Management of uveitis in pediatric patients: special considerations.

Uveitis refers to inflammation involving the uvea or middle coat of the eye. This condition occurs uncommonly, particularly in persons aged <or=16 years. However, pediatric uveitis deserves special consideration for reasons that include the relatively poor prognosis, unique systemic associations, and various age-related treatment considerations. Accurate diagnosis requires history from both patient and parents, a complete ophthalmic examination that may require general anesthesia, and carefully selected investigations. Infections and masquerade syndromes, such as leukemia and retinoblastoma, m

The use of electroconvulsive therapy in special patient populations.

BACKGROUND: Despite its well-established efficacy and its increasing use, electroconvulsive therapy (ECT) remains a controversial treatment. Lack of clarity in the issues related to its use in special patient populations (for example, in children, in adolescents, in pregnant women, in the elderly, and in the medically ill) often contributes to the debate about the use of ECT. METHOD: The literature on ECT use in special patient populations is reviewed, together with the commonly associated high-risk medical conditions in clinical practice. Specific reference is made in each case to the safety

Ocular nocardia infections with special emphasis on the cornea.

Nocardia are aerobic, gram-positive, nonmotile and branching filamentous bacteria. Corneal infection by Nocardia is rare. Trauma is the most common predisposing factor. Isolated case reports of nocardial infection associated with contact lens wear and laser in situ keratomileusis (LASIK) have been reported. The clinical picture usually consists of superficial patchy infiltrates, which may be arranged in a wreath pattern. Presence of gram-positive, branching, beaded filaments that stain with 1% acid-fast stain (using 1% sulfuric acid, modified Kinyoun's method) in smears of corneal scrapings is

[Specialized epidemic brigade during extreme situation].

Information on the main trends in the activity and on the organization of the work of the specialized antiepidemic brigade of the Stavropol Research Institute for Plague Control in Grozny in the first half of the year 2000 is presented. At the initial stage of this activity the specialists of the brigade were entrusted with all functions of the local state sanitary and epidemiological service which had lost its qualified personnel and material resources. At the initial stages of the work the main attention was drawn to the organization of the registration of patients with infectious diseases,

Injuries and special concerns of female figure skaters.

Figure skaters are often young athletes involved in an extremely stressful, structured environment that is focused solely on skating. These athletes are at risk for a multitude of psychologic concerns, including poor communication skills, performance anxiety, stress, and family issues. Helping athletes develop self-awareness, techniques to control stress and anxiety, motivational strategies, ability to maintain concentration, and emotional balance is best done with the guidance of a sports-trained counseling professional. Communication among the athlete, parents, and coaches is extremely impor

Neuromuscular rehabilitation and electrodiagnosis. 4. Specialized neuropathy.

This self-directed learning module briefly highlights the differential diagnosis for acute weakness in patients with acute respiratory failure requiring prolonged mechanical ventilation. It is part of the chapter on neuromuscular rehabilitation and electrodiagnosis in the Self-Directed Physiatric Education Program for practitioners and trainees in physical medicine and rehabilitation. This article includes a discussion on the role of exercise in the treatment of patients with the late effects of poliomyelitis or with acute inflammatory demyelinating polyradiculoneuropathy.

Enterovirus infections with special reference to enterovirus 71.

The enteroviruses comprise a large group of immunologically distinct serotypes of viruses belonging to the family of Picornaviridae. Many enteroviruses cause diseases in human, but the infections are generally mild as asymptomatic, therefore, enteroviruses are considered to be unimportant as human pathogens. However, enteroviruses may also result in serious or even fatal disease (as shown in the enterovirus 71 (EV71) epidemic in Taiwan in 1998). There are three types of polioviruses, Coxsackievirus group A and group B viruses, and echoviruses group. All together a total of 67 types are availab

Differences in problems of motivation in different special groups.

In general, children with a range of special needs have below-average motivation and perceived control. We have investigated whether differences exist between the types of problem in different special groups. Theory distinguishes between two types: low motivation and perceived control can be based either on a low perceived contingency, or on a low perceived competence. These two types of problem require different intervention strategies. The development of these types is related to several factors that might vary systematically in different special groups: specific histories of experience of s

Special considerations regarding CMV in lung transplantation.

Lung transplant recipients, more than other organ transplant recipients, are at particular risk for cytomegalovirus (CMV) infection and disease. A major factor in improving management of the lung transplant patient is the ability to prevent or minimize CMV disease, which itself is a major risk factor for opportunistic fungal infections and obliterative bronchiolitis. Strategies being evaluated to improve CMV prophylaxis and consequently graft and patient survival include longer-than-standard courses of prophylactic IV ganciclovir, sequential IV/oral therapy, addition of CMV IG, examining the i

A specialized apperception test for ambulatory-impaired adolescents.

A specialized apperception test for physically impaired adolescents was developed, and a pilot was conducted using ten nonimpaired and ten impaired adolescents. Ten cards depicting various scenes with physically impaired male and female models were designed to elicit themes that may be particularly relevant for this adolescent population. Two raters evaluated how well each card elicited issues identified in the literature as pertinent to physically challenged adolescents.

Special considerations in elderly individuals with urinary incontinence.

OBJECTIVES: To describe special considerations related to the etiology, evaluation, and treatment of urinary incontinence (UI) in the elderly. METHODS: The characteristics of UI in older patients are contrasted with those in younger patients. Recommended evaluations for elderly individuals are reviewed, and treatment issues are addressed. RESULTS: The etiology of UI is more complex in older than in younger patients, and a different clinical approach is required. Bladder overactivity is a common underlying component of UI in the elderly patient regardless of the complexity of the etiology. Th

Children with sensorimotor deficits: a special risk group.

Children with spina bifida, cerebral palsy, mental retardation, developmental delays, and seizure states are handicapped with sensorimotor deficits, including gait or coordination instability, temperature insensitivity, and mental simplicity. These handicaps make this distinct and unpretentious population more susceptible to lethal burns. A 30-year review was conducted in a pediatric burn center to examine the relationships between pediatric sensorimotor deficit and burn injury. Of the 4874 acute burn admissions, 66 children were identified with preexisting sensorimotor deficits. Data indicate

Connecticut State Special Olympics: observations and recommendations.

The Connecticut State Special Olympics provides an opportunity for thousands of developmentally challenged children and adults to participate in athletic events. This paper retrospectively reviews the demographics and documentation of medical problems in 1994, 1995, and 1996 Summer Games. The 1994 and 1995 summer games took place at the Coast Guard Academy and Connecticut College in New London. The 1996 summer games occurred at Fairfield and Sacred Heart Universities in Fairfield. The mean age of the participants ranged from 30 to 32 years of age. Minor trauma made up the largest percentage of

Length of stay for specialized pediatric urologic care.

BACKGROUND: Pediatric urologic specialists have been excluded from many recent managed care contracts because they are believed to be more expensive and of no better quality than general urologists in managing common urologic problems in children. We believed this to be inaccurate. OBJECTIVES: To compare the length of stay at the University of California, San Francisco Medical Center for 2 common pediatric urologic operative procedures with data from other northern California hospitals and to document our results and patient satisfaction. DESIGN: Retrospective analysis of HCIA statewide data
