Upon inducting general anesthesia in the operating room, we have observed a prompt increase in the bispectral index (BIS) after the intravenous injection of suxamethonium. We hypothesized that the cause of this BIS increase is muscle hyperactivity owing to fasciculation. Eight Indian tribes from the Amazon region of Brazil (Araweté, Arara, Yamamadi, Kararaô, Karitiana, Waiampi, Surui and Cinta Larga) were studied for the distribution of the atypical and C5 variants of serum cholinesterase. Complications due to intratracheal jet ventilation are often caused by the unrecognized recovery of laryngeal muscles. The rate of rise of pressure in a main bronchus alters with changes of the glottic aperture during jet ventilation. Suxamethonium has not yet been replaced to ease endotracheal intubation despite its many undesirable side effects. Hyperkalemia and muscle pain are two such side effects; they are not reliably prevented by giving, before suxamethonium, a small dose of a non depolarizing muscle relaxant, although it does decrease muscle fasciculations. All physicians performing surgery while the patient is under general anesthesia must be aware of the existence of idiopathic malignant hyperthermia. A young adult had a malignant hyperthermic reaction despite four prior uneventful administrations of general anesthetics. To my knowledge, this is the first report of a patient who survived following a hyperthermic reaction, having sustained considerable residual brain damage. There is a great need for continuous body-temperature monitoring during general anesthesia.

A non-lethal procedure for identifying pigs apt to develop malignant hyperthermia is described.
