, the operative technique should be selected in a patient-specific manner.
