Lateral stability of the proximal interphalangeal joint.

Current diagnostic criteria and therapeutic guidelines for injuries to the collateral ligaments of the proximal interphalangeal (PIP) joint are imprecise and vague. Laxity determinations, failure analysis, radiographic stress testing, and microscopic dissections were performed on 112 PIP joints. The lateral collateral ligament (LCL) is the primary restraint to varus and valgus angulation of the PIP joint. Its palmar fibers are tight in joint extension and provide the first line of resistance to lateral angulation. Failure of the LCL almost always occurs proximally in a sequential fashion that

Prevention of ankle sprains.

Two different methods for the prevention of ankle joint injuries in soccer were tested. Coordination training on an ankle disk improves functional stability and postural control, whereas an orthosis provides mechanical support. Both techniques reduce the frequency of ankle sprains in soccer players with previous ankle problems. The orthosis is an alternative to taping, and can be used during the rehabilitation period after injury or when playing on uneven ground. Coordination training on an ankle disk ought to be included in the rehabilitation of ankle injuries to prevent functional instabilit

Ulnar carpal instabilities.

Ulnar carpal instabilities are more common than previously suspected. To date, instability patterns have been described at the triquetrolunate and triquetrohamate joints. In this article, the pathomechanics, diagnosis, and treatment of these instability patterns are reviewed and several pertinent case reports are presented.

Non-articular rheumatism.

Non-articular rheumatism is one of the most common afflictions of humankind that arises anywhere in the musculoskeletal system (except the joints). Carefully differentiating non-articular rheumatism from arthritis or the generalized pain syndromes promotes effective management of some of the most common disorders of patients in a physician's practice.

Ankle sprains: surgical treatment for recurrent sprains. Report of 10 patients treated with the Chrisman-Snook modification of the Elmslie procedure.

A group of 10 patients (3 women and 7 men; 17 to 57 years old, average 23.9; mean, 21 years) was evaluated preoperatively and postoperatively following surgical repair with a modification of the Elmslie procedure (i.e., by using one-half of the tendinous portion of the peroneus brevis ligament to reconstruct the anterior and middle fasciculi of the lateral ligament). The patients were athletes who participated in bicycling, boxing, tennis, hockey, basketball, football, soccer, or a combination of sports. Each patient gave a history of spraining the ankle numerous times. Stress films were obtai

Early and late repair of lateral ligament of the ankle.

Utilizing an apparatus for separately testing the status of the anterior talofibular and the calaneofibular ligaments of the ankle in 25 healthy, 15- to 30-year-old adults, it became apparent that the stability of the ankle depends primarily upon the integrity of the anterior talofibular ligament. When the "fore n' aft" stress measurement exceeds 4 mm, a positive anterior drawer test is elicited, and the ankle ligament needs surgical repair. Tibial talar tilt normals ranged up to 18 degrees. Repair (early and late) is accomplished by suturing what one finds (there is always some ligament prese

Tibiofibular synostosis: a cause of ankle disability.

A syndrome of ankle pain on weight-bearing while running due to post-traumatic ossification of the tibiofibular ligament is described. Pain is caused by failure of normal downward and lateral motion of the fibula. Treatment consists of complete excision of the synostosis, followed by cast immobilization for 3 weeks.

Pulled elbow: a study of 100 patients.

Pulled elbow is a common condition but may not be recognized by practitioners unaware of its existence. Most of a series of 100 children with this condition were aged 1 or 2 years, and none were over 6 years. An important precipitating factor was the normal recalcitrance of the 1-3 age group. The three children who were over 5 years had all had earlier episodes which may have stretched the annular ligament. The wrist was the most common site of pain after the elbow. A single manipulation was successful in 96 cases but multiple manipulations may be needed.

Treatment of soft-tissue injuries by pulsed electrical energy.

Twenty pairs of patients matched for sex, age, weight, and degree of trauma were treated either with an active Diapulse machine emitting pulsed, high-frequency electrical energy or with a non-active placebo machine. They were examined for swelling, pain, and disability before treatment and after three days under double-blind conditions. The results show that the pulsed, high-frequency electrical treatment has a definite biological effect on recently injured soft tissues, especially in the reduction of pain and disability.
