Current management of laryngeal and laryngotracheoesophageal clefts.

Laryngeal and laryngotracheoesophageal clefts (L-LTEC) are uncommon anomalies in neonates that cause significant morbidity secondary to aspi … ible with microsurgical instrumentation. With types II to IV L-LTEC, an open approach must be used. Tracheotomy is a universal requirement, often for extended periods of time. Reported here is our experience with four cases of L-LTEC managed over the past 7 years at Texas Children's Hospital.

Endoscopic laser repair of posterior glottic, subglottic and tracheal stenosis by division or micro-trapdoor flap.

Laryngeal and tracheal stenosis have been refractory to a wide variety of treatments including dilation, stents, or have required major open … ent of the patients in the group studied obtained an adequate airway with good voice quality and no tracheotomy was required in those patients not already having one. In 19 patients there was no mortality and essentially no morbidity.

Diagnosis and therapy for acute laryngeal and tracheal trauma.

A laryngeal fracture should be suspected when there is hemoptysis and subcutaneous emphysema following blunt injury to the neck. Computed tomography of the neck should be used to define the extent of the injury. Cervical vertebral fractures and dislocations, perforation of the pharynx and esophagus, and vascular injuries must be excluded. Establishment of a secure airway by tracheotomy, avoidance of flexion or extension of the neck until cervical vertebral injuries are excluded, and evaluation for recurrent laryngeal paralysis are of great importance. The repair of the fractured larynx require…

Diagnosis and repair of traumatic tracheal and tracheobronchial disruptions.

During the period from January 1968 to December 1982, 8 patients received emergency operations for traumatic tracheobronchial rupture in the Department of Thoracic Surgery, Odense University Hospital. Half of the lesions were the result of blunt trauma; the other half were penetrating injuries or iatrogenic lesions. All of them were treated within 24 hours by direct suture of the lesion. In our experience soft, rapidly absorbable sutures should be used in the cartilaginous and membraneous parts of the larynx and trachea, and tracheotomy should be avoided where at all possible. Using these prin…

Separation of the larynx and trachea for intractable aspiration.

Aspiration becomes an intractable problem for some patients who have lost normal neurological function of the glottis. Over the long term, a cuffed tracheotomy tube is inadequate. Several methods have been developed to relieve this problem surgically. These include the use of muscle flaps, tracheoesophageal anastomosis, and use of an epiglottic flap. In this paper, we discuss these previous surgical attempts to alleviate aspiration and present our own technique, which involves creation of a tracheostome and closure of the larynx at the level of the first tracheal ring. This has been used with …

A Case of Displaced Cricoid Cartilage Fracture Successfully Treated With Non-Surgical Intervention.

Laryngeal trauma, particularly cricoid fractures, is rare but potentially life-threatening and often requires invasive airway management. However, factors contributing to successful conservative treatment remain unclear. We report the case of a 40-year-old woman diagnosed with a displaced cricoid cartilage fracture following a motorcycle accident. Despite the risk of upper airway obstruction due to a massive hematoma observed on fiberoptic laryngoscopy and a displaced cricoid cartilage fracture, the patient was successfully managed conservatively without tracheotomy or endotracheal intubation …

Paraneoplastic bilateral vocal cord paralysis in a patient with anti-Hu syndrome after receiving nivolumab: Case report and literature review.

Bilateral vocal cord paralysis (BVCP), a life-threatening condition often necessitating tracheotomy, has been infrequently reported as a paraneoplastic manifestation. We present the case of a 75-year-old male with small cell lung cancer who experienced distinct episodes of paraneoplastic neurological syndromes. The initial episode, occurring concomitant with the diagnosis of cancer and initiation of chemotherapy, was characterized by epileptic seizures and ataxia, which resolved following treatment with immunotherapy. The subsequent episode, presenting as BVCP, developed after the fourth dose …

How to choose a craniotomy for the intracranial foreign bodies when the airway was occupied: a case report.

BACKGROUND: Intracranial foreign bodies caused by trauma are rare in clinical emergency cases. Similar reports in the past pay less attention to establish artificial ventilation for these patients for the following anesthesia and surgery. CASE INFORMATION: We reported a 3-year-old boy who suffered from oral penetration of the cerebellum from the skull base by a chopstick after a fall. To avoid pediatric tracheotomy, we conducted a reconstruction analysis by spiral CT scan and found a transnasal tracheal intubation pathway allowing the endotracheal tube through, successfully established artifi…

[Successful Use of the i-gel and Dexmedetomidine for Tracheal Resection and Construction Surgery in a Patient with Severe Tracheal Stenosis].

