Markers of inflammation in sepsis.

Pathophysiology of sepsis is characterised by a whole body inflammatory reaction and concurrent activation of the host's anti-inflammatory mechanisms. The balance between pro- and anti-inflammatory reactions is critical for the outcome of the patient. Strongly activated phagocytes and high levels of proinflammatory cytokines occur in patients who are at risk of developing circulatory shock and multiple organ dysfunction. Extensive anti-inflammatory reaction, which is characterised by the presence of high levels of circulating anti-inflammatory cytokines and impaired innate and adaptive immune

[Atherothrombosis and inflammation].

Alterations of coagulation, fibrinolysis, platelets and low grade inflammation are causal pathophysiological factors in atherosclerosis. Considerable activation of several involved pathways occurs during the acute progression of atherosclerotic lesions, which is characterized by an occluding thrombus, and local and systemic inflammatory reactions as in patients with acute coronary syndromes. These patients become clinically compromised due to the reduction in coronary flow. Furthermore, a frequent occurrence of non-occluding thrombi may be assumed as a progression factor in atherosclerotic dis

Artificial nutrition in pancreatic disease: what lessons have we learned from the literature?

Acute pancreatitis is a disease process that begins with an initial injury to the pancreatic acinar cell due to the erroneous premature activation and intracellular release of digestive enzymes. The local injury is amplified through the induction of a systemic inflammatory response, mediated by the generation and release of cytokines and an aggressive inflammatory cell recruitment. Failure to maintain gut integrity may exacerbate the stress response and the systemic inflammatory reaction associated with this process, worsening the overall clinical severity of the pancreatitis and contributing

[Pharmacologic aspects of polyunsaturated fatty acids in parenteral nutrition].

Despite immense progress in intensive-care medicine, mortality rates of 30-70% in sepsis and SIRS are still an unresolved problem. Particularly the failure of respiratory and other vital functions is a major cause of death. Besides infectious stimuli (viruses, bacteria, fungi) a variety of non-infectious triggers (tissue damage, immune complexes, complement activation, etc.) can initiate the development of organ failure. These inflammatory reactions aim physiologically towards inactivation and removal of the stimulating agents as well as the induction of reparative processes. In states of prol

Gut barrier dysfunction, endotoxemia and inflammatory response in STEMI patients and effect of primary PCI.

BACKGROUND: Gut-derived bacterial and endotoxin translocation induce systemic inflammation, which exerts a pivotal pathogenetic role in all phases of atherosclerosis. OBJECTIVES: To investigate prospectively the gut barrier function, endotoxin translocation and inflammatory response in ST-elevation myocardial infarction (STEMI) patients undergoing primary percutaneous coronary artery intervention (PPCI). METHODS: Twenty-seven patients with STEMI that underwent successful PPCI were subjected to peripheral blood sampling at 3-time points; before PPCI (day0), 24 h (day1) and 96 h (day4) after P

[New clinical entity in perinatology: fetal inflammatory response syndrome].

In spite of the adequate therapy serious inflammatory complications affect the majority of preterm neonates. Recently, the role of fetal inflammatory response syndrome has been emerged as a possible mechanism. This new clinical entity is characterised by the generalised activation of fetal immune system which is often the result of chorioamnionitis. The level of inflammatory cytokines (interleukin-6) often elevates in fetuses affected by this syndrome. Elevated cytokine levels induce the infiltration of inflammatory cells into the amnion and activate matrix metalloproteases in the fetus. As a

Research Status on the Effects of Obesity on Acute Pancreatitis.

Acute pancreatitis (AP) is an inflammatory disease characterized by the premature activation of trypsinogen. Mild acute pancreatitis is self-limiting; however, approximately 15-20% of patients progress to severe acute pancreatitis (SAP), which is associated with pancreatic tissue necrosis, systemic inflammatory response syndrome, organ failure, and a mortality rate of up to 30%. Obesity, a growing global public health concern with increasing yearly incidence worldwide, is an established independent risk factor for the development and exacerbation of AP. This review provides a comprehensive sum…

Release of lipopolysaccharide toxicity-modulating proteins in patients undergoing cardiopulmonary bypass using noncoated and heparin-coated extracorporeal circuits. A clinical pilot study.

STUDY OBJECTIVE: Cardiopulmonary bypass (CPB) induces a generalized inflammatory response, including activation of leukocytes, contributing to postoperative morbidity. The inflammatory pathways leading to this systemic inflammatory response syndrome are considered identical to those involved in septic shock. Therefore, we studied the release of bactericidal/permeability-increasing protein (BPI), lipopolysaccharide binding protein (LBP), and soluble CD14 (sCD14)-all proteins that modulate the effects of lipopolysaccharide (LPS)-in patients undergoing CPB. In addition, the effect of heparin coat…

Liver-lung interactions in critical illness.

Organ interactions are increasingly recognized as key determinants of the pathogenesis and potential for resolution of tissue injury during critical illness. A paradigm for a systems model that takes into account the modulatory effects of organ interactions and incorporates the expanding number of molecular and cellular pro-inflammatory networks is still lacking. Unifying hypotheses for multiple organ dysfunction during the systemic inflammatory response syndrome have been slow to emerge. The liver plays a central role in the regulation of multiple host defense, immunologic, biochemical, and m…

Role of inflammasomes in inflammatory autoimmune rheumatic diseases.

Inflammasomes are intracellular multiprotein complexes that coordinate anti-pathogenic host defense during inflammatory responses in myeloid cells, especially macrophages. Inflammasome activation leads to activation of caspase-1, resulting in the induction of pyroptosis and the secretion of pro-inflammatory cytokines including interleukin (IL)-1β and IL-18. Although the inflammatory response is an innate host defense mechanism, chronic inflammation is the main cause of rheumatic diseases, such as rheumatoid arthritis (RA), systemic lupus erythematosus (SLE), ankylosing spondylitis (AS), and Sj

Antithrombotic agents in the treatment of severe sepsis.

Sepsis, a systemic inflammatory syndrome, is a response to infection and when associated with multiple organ dysfunction is termed severe sepsis. It remains a leading cause of mortality in the critically ill. The response to the invading microorganisms may be considered as a balance between a pro-inflammatory and an anti-inflammatory reaction. While an inadequate pro-inflammatory reaction and a strong anti-inflammatory response could lead to overwhelming infection and the death of the patient, a strong and uncontrolled pro-inflammatory response, manifested by the release of pro-inflammatory me

The mouse cyclophosphamide model of bladder pain syndrome: tissue characterization, immune profiling, and relationship to metabotropic glutamate receptors.

Abstract Painful bladder syndrome/Interstitial cystitis (PBS/IC) is a chronic disorder characterized clinically by recurring episodes of pelvic pain and increased urination frequency, significantly impairing patients' quality of life. Despite this, there is an unmet medical need in terms of effective diagnostics and treatment. Animal models are crucial in this endeavor. Systemic chronic administration of cyclophosphamide (CYP) in mice has been proposed as a relevant preclinical model of chronic bladder pain. However, molecular mechanisms underlying the pathogenesis of this model are lacking. H
