Cluster Analysis of a Database of Surgically Managed Adult Spine Deformity (ASD) Patients: Determining the Minimal Clinically Important Difference for Each Cluster, Comparing Two Clustering Approaches.

STUDY DESIGN: Retrospective analysis of prospectively collected data. OBJECTIVE: To investigate whether two clustering approaches applied to the same database would lead to differences in the minimal clinically important difference (MCID) for health-related quality of life parameters (HRQoL). SUMMARY OF BACKGROUND DATA: Machine learning approaches are being increasingly employed for the analysis of complex and heterogeneous settings such as that of adult spine deformity (ASD). However, it is not yet clear whether and how the choice of number and type of variables impacts the outcomes of a st

Changes in spinal curve during dentistry studies measured with a Spinal Mouse device: A five-year prospective study.

INTRODUCTION: Musculoskeletal disorders (MSDs) often arise and develop during dentistry studies. The most affected regions are related to the spine. Possible associations between spinal curve parameters and MSDs have not yet been investigated amongst dentistry students. This longitudinal observational study aimed to determine whether spinal curve changes during dentistry studies, analyse the relationship between objective findings and subjectively declared MSDs and compare spinal curve parameters with those published in the literature. MATERIALS AND METHODS: Seventy-three dentistry students a

The Importance of C2 Slope, a Singular Marker of Cervical Deformity, Correlates With Patient-reported Outcomes.

STUDY DESIGN: Retrospective review of a prospectively collected database. OBJECTIVE: To define a simplified singular measure of cervical deformity (CD), C2 slope (C2S), which correlates with postoperative outcomes. SUMMARY OF BACKGROUND DATA: Sagittal malalignment of the cervical spine, defined by the cervical sagittal vertical axis (cSVA) has been associated with poor outcomes following surgical correction of the deformity. There has been a proliferation of parameters to describe CD. This added complexity can lead to confusion in classifying, treating, and assessing outcomes of CD surgery.

Adult spinal deformity surgical decision-making score : Part 1: development and validation of a scoring system to guide the selection of treatment modalities for patients below 40 years with adult spinal deformity.

PURPOSE: We aimed to develop and internally validate a simple scoring system: the adult spinal deformity (ASD) surgical decision-making (ASD-SDM) score, which is specific to the decision-making process for ASD patients aged below 40 years. METHODS: A multicentre prospective ASD database was retrospectively reviewed. The scoring system was developed using data from a derivation cohort and was internally validated in a validation cohort. The accuracy of the ASD-SDM score was assessed using the area under the receiver operating characteristic curve (AUC). RESULTS: A total of 316 patients were r

Development and Validation of Prediction Equations for Spinal Curve Angles Based on Skin Surface Measurements.

OBJECTIVE: The purpose of this study was to develop, assess the reliability of, and validate prediction equations that estimate the sagittal curves of the spine from the skin surface. METHODS: Forty digital panoramic radiographs were used to develop the prediction equation, and 59 radiographs were used to assess reliability and validate the equations. For evaluation of the thoracic and lumbar curves, anatomical reference points were marked on the vertebral body, spinous process, and skin surface at the C6, C7, T2, T4, T6, T8, T10, T12, L2, L4, and S2 vertebrae. Total Hip Arthroplasty in the Spinal Deformity Population: Does Degree of Sagittal Deformity Affect Rates of Safe Zone Placement, Instability, or Revision?

BACKGROUND: Changes in spinal alignment and pelvic tilt alter acetabular orientation in predictable ways, which may have implications on stability of total hip arthroplasty (THA). Patients with sagittal spinal deformity represent a subset of patients who may be at particularly high risk of THA instability because of postural compensation for abnormal spinal alignment. METHODS: Using standing stereoradiography, we evaluated the spinopelvic parameters, acetabular cup anteversion, and inclination of 139 THAs in 107 patients with sagittal spinal deformity. Standing images were compared with supin

Spontaneous improvement of cervical alignment after correction of global sagittal balance following pedicle subtraction osteotomy.

