Immunocytochemistry of mucosal changes in patients infected with the intestinal nematode Strongyloides stercoralis.

AIM: To investigate the immunopathological changes in duodenal tissues induced by strongyloidiasis and to relate these to degrees of clinical severity. METHODS: Tissues taken from 21 patients showing mild, moderate or severe symptoms of strongyloidiasis, and from non-infected controls, were sectioned and stained immunocytochemically for IgA, secretory component (SC) and HLA-DR. Immunopathology was assessed by changes in numbers, intensity and distribution of stained cells. RESULTS: Parasitised individuals showed villous atrophy and crypt hyperplasia. There was notable infiltration of the lam…

Rare Case of Strongyloides stercoralis Hyperinfection in a Greek Patient with Chronic Eosinophilia.

Strongyloidiasis is a disease characterized by a diverse spectrum of unspecific manifestations that complicate its diagnosis. Although, the course of its chronic form is usually benign, in cases of immunosuppression, iatrogenic or not, it can evolve to a hyperifection syndrome with even fatal complications. Herein, we report a case of Strongyloides stercoralis hyperinfection in a Greek patient receiving corticosteroid treatment for chronic eosinophilia and angioedema. The case represents an extremely rare case of autochthonous strongyloidiasis in Greece and underlines the importance of the ear…

[Anguilluliasis hyperinfection in a patient receiving corticosteroids: hepatic and biliary involvement].

A case of strongyloidiasis hyperinfection is reported in a 59 year-old man who presented cortico-dependent bronchial asthma. Several points of this case are uncommon: the long latency period (40 years), simultaneous aggravation of asthma requiring increasing doses of corticosteroids, and above all, liver and biliary tract involvement. Disseminated strongyloidiasis is widely believed to be precipitated by impairment of cell-mediated immunity. It is therefore unlikely that infestation of the liver and the biliary tract can interfere with hepatic function.

Phylogenetic Positioning of a Strongyloides stercoralis Isolate Recovered from a Korean Patient and Comparison with Other Asian Isolates.

Strongyloidiasis is caused by Strongyloides stercoralis and is one of the most neglected tropical diseases in tropical and subtropical regions. Although several strongyloidiasis cases have been reported in Korea, genetic analysis of Korean isolates is still incomplete. In this study, a parasite was isolated from a 61-year-old man diagnosed with strongyloidiasis during the treatment of lymphoma on his retroperitoneal lymph node. Diffuse symmetric wall thickening from the ascending to descending colon and a nematode-infected intestine was observed following microscopic examination. Genomic DNA w…

Strongyloides stercoralis in solid organ transplantation: early diagnosis gets the worm.

PURPOSE OF REVIEW: Strongyloidiasis is a parasitic infection affecting millions of people worldwide. Complications of infection are strongly associated with alcoholism, immunosuppression, and organ transplantation. Delayed diagnosis results in hyperinfection syndrome and disseminated strongyloidiasis leading to mortality rates approaching 80%. Early detection, and prevention of infection and transmission are key to diminish this illness. RECENT FINDINGS: In this review, we cover the basic concepts in immunity, immunosuppression, and disorder necessary for understanding the infectious syndrome…

Skin test antigens for immediate hypersensitivity prepared from infective larvae of Strongyloides stercoralis.

More rapid and simplified diagnostic procedures are needed for the diagnosis of strongyloidiasis. One approach is the use of an immediate hypersensitivity skin test that would reliably identify infected people. Accordingly, somatic and excretion/secretion (E/S) antigens were prepared from filariform larvae of Strongyloides stercoralis and were treated to remove possible adventitious agents. By use of a quantitative method for measurement of skin reactions, several preparations of the 2 antigens were tested in uninfected controls and in various groups of patients. Doses of 0.35 microg of E/S an…

Strongyloides stercoralis infection in a Spanish regional hospital: Not just an imported disease.

