Do Transsacral-transiliac Screws Across Uninjured Sacroiliac Joints Affect Pain and Functional Outcomes in Trauma Patients?

BACKGROUND: Patients with pelvic ring displacement and instability can benefit from surgical reduction and instrumentation to stabilize the pelvis and improve functional outcomes. Current treatments include iliosacral screw or transsacral-transiliac screw, which provides greater biomechanical stability. However, controversy exists regarding the effects of placement of a screw across an uninjured sacroiliac joint for pelvis stabilization after trauma. QUESTIONS/PURPOSES: Does transsacral-transiliac screw fixation of an uninjured sacroiliac joint increase pain and worsen functional outcomes at

Flexible Intramedullary Nailing of Pediatric Femoral Fractures.

BACKGROUND: Flexible intramedullary nailing is performed for femoral fractures in pediatric patients who may be too large for spica cast immobilization but who have substantial growth remaining and who are not a candidate for rigid intramedullary nailing. Flexible nailing allows the surgeon to obtain correct alignment of the femur fracture so as to allow for healing without a lower extremity deformity. DESCRIPTION: The patient is positioned on a radiolucent table, flexible nails are chosen according to the diameter of the medullary canal, medial and lateral incisions are made along the distal

Surgical Technique for the Implantation of a Biomimetic Artificial Intervertebral Disc in a Goat Animal Model.

Cervical disc arthroplasty (CDA) has emerged as a surgical solution for patients with degenerative compressive cervical myelopathy but faces limitations, particularly in achieving stable integration. One of the primary challenges with current artificial discs is the risk of migration and dislocation due to insufficient osseointegration with the adjacent vertebrae. While CDA is an established treatment in human medicine, dogs with cervical myelopathy are routinely treated with decompressive surgeries. In veterinary medicine, there is particular interest in innovative therapies, such as disc art

Difficulty of Total Hip Arthroplasty Following Open Reduction and Internal Fixation of Acetabular Fractures.

BACKGROUND: The incidence of posttraumatic arthrosis after acetabular fractures is significant, and patients frequently require secondary total hip arthroplasty. Conversion arthroplasty is more technically difficult, and there is higher risk than with routine primary total hip arthroplasty. The goal of this study was to identify the challenges and risks of secondary total hip arthroplasty compared to primary total hip arthroplasty. METHODS: We retrospectively identified 30 patients who underwent secondary total hip arthroplasty after open reduction and internal fixation of an acetabulum fract

Understanding the implant performance of magnetically controlled growing spine rods: a review article.

PURPOSE: Early-Onset Scoliosis (EOS) (defined as a curvature of the spine ≥ 10° with onset before 10 years of age) if not properly treated, can lead to increased morbidity and mortality. Traditionally Growing Rods (TGRs), implants fixated to the spine and extended every 6-8 months by surgery, are considered the gold standard, but Magnetically Controlled Growing Rods (MCGRs) avoid multiple surgeries. While the potential benefit of outpatient distraction procedure with MCGR is huge, concerns still remain about its risks, up to the release of a Medical Device Alert (MDA) by the Medicines and Heal

Pathological Fracture of Calcaneum: A Case Report.

INTRODUCTION: Only a few reports have described calcaneus fractures after total knee arthroplasty (TKA). Therefore, in this report, we describe a case of calcaneal avulsion fracture that occurred 5 weeks after a TKA in a relatively young male patient with syphilis. CASE REPORT: A 63-year-old man with syphilis had Charcot joint of the right knee. The patient developed severe varus deformity and contracture and experienced severe pain in the knee. TKA was performed to alleviate the pain and improve the patient's gait. The patient noticed slight heel pain 4 weeks after the TKA, and a calcaneus a

The Use of Brainlab Navigation in Le Fort III Osteotomy.

