Cyclosporin-A therapy in severe uveitis of Behçet's disease.

PURPOSE: Cyclosporine-A (CSA) combined with corticosteroid therapy was administered to 12 patients with severe Behçet's uveitis who were resistant to colchicine or cytotoxic therapy. METHODS: Previous colchicine or cytotoxic therapies were tapered off one month before CSA therapy. All patients were started on an initial oral dose of 5 mg/kg/day of CSA. After the first three months, the CSA dose was reduced to a maintenance dose according to the intraocular inflammatory response. RESULTS: The average follow-up period was 20 (12-36) months. Visual acuity remained the same in 12 (%54.5) and imp…

Immunologic mechanisms of uveitis. New targets for immunomodulation.

The uvea consists of the choroid, ciliary body, and iris, and inflammation of the uveal tract is termed uveitis. Nevertheless, uveitis is commonly used to more generally describe intraocular inflammation involving not only the uvea, but also the retina, vitreous, and sclera. Fifty years ago, infectious organisms such as syphilis and tuberculosis were thought to be the cause of most forms of uveitis. Since that time, many causes of uveitis have been described as infectious and noninfectious. Scientists have shown that the immune response plays a critical role in the development of infectious an…

[Clinical aspects of traumatic uveitis and causes of enucleation].

The authors analyze 37 cases with grave traumatic uveitis eventuating in enucleation. Terms of enucleation after the injury, type of the injury, clinical picture of chronic uveitis, echographic and electrophysiological findings are presented. In ten patients signs of sensitization to ocular tissues were detected in various periods after the injury. Sensitization was detected by the leukocyte migration inhibition test (LMIT). The authors regard the results as a response to release of tissue antigens in the bloodflow because of impairment of the blood-eye barrier caused by the trauma. The author…

Uveitis associated with rifabutin therapy: a clinical alert.

BACKGROUND: The incidence of Mycobacterium avium complex (MAC) infection is increasing among patients with AIDS. Rifabutin therapy has been demonstrated to be effective in the treatment and prophylaxis of MAC infection. over the past 3 years there have been several reports of uveitis associated with rifabutin therapy. The uveitis is almost always interior, begins unilaterally and progresses to involve both eyes. There is almost always a hypopyon present, and the uveitis progresses aggressively. It responds well to conventional treatment along with the discontinuance of rifabutin. METHODS/RESU…

[Incidence of uveitis at Tokyo Medical College Hospital].

We reviewed the records of 980 patients with endogenous uveitis who were followed in the uveitis clinic of the Tokyo Medical College Hospital, Department of Ophthalmology. A group of 693 patients examined within a 5-year period from January, 1989 through December, 1993 (Group A) were compared to an earlier group of 287 patients examined within a 3-year period from January, 1982 through December, 1984 (Group B). The incidence of Behçet's disease, intermediate uveitis, and toxoplasmosis showed an apparent decrease. In contrast, the incidence of sarcoidosis and acute anterior uveitis increased. I…

Scleritis, uveitis, and glaucoma in a patient with rheumatic fever.

PURPOSE: To determine the origin of scleritis, uveitis, and glaucoma in a boy with fever, sore throat, joint pains, and malaise. METHODS: The patient underwent extensive hematologic, serologic, rheumatologic, and radiologic examinations. RESULTS: The patient recovered after extensive medical therapy. He met the Jones criteria for the diagnosis of acute rheumatic fever and had an increased result for antistreptolysin O antibody titer. CONCLUSIONS: Antistreptococcal antibody testing is helpful in diagnosing rheumatic fever as a cause of scleritis, uveitis, and glaucoma.

Trabeculectomy with mitomycin C in glaucoma associated with uveitis.

Twenty-four trabeculectomies performed in 24 patients for control of uveitic glaucoma were retrospectively evaluated to analyze the effect of intraoperative application of mitomycin C (MMC) on the short-term outcome of trabeculectomy for glaucoma associated with uveitis. Success rates, postoperative levels of intraocular pressure (IOP), and complications were studied. After a mean follow up of 9.87 months (range, 3 to 27 months), 18 eyes (75%) achieved an IOP of 21 mm Hg or less without antiglaucoma medications. The same IOP level with one antiglaucoma medication was achieved in four eyes (16.…

Ultrastructural pathology of experimental autoimmune uveitis in the rat.