INTRODUCTION: Strongyloides stercoralis infection is more prevalent in tropical regions but autochthonous cases have been reported in Spain, mainly in La Safor (Valencia). The objective is to describe the strongyloidiasis cases registered in a regional hospital of Alicante province (Spain) and to determine if they were autochthonous cases. METHODS: Retrospective study of all diagnosed cases of strongyloidiasis in Vega Baja Hospital (Orihuela, Alicante) between January 1999 and March 2016. RESULTS: A total of 10 cases were found, four of which were autochthonous cases. Two of them presented w…

Syndrome of inappropriate secretion of antidiuretic hormone and nonpalpable purpura in a woman with Strongyloides stercoralis hyperinfection.

Strongyloidiasis stercoralis hyperinfection presenting as vasculitic-like skin lesions is rare. An autoinfection cycle allows intestinal strongyloidiasis, usually a benign infection, to persist for many decades. We report a woman with disseminated S stercoralis infection presenting as nonpalpable purpuric skin rash and syndrome of inappropriate secretion of antidiuretic hormone (SIADH). Upon admission, she was treated with corticosteroids for her vasculitic skin lesions, which then worsened her status. When the diagnosis was recognized, steroids were stopped, thiabendazole treatment was instit…

The diagnosis of human and companion animal Strongyloides stercoralis infection: Challenges and solutions. A scoping review.

Strongyloidiasis is the infection caused by soil-transmitted nematodes of Strongyloides species, infecting humans and some animals. Strongyloides stercoralis is the species with most clinical and epidemiological relevance in humans and dogs, due to its high prevalence and its capacity of inducing a life-threatening hyperinfection. Diagnosis of strongyloidiasis is challenging, due to the absence of a single reference standard test with high sensitivity and specificity, which also hampers the estimation of the accuracy of other diagnostic tests. In this chapter, we review the deployment and perf…

Histopathological confirmation of disseminated larvae (iL3) of Strongyloides ratti in an immunosuppressed Wistar rat.

Human strongyloidiasis research requires a large supply of Strongyloides stercoralis. This can be achieved through in vivo maintenance of Strongyloides stercoralis in Meriones unguiculatus, but isolating a large quantity of Strongyloides stercoralis to establish the colony from an infected patient is too difficult to achieve. Hence, Strongyloides ratti have been used as a model in human strongyloidiasis research. This study describes a successful establishment and maintenance of Strongyloides ratti infection in experimentally immunosuppressed Wistar rats. Large quantities of filariform (iL3) l…

Prevention of strongyloides hyperinfection syndrome: a rheumatological point of view.

BACKGROUND: Strongyloides stercoralis (S.stercoralis) is a parasite that infects humans and in conditions of immunodeficiency may disseminate, causing the potentially fatal strongyloides hyperinfection syndrome (SHS). The aim of this review was to investigate the literature evidence on the prophylaxis of SHS in immunosuppressed patients with rheumatological disorders. MATERIAL AND METHODS: The MEDLINE database (from 1966 to 2008) was searched using the following terms: "strongyloidiasis", "disseminated strongyloidiasis", "Strongyloides stercoralis", "Strongyloides stercoralis dissemination", …

Histopathology of gastric and duodenal Strongyloides stercoralis locations in fifteen immunocompromised subjects.

CONTEXT: Strongyloidiasis is a worldwide parasitic infection affecting approximately 75 million people. In Italy, it was more prevalent in the past among rural populations of irrigated areas. OBJECTIVE: To determine the histopathologic alterations of the gastric and duodenal mucosa associated with the presence of Strongyloides stercoralis parasites. DESIGN: Fifteen cases of strongyloidiasis were observed in immunocompromised patients during a recent 6-year period in Italy. S. stercoralis was found histologically in gastric biopsies (10 cases), in a gastrectomy (1 case), and in duodenal biops…

Parasitic disease screening among HIV patients from endemic countries in a Toronto clinic.