A 51-year-old man, 170 cm, 86 kg, was diagnosed with a tracheal tumor existing just below the glottis occupying more than 80% of his tracheal lumen, and was scheduled for tracheal resection and construction. The patient had a strider due to the severe tracheal stenosis. We could insert i-gel easily under dexmedetomidine sedation. After successful i-gel insertion, tracheotomy and endotracheal intubation were performed. Until ETT insertion, the patient maintained spontaneous respiration without any hypoxic event. Followed by ETT insertion, tracheal resection and construction were performed under…

[Chronic respiratory failure: the role of home mechanical ventilation].

While negative pressure ventilation using cuirass respirators or iron-lung machines was prevailing in the first part of the 20th century, the polio epidemic in Copenhagen 1952 marks the turning point at which positive pressure ventilation following tracheotomy was started. Furthermore, following the introduction of facial masks and starting 1985 in Germany non-invasive positive pressure ventilation has meanwhile been developed as a routine procedure for the long-term treatment of patients with chronic ventilatory failure today. The current article provides an overview of these developments and…

[Laryngomucoceles and subtotal C.H.E.P. reconstructive laryngectomy. Report of 6 cases].

UNLABELLED: We conducted a retrospective analysis of 131 patients who underwent subtotal CHEP laryngectomy for a tumor of the larynx between 1990 and 1999 in order to determine the incidence of laryngomucocele after surgery. RESULTS: Five patients developed laryngomucocele late after surgery. A sixth patient underwent surgery in another unit. Laryngomucocele developed progressively or was disclosed by acute episodes of dyspnea, requiring tracheotomy again in two cases. One patient developed bilateral mucocele. Three patients had cervicotomy, and three others were treated by CO(2) laser endosc…

Delayed airway obstruction and neck abscess following manual strangulation injury.

Few reports describing manual strangulation injury to the neck are found in the otolaryngologic literature. Since most victims sustain immed … lowing manual strangulation developed acute upper airway obstruction and neck abscess necessitating tracheotomy, neck exploration, and drainage. Patients suffering this unique type of compression injury may present initially with deceptively benign symptoms and signs. Total cervical tracheal stenosis: treatment by laser, dilation, and stenting.

Twelve patients with total cervical tracheal stenosis were treated by endoscopic laser excision (neodymium:yttrium aluminum garnet or carbon dioxide laser), bronchoscopic dilation, and prolonged stenting with a silicone T-tube. All patients had previous traumatic or prolonged endotracheal intubation requiring a tracheotomy and presented with aphonia as the major complaint. Multiple laser and dilation treatments were necessary in ten patients. Average duration of T-tube placement was 6 months. Excellent results (decannulation and good voice) were achieved in eight patients with a follow-up of 9…

Laryngeal problems following infant esophageal surgery.

This paper focuses on vocal cord paralysis in children after operation for tracheoesophageal fistula and/or esophageal atresia. We reviewed the charts of 65 children who were operated upon for congenital esophageal atresia and/or tracheoesophageal fistula during a period from 1964 to 1974. Ten of these children manifested laryngeal symptoms. Five had laryngeal paralysis. Two were untreated. One was treated with cordectomy and stent. Two were treated successfully with the Thornell arytenoidectomy, one of these being done without a tracheotomy in place. Mention is made of a third case of bilater…

Survival in Vivo Canine Phonation Model Without Stimulation.

OBJECTIVES: We describe a survival nonstimulated in vivo canine phonation model using distending laryngoscope, cramp frame, and constant humidified glottal airflow to elicit phonation. METHODS: Five beagle dogs were involved in this study. One cuffed endotracheal tube was placed below the glottis through the tracheotomy and delivered humidified airflow to the glottis. Arytenoids approximation was maintained using a clamp under the distending laryngoscope. Acoustic and aerodynamic parameters were measured using synchronous signal collection system and analysis software. Vocal oscillation also …

Tracheomegaly: a complication of fetal endoscopic tracheal occlusion in the treatment of congenital diaphragmatic hernia.

BACKGROUND: Fetal endoscopic tracheal occlusion (FETO) is a promising treatment for severe congenital diaphragmatic hernia, a condition that carries significant morbidity and mortality. It is hypothesised that balloon occlusion of the fetal trachea leads to an improvement in lung growth and development. The major documented complications of FETO to date are related to preterm delivery. OBJECTIVE: To report a series of five infants who developed tracheomegaly following FETO. MATERIALS AND METHODS: Review of all children referred with tracheomegaly to the paediatric intensive care and tracheal

Airway management in Nager Syndrome.

Nager acrofacial dysostosis is a rare congenital syndrome characterized by malformed mandibulofacial structures and pre-axial upper limbs. Trismus and glossoptosis from mandibular abnormalities predisposes infants to life-threatening respiratory distress. A case of a Nager Syndrome mother delivering a similarly afflicted fetus is presented, with approaches to maintaining both tenuous airways described. Distinguishing this condition from similar syndromes is critical for care and prognosis.
