Diagnosis and Treatment of Latent Tuberculosis Infection due to Initiation of Anti-TNF Therapy.

Patients with immune-mediated inflammatory diseases (IMIDs) are increasingly being treated with anti-tumor necrosis factor (TNF) agents and are at increased risk of developing tuberculosis (TB). Therefore, diagnosis and treatment of latent TB infection (LTBI) is recommended in these patients due to the initiation of anti-TNF therapy. Traditionally, LTBI has been diagnosed on the basis of clinical factors and a tuberculin skin test. Recently, interferon-gamma releasing assays (IGRAs) that can detect TB infection have become available. Considering the high-risk of developing TB in patients on an

[Recommendations for the prevention and management of tuberculosis in patients treated with tumor necrosis factor alpha inhibitors: a consensus of lithuanian pulmonologists and rheumatologists].

Patients receiving tumor necrosis factor alpha inhibitors for the treatment of rheumatic diseases (rheumatoid arthritis, psoriatic arthritis, ankylosing spondylitis) are at high risk of developing tuberculosis during treatment. This article gives the recommendations for the prevention and management of tuberculosis in patients with rheumatic diseases before initiating therapy with tumor necrosis factor alpha inhibitors. They are adapted considering the high prevalence of tuberculosis, high drug resistance of Mycobacterium tuberculosis, and extensive bacille Calmette-Guérin vaccination against

The diagnosis of tuberculosis in dialysis patients.

Tuberculosis is an important issue for nephrologists caring for dialysis patients. Because dialysis patients are immunocompromised, they are at higher risk for reactivation of latent tuberculosis, and they frequently have atypical presentation. Furthermore, hemodialysis units may foster rapid spread of active pulmonary tuberculosis. The diagnosis of active pulmonary tuberculosis still depends on detection of organisms by smear and culture. Newer nucleic acid detection techniques are more sensitive and specific. Nephrologists should remember that nonspecific presentation of tuberculosis includi

Relationship between bacille Calmette-Guérin vaccination, Mantoux test positivity, and geohelminth infection.

To investigate the potential protective effects of Bacille Calmette-Guerin (BCG) vaccination scar and sensitization to tuberculin against geohelminth infections, we conducted a cross-sectional study among school age children in rural communities in Pichincha Province in Ecuador where BCG vaccination is routinely given at birth. A total of 944 children aged 8-14 years were evaluated for the presence of BCG scars and sensitization to tuberculin, and underwent faecal examination for geohelminth parasites. BCG scars were present in 88.2% of children and positive Mantoux tests were observed in 19.1

[Diagnosis of latent tuberculous infections (healthy, currently or potentially immunocompromised subjects)].

Latent tuberculosis infection is a pathophysiological and clinical entity. The diagnosis is based on immunological tests such as the tuberculin skin test or interferon-gamma release assays. Interpretation of the results depends on the clinical context. Their predictive value for the occurrence of tuberculous disease is low, except in the case of young children and immunocompromised patients. Guidelines for the diagnosis of latent tuberculous infection in different countries are reviewed. The development of more predictive tests is desirable.

Suppression of delayed-type hypersensitivity to PPD and PHA in elderly HTLV-I carriers.

In a previous study on immune responsiveness among asymptomatic human T-cell leukemia virus type I (HTLV-I) carriers, we found that carriers had significantly reduced delayed-type hypersensitivity (DTH) response to purified protein derivative (PPD) skin testing. The association was strongest among persons at least 60 years of age. In order to evaluate this finding further, we evaluated the response to both PPD and phytohemagglutinin (PHA) in an elderly population. Fifty-six consecutive hospitalized patients with nonimmunosuppressive diseases were examined. None had a history of tuberculosis no

Application and interpretation of an interferon-gamma release assay: Results of an audit in a Canadian centre.

BACKGROUND: Interferon-gamma release assays (IGRAs) are newly approved for diagnosing latent tuberculosis infection (LTBI). An internal audit was conducted to review the use of a newly implemented IGRA at the Hôpital du Sacré-Coeur de Montréal (Montréal, Québec) to evaluate its concordance with Canadian recommendations and its implication on diagnosis. METHODS: From April 2007 to January 2009, all Quantiferon TB Gold In-Tube (QFT, Cellestis inc, USA) tests performed in at the Hôpital du Sacré-Coeur de Montréal were retrieved. Strategies used to investigate LTBI and clinical interpretation of

Utility of the antigen-specific interferon-gamma assay for the management of tuberculosis.