BACKGROUND: Many North American-based HIV patients originate from parasitic disease-endemic regions. Strongyloidiasis, schistosomiasis and filariasis are important due to their wide distribution and potential for severe morbidity. OBJECTIVES: To determine the prevalence, as determined by serological screening, of strongyloidiasis, schistosomiasis and filariasis among patients in an HIV-focused, primary care practice in Toronto, Ontario. A secondary objective was to determine factors associated with positive serological screens. METHODS: A retrospective review of electronic patient records wa…

The One Health approach to Strongyloides stercoralis.

The One Health concept systematically considers the health of humans, animals and the environment, emphasizing population and ecosystem health based on individual health. Strongyloides stercoralis is a soil-transmitted opportunistic pathogenic parasite that can cause strongyloidiasis in both humans and animals, potentially leading to severe disease or even death in immunocompromised hosts. This article outlines the epidemiological status, diagnosis and treatment methods of strongyloidiasis. It also discusses effective response strategies based on the One Health approach to actively cope with t…

[Parasite screening for immunosuppressed traveler].

Before starting immunosuppressive therapy, it is important to screen for latent tuberculosis and, in case of particular exposures (e.g. travel, origin,…), for parasitosis such as amoebiasis, echinococcosis, strongyloidiasis and American trypanosomiasis. The Division of Tropical and Humanitarian Medicine of the Geneva University Hospitals has developed an algorithm to identify which screening is recommended according to exposure. Analysis of this practice shows that the most frequently detected latent disease is tuberculosis, followed by strongyloidiasis. For the latter, as the sensitivity of t…

Case report of a computerized tomography sign in Strongyloides stercoralis infection.

This is a case of strongyloidiasis showing colon enhancement on a computerized tomography (CT) scan. The patient presented with chief complaints of diarrhea and abdominal pain. She gave a history of recent travel to El Salvador and her stool was positive for Strongyloides stercoralis. A CT scan revealed a circumferential enhancement of the sigmoid colon. This CT sign was unusual in strongyloidiasis and when combined with the symptoms, caused us to rule out inflammatory bowel disease and shock bowel. Stool examination revealed ova and parasites of Strongyloides stercoralis. She was then treated…

Opportunistic Strongyloides stercoralis infection in lymphoma patients. Report of a case and review of the literature.

Strongyloides stercoralis is an intestinal parasite that may cause fatal opportunistic infections in immunocompromised patients. Herein is reported a patient who developed fatal disseminated strongyloidiasis 6 weeks after the initiation of chemotherapy for a large cell lymphoma of the small intestine. After reviewing the clinical and epidemiologic features of 16 other cases of disseminated strongyloidiasis in patients with malignant lymphomas, the currently available laboratory methods for the diagnosis of this parasite are outlined. Because uncomplicated infections are treatable, candidates f…

Mixed-method analysis of screening for Strongyloides stercoralis prior to immunosuppression: a problem of limited bandwidth?

BACKGROUND: Guidelines recommend screening for strongyloidiasis prior to immunosuppression in those at epidemiological risk, as hyperinfection following immunosuppression is often fatal. The uptake of this recommendation is unknown and we aimed to explore this in our setting. AIMS: To determine the proportion of adult patients at epidemiological risk for strongyloidiasis who were screened prior to immunosuppression at the Royal Melbourne Hospital, and to explore the factors that influenced clinicians' decision to screen for strongyloidiasis prior to immunosuppression. METHODS: This study use…

Prevalence of Strongyloides in Southeast Asia: a systematic review and meta-analysis with implications for public health and sustainable control strategies.

BACKGROUND: Strongyloidiasis, caused by the nematodes Strongyloides stercoralis and Strongyloides fuelleborni, is estimated to affect over 600 million individuals worldwide. The disease is endemic in Southeast Asia, where a warm-humid climate and socio-economic conditions maintain the parasite's life cycle and transmission. However, the current diagnostic methods may not be sufficiently sensitive, suggesting that the true prevalence of strongyloidiasis could be seriously underestimated in this. This study aims to determine the prevalence of strongyloidiasis in Southeast Asia through a systemat…