OBJECT: Sagittal spinopelvic malalignment is a significant cause of pain and disability in patients with adult spinal deformity. Surgical correction of spinopelvic malalignment can result in compensatory changes in spinal alignment outside of the fused spinal segments. These compensatory changes, termed reciprocal changes, have been defined for thoracic and lumbar regions but not for the cervical spine. The object of this study was to evaluate postoperative reciprocal changes within the cervical spine following lumbar pedicle subtraction osteotomy (PSO). METHODS: This was a multicenter retros

The femoro-sacral posterior angle: an anatomical sagittal pelvic parameter usable with dome-shaped sacrum.

The sagittal pelvic morphology modulates the individual alignment of the spine. Anatomical angular parameters were described as follows: the "Pelvic Incidence" (PI) and the Jackson's angle "Pelvic Lordosis" (PR-S1). Significant chains of relationships were expressed connecting these angles with pelvic and spinal positional parameters. This allows an individual assessment of the harmony of the sagittal spinal balance. But in case of spondylolysis with high-grade listhesis, the upper plate of the sacrum shows a dome-shaped deformity. The previous anatomical parameters are therefore imprecise. In

Spinal deformity following selective dorsal rhizotomy.

The authors performed a review of 79 patients treated by selective dorsal rhizotomy with laminoplasty, 78 of whom were ambulatory, to determine the prevalence of spinal deformities. The mean radiographic follow-up was 4.2 years, the mean clinical follow-up 5.8 years. Scoliosis (11 degrees -24 degrees ) was identified in 13 children, none of whom had a preexisting deformity. There were no significant differences between preoperative and follow-up thoracic kyphosis or lumbar lordosis, although there was a significant difference in lumbar lordosis between sitting and standing radiographs. No prog

Familial aggregation in Rett syndrome: what is the evidence for clustering of other disorders in families of affected girls?

Rett syndrome is a neurodevelopmental disorder of unknown cause which affects girls almost exclusively. Apparently normal development in the first year of life is usually followed by loss of skills and the development of stereotypic hand movements. This study has used genetic epidemiological methods including a case control design to examine the evidence for aggregation of other disorders in families of girls with Rett syndrome. In one family there were two sisters with a condition consistent with Rett syndrome. Intellectual disability was not reported more commonly in case families (P = 0.46)

Idiopathic adolescent scoliosis and obesity: prevalence study.

PURPOSE: Adolescent idiopathic scoliosis (AIS) is a three-dimensional spinal deformity that affects 5% of the population. This pathology has multiple known etiological factors such as family predisposition, female gender, low body mass index, decrease in lean and fat masses. However, recent studies suggest that ciliary dysfunction could be the origin of certain types of obesity and AIS. This study aims to verify the existence of a link between these two pathologies. METHODS: A retrospective, cross-sectional, descriptive and monocentric study, based on a cohort of adolescents with obesity trea

[Scoliosis and spondylolisthesis (author's transl)].

The coincidence of a scoliosis with a lumbosacral spondylolysis or spondylolisthesis has remained largely ignored in the German language literature. After a survey of the foreign literature the pathogenesis of various combination forms is discussed. Primarily with the aid of oblique X-rays of lumbar scoliosis a scheme of classification involving 7 categories is worked out. The two main categories comprise unstable spondylolisthetic scolioses with increasing abnormal posture and scoliotic spondylolistheses. In these cases a lumbar scoliosis probably induces an asymmetric spondylolysis. The sche

The effect of orthotic treatment on plantar pressure distribution in patients with adolescent idiopathic scoliosis: A prospective observational study.

Orthotic treatment is one of the most effective nonoperative methods for preventing scoliosis progression, most studies have proved that spinal orthoses could control or decrease spinal curvature to below the surgery threshold. However, few studies have investigated whether the plantar pressure distribution of patients with scoliosis could be affected by spinal orthoses. Therefore, this study aims to investigate the effect of spinal orthosis on the plantar pressure distribution in adolescent idiopathic scoliosis (AIS). Thirty patients with AIS who met the eligibility criteria were screened and

How Does the Rib Cage Affect the Biomechanical Properties of the Thoracic Spine? A Systematic Literature Review.

The vast majority of previous experimental studies on the thoracic spine were performed without the entire rib cage, while significant contributive aspects regarding stability and motion behavior were shown in several other studies. The aim of this literature review was to pool and increase evidence on the effect of the rib cage on human thoracic spinal biomechanical characteristics by collating and interrelating previous experimental findings in order to support interpretations of in vitro and in silico studies disregarding the rib cage to create comparability and reproducibility for all stud