PURPOSE OF REVIEW: The tuberculin skin test, now over a century old, is prone to reader variability, and outcomes are influenced by cross-reactivity with environmental mycobacteria, previous bacillus Calmette-Guerin (BCG) vaccination, and anergy in immunosuppressed individuals. More recently, T-cell-based interferon-gamma responses to Mycobacterium tuberculosis - specific antigens have been investigated for their role in diagnosing latent tuberculosis infection. RECENT FINDINGS: We review the evidence supporting the utility of the interferon-gamma assay for the diagnosis of latent tuberculosi

Congenital Tuberculosis in a Stillborn Calf.

Bovine tuberculosis is a zoonotic infectious disease of cattle caused by the bacterium Mycobacterium bovis. In adult cattle, transmission is mainly via the respiratory route, whereas, in young calves, oral infection is also common. Congenital tuberculosis is rare. The purpose of this study was to describe in utero infection of a bovine stillbirth. The fetus was necropsied and tissue samples were processed for histopathology, bacteriology and PCR; a sample of the isolated strain was genotyped using spoligotyping. The whole herd was tuberculin skin tested and the dam's serum was also assessed fo

Tuberculin reactivity in first-year schoolchildren in Madagascar.

OBJECTIVE: The tuberculin skin test (TST) is an important tool in the diagnosis of tuberculosis infection in children. However, the interpretation of TST may be complicated by prior Bacillus Calmette-Guerin (BCG) vaccination. We evaluated the effect of vaccination with BCG on TST reactivity in first-year pupils attending state schools in Antananarivo. METHODS: STs were performed on 376 first-year schoolchildren, aged 6 and 7, attending two state primary schools. The relationships between epidemiological information, BCG status (vaccination, BCG scars) and TST reactivity were assessed to compa

Isolation of mycobacteria from dairy creamery effluent sludge.

Sixty three samples of dairy creamery effluent were examined for the presence of mycobacteria. Thirty two strains were isolated from 27 samples. These were classified as follows: M. fortuitum (13), M. peregrinum (6), M. gordonae(5), M. marianum (scrofulaceum) (4), unidentified (4). Ten strains, representative of the groups isolated, were tested for their effect on experimental animals. None were pathogenic for guinea pigs or mice but a number produced a minimal amount of skin sensitization in guinea pigs to avian and mammalian tuberculin.

The diagnosis of tuberculosis.

Diagnostic testing for tuberculosis has remained unchanged for nearly a century, but newer technologies hold the promise of a true revolution in tuberculosis diagnostics. New tests may well supplant the tuberculin skin test in diagnosing latent tuberculosis infection in much of the world. Tests such as the nucleic acid amplification assays allow more rapid and accurate diagnosing of pulmonary and extrapulmonary tuberculosis. The appropriate and affordable use of any of these tests depends on the setting in which they are employed.

Ichthyosiform sarcoidosis.

A 31-year-old black woman had acquired ichthyosis. Histologic examination of her skin revealed a sarcoidal reaction in the dermis. The diagnosis of sarcoidosis was supported by negative tuberculin (intermediate strength purified protein derivative) and Candida albicans extract intradermal skin tests; by a positive Kveim test; and by roentgenographic evidence of bilateral hilar adenopathy, paratracheal node enlargement, and diffuse pulmonary parenchymal changes. Sarcoidosis must be considered when acquired ichthyosis develops in a patient.

The presence of a booster phenomenon among contacts of active pulmonary tuberculosis cases: a retrospective cohort.

BACKGROUND: Assuming a higher risk of latent tuberculosis (TB) infection in the population of Rio de Janeiro, Brazil, in October of 1998 the TB Control Program of Clementino Fraga Filho Hospital (CFFH) routinely started to recommend a two-step tuberculin skin test (TST) in contacts of pulmonary TB cases in order to distinguish a boosting reaction due to a recall of delayed hypersensitivity previously established by infection with Mycobacterium tuberculosis (M.tb) or BCG vaccination from a tuberculin conversion. The aim of this study was to assess the prevalence of boosted tuberculin skin tests
