Radiographic analysis of polyethylene glenoid components using modern cementing techniques.

The purpose of this study was to examine the initial radiographic appearance and changes occurring over time in patients who have undergone total shoulder arthroplasty by modern methods of bone preparation and current cement techniques. Sixty-five patients underwent seventy total shoulder arthroplasties by use of a cemented all-polyethylene, keeled glenoid component. The surface radius was equal to the radius of a one-piece humeral component. The mean clinical follow-up was 4.18 years (range, 2-8.6 years); radiographic follow-up averaged 3.9 years (range, 2-8.6 years). Three observers evaluate

Seidel intramedullary nailing of humeral diaphyseal fractures: a preliminary report.

Forty-two humeral diaphyseal fractures in 41 patients were treated at three centers between April 1988 and November 1989. There were 28 acute fractures; four were open. Average time to union was 8 weeks. There were no infections. Six patients with seven pathologic fractures due to metastatic disease died during the course of this study, but the nail had allowed them to be functional with minimal surgical dissection. Five of six nonunions united with one procedure. There was one residual nonunion in a patient with a wide canal and an arthrodesed shoulder above the nonunion. There were three pre

Locked posterior dislocation of the shoulder.

Of forty patients with forty-one locked posterior dislocations of the shoulder, the diagnosis had been missed by the initial physician in the majority. A motor-vehicle accident, a seizure, an alcohol-related injury, or electroshock therapy had caused the dislocation in these patients, and the average interval from injury to diagnosis was one year. Twenty-five of the forty-one dislocations had been diagnosed in less than six months. An axillary radiograph confirmed the diagnosis in all shoulders and demonstrated the approximate size of the impression defect. There were no associated displaced f

Microbiology of polymicrobial prosthetic joint infection.

Prosthetic joint infection (PJI) is a rare but challenging complication of arthroplasty. Herein, we describe the epidemiology and microbiology of PJI, with a focus on analyzing differences between the microbiology of polymicrobial versus monomicrobial infection of hip, knee, and shoulder prostheses. In addition, we report the most frequent co-pathogens in polymicrobial infections, as detected by culture. A total of 373 patients diagnosed with PJI at Mayo Clinic were studied. For hip and knee arthroplasties, a higher proportion of fractures (P = 0.02) and a shorter time between the implantation

Biomechanical comparison of a proximal humeral locking plate using two methods of head fixation.

HYPOTHESIS: Locking plates have emerged as the implant of choice for stabilization of proximal humeral fractures. The biomechanical properties of a locked plating system using smooth pegs vs threaded screws for fixation of the humeral head were compared to test the hypothesis that there would be no biomechanical difference between pegs and threaded screws. MATERIALS AND METHODS: Sixteen pairs of fresh frozen cadaveric humeri were randomized to have a surgical neck gap osteotomy stabilized with a locked plate using threaded screws (n=8) or smooth pegs (n=8). The intact contralateral humerus se

[Traumatic aortic regurgitation complicating chronic bronchitis; report of a case].

A 61-year-old man with diabetes mellitus and chronic bronchitis was brought to the hospital after falling from a roof accidentally. He received blunt trauma to the left chest wall including left rib fractures, pneumothorax, hemothorax and a fracture of left scapula. After endotracheal intubation and chest drainage, he was transferred to the intensive care unit. Sixteen days later, congestive heart failure appeared. Echocardiography showed a severe aortic valve regurgitation with a prolapse of non-coronary cusp. Eight months after the injury, his heart failure deteriorated. He underwent aortic

Periprosthetic infections after total shoulder arthroplasty: a 33-year perspective.

BACKGROUND: To examine the rates and predictors of deep periprosthetic infections after primary total shoulder arthroplasty (TSA). METHODS: We used prospectively collected data on all primary TSA patients from 1976-2008 at Mayo Clinic Medical Center. We estimated survival free of deep periprosthetic infections after primary TSA using Kaplan-Meier survival. Univariate and multivariable Cox regression was used to assess the association of patient-related factors (age, gender, body mass index), comorbidity (Deyo-Charlson index), American Society of Anesthesiologists class, implant fixation, and

Removal of well-fixed flanged humeral prostheses may require humeral expansion.

BACKGROUND: A flanged humeral stem design can be advantageous in achieving an interlock between the prosthetic and bone interface leading to a long-term stable loading pattern. The purpose of this study is to report a short case series involving revision of a flanged humeral prosthesis. Our hypothesis was that a greater percentage of the flanged prostheses undergoing revision would require some form of bone expansion to achieve stem removal compared to the nonflanged. METHODS AND RESULTS: In the period from October 2004 to July 2008, 43 patients underwent revision of a humeral prosthetic stem

Total clavicle reconstruction with free peroneal graft for the surgical management of chronic nonbacterial osteomyelitis of the clavicle: a case report.

