[Nephrureterectomy with endoscopic resection of the urinary bladder, ureteral ostium and terminal part in papillary tumors of the upper urinary tracts].

Short- and long-term results of endoscopically assisted nephrureterectomy were studied in 15 patients (7 males and 8 females aged 31-79 years) with papillary tumours of the upper urinary tracts (UUT). Of them, 11 patients have undergone transurethral endoresection of the urinary bladder, ostium and terminal part of the ipsilateral ureter followed by nephrureterectomy from the lumbotomic approach (technique 1) and 4 patients had nephrureterectomy followed by endoscopic intervention (technique 2). Results of standard open nephrureterectomy with resection of the bladder were analyzed retrospectiv…

Urothelium-derived prostanoids enhance contractility of urinary bladder smooth muscle and stimulate bladder afferent nerve activity in the mouse.

The transitional epithelial cells (urothelium) that line the lumen of the urinary bladder form a barrier between potentially harmful pathogens, toxins, and other bladder contents and the inner layers of the bladder wall. The urothelium, however, is not simply a passive barrier, as it can produce signaling factors, such as ATP, nitric oxide, prostaglandins, and other prostanoids, that can modulate bladder function. We investigated whether substances produced by the urothelium could directly modulate the contractility of the underlying urinary bladder smooth muscle. Force was measured in isolate…

Ultrasound estimated bladder weight and measurement of bladder wall thickness--useful noninvasive methods for assessing the lower urinary tract?

PURPOSE: In the last decade interest has arisen in the use of ultrasound derived measurements of bladder wall thickness, detrusor wall thickness and ultrasound estimated bladder weight as potential diagnostic tools for conditions known to induce detrusor hypertrophy. However, to date such measurements have not been adopted into clinical practice. We performed a comprehensive review of the literature to assess the potential clinical usefulness of these measurements. MATERIALS AND METHODS: A MEDLINE® search was conducted to identify all published literature up to June 2009, investigating measur

Urinary metabolomics identifies lactate as a biomarker for bladder cancer detection and progression.

Bladder cancer (BC) is the ninth most prevalent malignancy worldwide. It remains a significant clinical burden due to high recurrence rates and the need for reliable, non-invasive diagnostic tools. Metabolomics is a powerful strategy for non-invasive cancer detection, with urine representing an ideal biofluid for biomarker discovery, given its direct contact with the urinary tract and its rich diversity of metabolites. This study aimed to identify urinary metabolites showing significant differences in urinary levels between BC patients and controls, and to evaluate their potential for diagnosi

From contraception to urolithiasis: Migration of a contraceptive device into the urinary bladder lumen.

Intrauterine devices (IUDs) are widely used and generally safe contraceptives, though rare complications such as migration into adjacent organs may occur. We present a 34-year-old woman with suprapubic pain, dysuria, intermittent hematuria, and fatigue. Ultrasound revealed a bladder foreign body, further confirmed by multislice CT as an IUD. Cystoscopy demonstrated partial bladder wall embedding with stone encrustation. The device was successfully removed transurethrally. This case highlights the importance of considering IUD migration in women presenting with unexplained urinary tract symptom

A case of canine urothelial carcinoma of the urinary bladder with skull and skeletal metastases.

Canine urinary bladder neoplasia is uncommon, representing less than 1% of canine neoplasms. Amongst cases of urinary bladder neoplasia in dogs, primary urothelial carcinoma is the most frequent. Urothelial carcinomas are malignant invasive tumours which tend to be slow growing and metastasise late. However, the metastatic rate is high, with approximately 50% of cases developing metastases typically to the regional lymph nodes and lungs. Skeletal metastases are rare, with the most common reported sites being the femur and the vertebrae. Metastases to the skull are even rarer, with only a few p…

Advancing Personalized Care for Neurogenic Bladder: The Case for Continent Urinary Diversion.

INTRODUCTION: Neurogenic lower urinary tract dysfunction (NLUTD) with a poorly compliant bladder that is refractory to minimally invasive therapies (MIT) represents an "end-stage" phenotype in which the principal objectives are durable upper urinary tract protection and restoration of continence with acceptable long-term morbidity. For most surgically eligible patients in this setting, continent urinary diversion-most commonly a continent cutaneous catheterizable reservoir (e.g., Indiana pouch) and, in highly selected cases, an orthotopic continent diversion-offers a uniquely definitive strate

Urinary bladder invasion secondary to dissemination of gallbladder cancer: an unusual presentation.

Gallbladder cancer is an aggressive malignancy that is often diagnosed at an advanced stage, with frequent metastatic spread. Urinary bladder invasion caused by disseminated gallbladder cancer is extremely rare. A 56-year-old man presented with gross hematuria. Imaging revealed tumors in the gallbladder and urinary bladder with peritoneal dissemination. Transurethral resection of the urinary bladder tumor showed adenocarcinoma. Immunohistochemical analysis demonstrated positivity for CK7 and CDX2 and negativity for CK20, findings inconsistent with urachal carcinoma. Biopsy specimens from the g…

A Tumor of Intrigue: Undifferentiated Carcinoma With Osteoclastic Giant Cells of the Urinary Bladder.

Undifferentiated carcinoma with osteoclastic giant cells (UCOGC) of the urinary bladder is a recently described entity classified as a subtype of poorly differentiated urothelial carcinoma, which is a form of invasive urothelial carcinoma. It is an extremely rare and aggressive malignancy that occurs predominantly in elderly men. The rarity of this neoplasm and its morphological similarity to other giant cell-rich lesions may present diagnostic challenges. Here, we present a case of urinary bladder carcinoma with a biphasic appearance. Histomorphological examination and immunohistochemical stu…

Incidental Finding of an Inflammatory Myofibroblastic Tumor of the Urinary Bladder: A Case Report.

Bladder inflammatory myofibroblastic tumors (IMTs) are rare lesions with intermediate malignant potential and unclear management guidelines. We report the case of a 61-year-old male presenting with lower urinary tract symptoms and a bladder mass. Histology revealed spindle cells with mild atypia and chronic inflammation, and anaplastic lymphoma kinase (ALK) positivity suggested a TPM3-ALK fusion. The tumor was completely resected via transurethral resection of the bladder tumor (TURBT), with no recurrence at the six-month follow-up. This case illustrates the diagnostic complexity of bladder IM

Exploring the Genetic Landscape of Primary Marginal Zone Lymphoma of the Urinary Bladder.

Extranodal marginal zone B-cell lymphoma (MZL) of mucosa-associated lymphoid tissue is the most frequent primary lymphoma of the urinary bladder. Although MZLs from various anatomical sites are often associated with autoimmune disorders, infections, and site-characteristic genetic alterations, the molecular foundations and potential infectious triggers of urinary bladder MZL remain poorly understood. To elucidate the disease etiology and correlation with MZLs arising in other locations, we examined a cohort of 17 cases (11 women and 6 men) diagnosed with primary bladder MZL between 2005 and 20…

Bladder Cancer and the Urinary Microbiome-New Insights and Future Directions: A Review.

The presence of a microbiome in the urinary system has been established through recent advancements in technology and investigation of microbial communities in the human body. The study of the taxonomic and genomic ecology of microbial communities has been greatly improved by the use of metagenomics. The research in this area has expanded our understanding of microbial ecosystems and shows that the urinary tract contains over 100 species from over 50 genera, with Lactobacillus, Gardnerella, and Streptococcus being the most common. Previous studies have suggested that the microbiota in the urin
