Currently there is no evidence for prophylactic cholecystectomy to prevent gallstone formation (grade B). Cholecystectomy cannot be recommended for any group of patients having asymptomatic gallstones except in those undergoing major upper abdominal surgery for other pathologies (grade B). Laparoscopic cholecystectomy is the preferred treatment for all patient groups with symptomatic gallstones (grade B). Patients with gallstones along with common bile duct stones treated by endoscopic sphincterotomy should undergo cholecystectomy (grade A). Laparoscopic cholecystectomy with laparoscopic commo…

Fissure in ano is a simple condition that causes considerable discomfort. The acute forms should be treated by conservative nonsurgical regimens initially. If the patient finds the symptoms intolerable, or if the fissure demonstrates signs of chronicity, then lateral subcutaneous sphincterotomy is recommended as an ideal simple surgical procedure that has the advantages of a low complication rate and low morbidity. It is easily performed under local anesthesia and does not require hospitalization. It is gratifyingly satisfying for both the patient and surgeon. Certain types of anal stenosis, e…

The most common complication of hydatid cysts of the liver is cholestasis due to rupture of hepatic cysts into the biliary tree. Such a complicated cyst is usually managed by surgical cyst resection. Endoscopic retrograde cholangiogram with sphincterotomy is a well-established method for preoperative and postoperative extraction of cyst material from the biliary tree. Successful nonsurgical treatments of complicated hydatid cyst are reported with a combination of pharmaceutical therapy and endoscopic techniques consisting of endoscopic sphincterotomy and instillation of hypertonic saline solut…

More and more authors believe, that the cause of the chronic anal fissure is the increased anal resting pressure. It maintains a vicious circle causing ischaemia and ulcer on the anoderm. There are many traditional surgical methods treating fissures apart from the conservative ways. However, these are likely to cause significant morbidity. Chemical sphincterotomy seems to be a novel way and its most effective drugs are the nitrates. 75-80% of the patients can be cured with the topical use of these. This treatment has to be accompanied with the normalisation of the stools. If the symptoms persi…

PURPOSE: The optimal method of managing paediatric choledocholithiasis is controversial. Endoscopic retrograde cholangiopancreatography (ERCP) and sphincterotomy are effective in adults; however, the long-term outcome in the paediatric population is unknown. We report our experience with laparoscopic common bile duct (CBD) exploration to relieve choledocholithiasis in children and propose a management algorithm.
METHODS: A retrospective chart review of 124 children, who underwent cholecystectomy over 5 years was conducted. Data collected included age at onset, duration of symptoms, length of …

To verify the appropriateness of sphincterotomy as the treatment of choice of choledocholithiasis, since 1980 we have been using endoscopic retrograde cholangiopancreatographic (ERCP) manometry of the sphincter of Oddi (SO). This method allows direct investigation of SO motor activity and provides useful information regarding the presence of benign papillary stenosis (BPS). Thirty-four patients were investigated because the radiological examination indicated BPS might be present. Of these, 20 had common bile duct (CBD) stones, while the remaining 14 presented with biliarylike pain and one or m…

BACKGROUND: Microbial communities were recently revealed in the biliary tract of pancreaticobiliary disorders. However, evidence is limited and comparative data are lacking.
AIMS: We aimed to characterize the biliary microbiota in patients with naïve papilla affected by obstructive jaundice eligible for endoscopic treatment.
METHODS: 222 consecutive patients undergoing ERCP were prospectively enrolled from July 2022 to August 2023. Bile was sampled before and after sphincterotomy, then stored for cultures and resistance profiles.
RESULTS: Pre-sphincterotomy (66,6%) and post-sphincterotomy sa…

Three patients have been treated, with instillation of MTBE through a transcutaneous biliary drainage after transduodenal sphincterotomy. The first had gallbladder stones, the second common bile duct stones and the last intrahepatic stones (V segment). While in the first case there has been only a partial success in the other two we have obtained a total dissolution of the stones, with complete clinical recovery. This treatment has not caused immediate side effects but the absence of long term damage has not yet been proven. We think that the use of MTBE is not indicated in gallbladder stone d…

Reduced gallbladder ejection fraction (GBEF) with normal laboratory tests and no gallstones on ultrasound can be attributed to conditions such as biliary dyskinesia, chronic acalculous cholecystitis, cystic duct syndrome, sphincter of Oddi dysfunction, and subclinical biliary stricture. We present a case of a patient with chronic right upper quadrant pain, diagnosed with reduced GBEF, normal labs, and no gallstones. The patient's symptoms resolved after endoscopic retrograde cholangiopancreatography and sphincterotomy, revealing a mild, non-obstructive stricture in the middle third of the comm…

An anal fissure is a painful lesion of the anal canal. The acute form often heals spontaneously and should be treated conservatively. The treatment of chronic anal fissures is the subject of many studies but the treatment recommendations of international guidelines are contradictory. Conservative treatment consists of chemical sphincterotomy using sphincter-relaxing drugs, such as calcium channel antagonists, nitrates or botulinum toxin and leads to healing in only about 50% of patients with a prolonged healing time and a high rate of recurrence. In contrast surgical treatment is much more eff…

Lateral internal sphincterotomy is widely used in the treatment of chronic anal fissure. However, it is associated with a high rate of irreversible incontinence. For this reason the botulinum toxin has become a medical means of reversible sphincterotomy. Indeed, this neurotoxin induces relaxation of the smooth internal anal sphincter lasting one to three months after one injection. We reviewed the published studies about the use of this technique in the management of chronic anal fissure. Healing occurred in more than 70% of fissures without irreversible incontinence. Although further studies …

This report describes a guidewire fracture during an attempt at endoscopic treatment of pancreatic duct calculi in a patient with chronic pancreatitis, multiple pancreatic duct stenoses and pancreatic duct stones. This patient underwent endoscopic sphincterotomy in order to perform pancreatic duct drainage prior to ESWL. After sphincterotomy, a guidewire was introduced into the main pancreatic duct, but was jammed by the calculi. Neither could the guidewire be removed nor a contrast medium catheter be pushed over it, thus making ESWL impossible. A stronger pull resulted in a fracture of the wi…
