Peroxisome proliferator-activated receptor (PPAR) gamma is an important signaling molecule in cells of mesenchymal origin, inducing differentiation and regulating cell proliferation in several cell types such as vascular smooth muscle cells. Leiomyomas arise from smooth muscle cells of the uterine myometrium with an incidence rate as high as 70% in women of reproductive age. Leiomyomas are benign mesenchymal neoplasms mostly seen in the uterus and are one of the most common pelvic masses seen in women, but primary ovarian leiomyomas are rare among all the benign ovarian tumors, which account only for 0.5%-1%. Gastrointestinal (GI) schwannoma is one of the rarest tumors of the GI tract. We herein describe the case of a 70-year-old female patient who presented for surveillance colonoscopy. The examination detected a 1-cm polyp that was removed by snare cautery polypectomy. Immune and histochemical staining revealed spindle cells that were positive for S-100 and vimentin, but negative for CD34 and smooth muscle actin, consistent with GI schwannoma. Neuron-derived orphan receptor 1 (NOR1), also called nuclear receptor subfamily 4 group A member 3 (NR4A3), is a nuclear receptor belonging to the NR4A family. Since no endogenous ligand has been identified to date, NOR1 is also referred to as an orphan receptor. NOR1 is expressed in a variety of cells and tissues, including neurons, vascular smooth muscle cells, T lymphocytes, dendritic cells, tumor cells, heart, liver, and pancreas. Because NOR1 was first identified in apoptotic neurons, it is functionally associated with the regulation of cell migration and the growth of neuronal synapses.

A series of soft tissue tumors, melanomas, carcinomas, and lymphomas were studied immunohistochemically for the presence of muscle actins (MA) with the monoclonal antibody HHF-35, and for the presence of desmin for comparison. We report a case of a uterine mesenchymal tumor with myogenic differentiation and SRF::RELA fusion occurring in a 23-year-old woman manifesting as an endometrial polyp.