Le Fort III osteotomy is commonly used in the surgical correction of midface hypoplasia, specifically in patients with syndromic craniosynostosis. These osteotomies can be associated with significant complications, which are often the result of incomplete or inaccurate osteotomies. Brainlab, a technology first developed for neurosurgery, has been applied to numerous surgical subspecialties. The aim of this study was to report our initial experience using the Brainlab VectorVision2 and Brainlab Curve (Brainlab, Westchester, IL) as an intraoperative guidance system for osteotomy placement during

W. James Gardner: pioneer neurosurgeon and inventor.

W. James Gardner, a skillful neurosurgeon and inventor, is best remembered for his cervical tongs and hydrodynamic theory of syringomyelia. A pioneer of modern neurosurgery, Gardner trained under Charles Frazier in Philadelphia, and in 1929 he moved to Ohio where he became chief of neurosurgery at the Cleveland Clinic, a position he was to hold for the next 33 years. A large surgical practice made it imperative for Gardner to develop surgical methods that were quick, effective, and advantageous for patient and surgeon. He was an early proponent of the sitting position for patients undergoing c

Revision of an Eight-lead Boston Scientific® Spinal Cord Stimulator With Epidural Migration: A Case Report.

Spinal cord stimulation (SCS) is a minimally invasive neuromodulation technique utilized to manage chronic pain conditions such as complex regional pain syndrome (CRPS), post-laminectomy syndrome, and peripheral neuropathy. Despite its efficacy, complications, including lead migration, discomfort, and device malfunction, can occur. We present the case of a male patient in his 60s with a history of chronic left lower extremity pain who underwent SCS implantation. Initial management included a successful trial period followed by permanent implantation. However, postoperative imaging revealed lea

Early results of an intraosseous device for arthrodesis of the hallux metatarsophalangeal joint.

BACKGROUND: Arthrodesis of the hallux metatarsophalangeal (MTP) joint is commonly done as a primary procedure either to correct severe hallux valgus deformities or for rheumatoid arthritis, hallux rigidus, in patients with neuromuscular disorders and as a salvage procedure for failed bunion surgery or infection. Prominent metalwork frequently can cause soft tissue impingement and thus require removal. In contrast, osteosynthesis with a completely intraosseous implant has the advantage of less damage to the periosteal circulation. We describe a surgical technique and the early results of arthro

[Spontaneous elimination by the natural tracts of screws of anterior cervical osteosynthesis. Apropos of a case].

PURPOSE OF THE STUDY: The authors report a case of a wellknown but very unusual complication of cervical spine anterior osteosynthesis: spontaneous recurrent elimination of anterior fixation device through the gastrointestinal track, with good outcome. MATERIALS AND METHODS: The patient (75 years old) was operated on for cervical myelopathia due to cervical stenosis. Surgical treatment included an anterior release with corporectomy of C4, C5 and C6 and iliac graft insertion, and fixation using plate and screws. Immediate post-operative course was uneventful. Plate incurvation and rupture were

Local repair of a trans-stomal ileocecal prolapse by stapler device.

The creation of a stoma, although considered a simple surgical procedure, is burdened by various complications, one of the most common being prolapse. Usually prolapse increases gradually, involving generally limited functional disorders and it can be well managed with non-operative measures. Occasionally it requires major surgery, for severe skin excoriation, bleeding or incarceration of the bowel. However, for critical clinical conditions of patients, often is not possible to use classical techniques, as bowel-pexy fixation or resection of the prolapse. Maeda has described a simple method fo

Increased use of intramedullary nails for intertrochanteric proximal femoral fractures in veterans affairs hospitals: a comparative effectiveness study.

BACKGROUND: Intramedullary nails for stabilizing intertrochanteric proximal femoral fractures have been available since the early 1990s. The nails are inserted percutaneously and have theoretical mechanical advantages over plates and screws, but they have not been demonstrated to improve patient outcomes. Still, use of intramedullary nails is becoming more common. The goal of this study was to examine trends in the use and associated outcomes of intramedullary nailing compared with sliding hip screws in Veterans Affairs (VA) hospitals. METHODS: Review of the VA Surgical Quality Improvement Pr

[Application of the Alexis O wound protector/retractor ring as an alternative silo system in the management of gastroschisis].

Gastroschisis is a congenital abdominal wall defect associated with extraabdominal herniation of the bowels and, rarely, other abdominal organs. In cases of severe visceroabdominal disproportion, primary abdominal wall closure may not be feasible, therefore staged reduction using silo systems is required. The use of the Alexis wound protector/retractor as an alternative silo system in gastroschisis was first described by Kusafuka et al. in 2005. We present the case of a restricted neonate born at 38 weeks of gestation with prenatally diagnosed gastroschisis. Due to the hypoplastic abdominal ca