BACKGROUND: Chronic nonbacterial osteomyelitis (CNO) is a rare chronic autoinflammatory syndrome affecting mainly children and young adults. The natural history of the disease is marked by recurrent pain as the mainstay of inflammatory outbreaks. Typical radiographic findings are osteosclerosis and hyperostosis of the medial clavicle, sternum and first rib. Compression of the brachial plexus is exceedingly rare and one of the few surgical indications. Literature on total clavicle reconstruction is scarce. While claviclectomy alone has been associated with fair functional and cosmetic outcomes,

Clinical and functional outcomes of plate and screw osteosynthesis in mason type III and IV radial head fractures in the absence of a radial head prosthesis.

INTRODUCTION: Radial head and neck fractures represent a significant portion of elbow fractures in adults, particularly affecting younger patients due to falls onto outstretched hands. Surgical treatment is often necessary for displaced and comminuted fractures, especially those with associated ligamentous injuries. This study evaluates the long-term clinical, functional, and radiological outcomes of patients with Mason type III and IV radial head fractures managed through open reduction and internal fixation using plate and screw osteosynthesis, due to the unavailability of radial head prosth

After failed radial head arthroplasty, what are the options? Risk factors and results of revisions in a multicenter study.

INTRODUCTION: Few multicenter studies have analyzed the outcome of revision surgery of radial head arthroplasties (RHA) in the medium term follow up. The objective is twofold: to determine the factors associated with revision of RHAs and to analyze the results of revision with 2 surgical techniques: isolated removal of the RHA or revision with a new RHA (R-RHA). HYPOTHESIS: There are associated factors of RHA revision and RHA revision results in satisfactory clinical and functional outcomes. METHODS: Twenty-eight patients were included in this multicenter retrospective study, with all surgic

Reverse shoulder replacement for patients with inflammatory arthritis.

PURPOSE: Patients with inflammatory arthritis frequently develop destructive shoulder arthritis and rotator cuff tearing. Reconstruction with anatomical shoulder replacement produces lesser results than for other etiologies such as osteoarthritis. We postulated that reconstruction with reverse shoulder prostheses would lead to early satisfactory results for these patients. METHODS: We carried out a retrospective review of 19 shoulders treated with reverse shoulder prostheses at a mean follow-up of 37 months. We rated patients' pain using a visual analog pain scale (VAPS) and noted active rang

Endopelvic migration of a sternoclavicular K-wire. Case report and review of literature.

We report a unique case, never before published, of sternoclavicular joint fixation K-wire migration to the pelvic region, in a 56 year-old man. Two years previously, sternoclavicular dislocation had been fixed by three wires. A transitory episode of precordial thoracic pain followed by iterative abdominal pain accompanied the migration. Extraction was performed five years later. Scapular K-wire migration is frequent. The proximity of cardiovascular structures may have fatal consequences. This type of internal fixation raises questions, and migration prevention needs to be taken into account.

Importance of neurological evaluation before reverse shoulder arthroplasty in patients with previous poliomyelitis.

We present a case report of a patient, affected by post-polio syndrome and cuff tear arthropathy of his right shoulder, who underwent reverse shoulder arthroplasty. A previous radial deficiency was unmasked in the post-operative follow-up, with passive elbow bending when the forearm was put in an antigravitational position. A precise preoperative neurological evaluation is mandatory in shoulder replacement in order to estimate the functional outcome after surgery.

Minimally invasive reduction and osteosynthesis of articular fractures of the humeral head.

Percutaneous reduction and fixation of severe humeral head fractures would be the treatment of choice since it will not increase the risk of necrosis already inherent in these fractures. Nevertheless, the question arises of whether anatomical reduction is possible with the percutaneous technique and whether the reduced fracture can be adequately stabilized. It is important to study the fracture closely before the operation in order to determine the fracture type and identify the relationship of the individual fragments to each other. Radiographs taken in at least two planes are essential and a

Management of the Failed Arthroplasty for Proximal Humerus Fracture.

A variety of reasons exist for failure of arthroplasty performed for management of proximal humerus fracture. Revision surgery for these failures is complex and has a high likelihood of inferior outcomes compared with primary arthroplasty. Successful management requires consideration of various modes of failure including tuberosity malunion or resorption, rotator cuff deficiency, glenoid arthritis, bone loss, component loosening, stiffness, or infection. Although revision to a reverse shoulder arthroplasty is an appealing option to address instability, rotator cuff dysfunction, and glenoid art

Glenoid component retroversion is associated with osteolysis.

BACKGROUND: It has been suggested that glenoid component retroversion and eccentric loading are an important mechanism leading to glenoid component loosening, but little clinical data have been published to support this concept. METHODS: Sixty-six shoulders underwent total shoulder replacement with an all-polyethylene press-fit pegged glenoid component designed for osseous ingrowth for treatment of osteoarthritis. These shoulders were followed clinically and with radiographs for an average (and standard deviation) of 3.8 ± 1.8 years (range, two to seven years). Preclinical radiographic loosen