Experimental autoimmune uveoretinitis (EAU) has been extensively studied as a model for human posterior uveitis, however, few ultrastructural studies of EAU in the rat have been reported. In the present report we document a systematic time-course study of the posterior segment changes in the Lewis rat. The disease varied somewhat in severity in animals sacrificed at identical times after immunisation. In the prodromal stage of the disease, usually around day 14, early pathological changes included mild peripapillary vasculitis and low grade mononuclear and neutrophilic infiltration of the subr…

The role of chorioretinal biopsy in the management of posterior uveitis.

BACKGROUND: The ideal management of intraocular inflammation may require a tissue diagnosis. Diagnostic vitrectomy is a well-established method for acquiring such tissue. However, in some patients, the diagnostic pathology is limited to the choroid and retina and vitrectomy may yield no useful information. In such cases, a more aggressive surgical approach to obtain tissue may be useful. METHODS: The authors performed chorioretinal biopsies on seven patients with progressive chorioretinal lesions of unknown etiology. Indications for biopsy included: (1) macular-threatening lesions unresponsiv

Fuchs' Heterochromic Uveitis: a reappraisal of the clinical spectrum.

The varying clinical appearance within a group of 103 patients with Fuchs' Heterochromic Uveitis (FHU) is discussed. Bilateral cases accounted for 7.8% of patients. Cataract was found in 80.2% of cases after 8.8 years mean follow-up, and glaucoma affected 26.2% of patients. The most important diagnostic criteria are the typical pattern of iris atrophy and depigmentation, the characteristic intraocular inflammation, and complicated cataract. These are described in detail. The correct diagnosis of FHU is important because it affects future management and prognosis. The presence or absence of mac…

Clinical patterns and associated conditions in chronic uveitis.

We determined the relative frequencies of the different types of chronic uveitis, and the possible associated conditions, among 400 consecutive patients with chronic uveitis examined during the years 1982 through 1988. Of the 400 patients, 183 (45.75%) had anterior uveitis, 98 (24.5%) ahd panuveitis, 61 (15.3%) had intermediate uveitis, and 58 (14.5%) had isolated posterior uveitis. Only four of the 98 patients with panuveitis (4.1%) were considered idiopathic after a comprehensive examination, whereas 94 of the 183 patients with anterior uveitis (51.4%) were similarly classified. We found an …

[Role of enterovirus infection in the etiology of uveitis in children].

A total of 224 cases with endogenic uveitis in infants are analyzed. The results of complex clinical and immunological analysis and serologic findings bring the authors to a conclusion that enteroviral uveitis makes up 42 percent of all uveitis in children aged under 3 and that this condition is prevalent in this country. Typical late complications, long persistence of virus-neutralizing antibodies in high titers (up to 8-10 years after convalescence) permit a retrospective etiological diagnosis.

Secondary syphilis presenting with optic perineuritis and uveitis.

We describe a 40 year-old man with optic perineuritis due to secondary syphilis. This condition which is due to inflammation of the optic nerve sheath is often mistaken for papilloedema or papillitis. We discuss its diagnosis and stress the importance of recognising it because it is easily treated by a course of Penicillin.

Invasive diagnostic techniques for uveitis and simulating conditions.

Many patients with uveitis have such characteristic ocular signs and symptoms, associated systemic disorders, and laboratory abnormalities that a satisfactory clinical diagnosis can be established without the need for invasive intraocular studies. Most other patients with uveitis have mild, self-limited and/or readily controllable disease that does not warrant aggressive invasive testing. In contrast, some patients with uveitis have atypical ophthalmic and/or systemic features or do not respond to conventional antiinflammatory therapies. Such patients may be candidates for invasive diagnostic …

[Importance of ocular biopsies in the etiologic diagnosis of uveitis].

In many uveitis cases a biopsy of ocular tissues can enable an exact diagnosis to be established, unnecessary additional examinations to be avoided, and an appropriate therapeutic regimen to be selected. In general, follicle biopsy is performed where sarcoidosis is suspected, keratocentesis in viral uveitis and toxoplasmosis, and vitreous biopsy in tentatively diagnosed primary intraocular lymphoma (reticulosarcoma) and bacterial or mycotic endophthalmitis.
