BACKGROUND: Degenerative spinal stenosis and instability requiring multilevel spine surgery has been associated with large blood losses. Factors that affect perioperative blood loss include time of surgery, surgical procedure, patient height, combined anterior/posterior approaches, number of levels fused, blood salvage techniques, and the use of anti-fibrinolytic medications. This study was done to evaluate the efficacy of tranexamic acid in reducing blood loss in spine surgery.
METHODS: This retrospective case control study includes 97 patients who had to undergo surgery because of degenerat…

BACKGROUND: Hemorrhagic acute brain injury-encompassing traumatic brain injury, spontaneous intracerebral hemorrhage, and subarachnoid hemorrhage-carries substantial morbidity and mortality. Active hemorrhage and hematoma progression constitute shared pathological mechanisms across these conditions. As a lysine analog anti-fibrinolytic, tranexamic acid (TXA) has demonstrated mortality reduction in extracranial bleeding; however, its efficacy and safety in intracranial hemorrhagic lesions remain inconclusive. Prior systematic reviews predominantly targeted single disease categories, inadequatel…

BACKGROUND AND OBJECTIVES: Tranexamic acid (TXA) as an adjunct to prophylactic platelet transfusion is sometimes used to prevent bleeding in patients with malignancies experiencing chemotherapy-induced thrombocytopenia. However, there is little biological evidence in support. This pilot exploratory study aimed to evaluate the haemostatic efficacy of TXA before and after platelet transfusion versus platelet transfusion alone in patients with haematological malignancies experiencing chemotherapy-induced thrombocytopenia.
MATERIALS AND METHODS: Rotational thromboelastometry (ROTEM; EXTEM, tissue…

INTRODUCTION: Hemarthrosis, particularly in the knee, accounts for most bleeding episodes in haemophilia. While joint aspiration has proven effective, the role of intra-articular (IA) tranexamic acid (TXA) in managing acute hemarthrosis remains unexplored.
AIM: To assess the efficacy and safety of knee aspiration followed by IA TXA injection in acute haemophilic knee hemarthrosis.
METHODS: Forty-four adult haemophilia patients with acute knee hemarthrosis (< 24 h) were randomized to undergo joint aspiration with (TXA group) or without (non-TXA group) IA TXA (1.5 g/15 mL) injection. Both grou…

BACKGROUND: Tranexamic acid, given within 3 h of birth, reduces bleeding deaths in women with postpartum haemorrhage. We examined whether giving tranexamic acid shortly after birth can prevent postpartum haemorrhage in women with moderate or severe anaemia.
METHODS: This international, randomised, double-blind, placebo-controlled trial was done in 34 hospitals across four countries (Nigeria, Pakistan, Tanzania, and Zambia). We recruited women of any age in active labour with moderate or severe anaemia (haemoglobin <100 g/L). We randomly assigned women (1:1) who had given birth vaginally to re…

Background: Postpartum haemorrhage (PPH) is responsible for over 50,000 maternal deaths every year. Most of these deaths are in low- and middle-income countries. Tranexamic acid (TXA) reduces bleeding by inhibiting the enzymatic breakdown of fibrin blood clots. TXA decreases surgical bleeding and reduces deaths from bleeding after traumatic injury. When given within three hours of birth, TXA reduces deaths from bleeding in women with PPH. However, for many women, treatment of PPH is too late to prevent death. World-wide, over one-third of pregnant women are anaemic and many are severely anaemi…

BACKGROUND: Tranexamic acid (TXA) is often used to prevent excessive blood loss during bilateral TKA. Although it diminishes blood loss, TXA may have a potentially elevated thrombogenic risk with extra, unnecessary doses of TXA in this high-risk population. To date, the most efficacious dosing protocol in this setting has not yet been ascertained.
QUESTIONS/PURPOSES: We compared one versus two doses of intravenous TXA in the setting of same-day bilateral TKA in terms of (1) perioperative blood loss that occurred during the hospital stay, (2) transfusion usage during the hospital stay, and (3)…

BACKGROUND: The role of tranexamic acid (TXA) in controlling blood loss during spine surgery remains unclear. With the publication of new randomized controlled trials (RCTs), we conducted a meta-analysis to determine the safety and efficacy of TXA in spine surgery.
METHODS: PubMed, Embase, Web of Science, and Cochrane databases were searched for relevant studies through 2022. Only RCTs were eligible for this study. The extracted data were analyzed using RevMan 5.3 software for meta-analysis.
RESULTS: Twenty RCTs including 1497 patients undergoing spine surgery were included in this systemati…

BACKGROUND: Trauma patients with hyperfibrinolysis and depletion of fibrinolytic inhibitors (DFIs) measured by thrombelastography (TEG) gain clot strength with TXA, but TEG results take nearly an hour. We aimed to develop an assay, plasmin TEG (P-TEG), to more expeditiously stratify risk for massive transfusion (MT), mortality, and hyperfibrinolysis.
METHODS: Trauma patients (N = 148) were assessed using TEG assays without exogenous additives (rapid/native), with exogenous plasmin (P-TEG) or tissue plasminogen activator (tPA TEG). The plasmin dose used does not effect healthy-control clot lys

BACKGROUND: Early tranexamic acid (TXA) treatment reduces head injury deaths after traumatic brain injury (TBI). We used brain scans that were acquired as part of the routine clinical practice during the CRASH-3 trial (before unblinding) to examine the mechanism of action of TXA in TBI. Specifically, we explored the potential effects of TXA on intracranial haemorrhage and infarction.
METHODS: This is a prospective substudy nested within the CRASH-3 trial, a randomised placebo-controlled trial of TXA (loading dose 1 g over 10 min, then 1 g infusion over 8 hours) in patients with isolated head …

INTRODUCTION: Hemorrhagic syndrome accounts for a significant proportion of the mortality and morbidity associated with snakebites in sub-Saharan Africa. Hyperfibrinolysis, which has been reported in some cases, suggests the potential benefit of antifibrinolytics; however, these have been little studied in this context. This study aimed to evaluate the effectiveness of combining tranexamic acid and antivenom in treating hemorrhagic syndrome and coagulopathy associated with Echis ocellatus (West African Carpet Viper) envenomation.
MATERIALS AND METHODS: Patients presenting with hemorrhagic syn…

Aneurysmal subarachnoid hemorrhage (aSAH) represents one of the acute neurological conditions associated with the highest morbidity and mortality, often complicated by early rebleeding in the hyperacute phase. Tranexamic acid (TXA), an antifibrinolytic agent, has been proposed to reduce the risk of rebleeding; however, its impact on outcomes in patients admitted early (≤ 72 h) remains unclear. The authors searched PubMed, Embase, Web of Science, and Cochrane for randomized controlled trials evaluating TXA use among aSAH patients admitted early to the hospital (≤ 72 h). The authors performed a …
