Since the advent of H2-receptor antagonists, elective ulcer surgery is rare. The need for operation for complications of peptic ulcer disease, however, remains unchanged. Truncal vagotomy and drainage is still the commonest operation for duodenal ulcer in the United Kingdom, despite its known association with diarrhoea. Shown in the paper is a possibility of surgical correction of disturbances in the intestinal phase of gastric secretion in treating peptic ulcer and its complications which are encountered in an overwhelming majority of patients. Sympathectomy of relevant vascular trees is regarded as a pathogenetically validated procedure as is correction of duodenal motility combined with selective proximal vagotomy and excision of the ulcer. It is shown that truncal and selective vagotomy can aggravate the existing disorders of the intestinal phase of gastric secretion.

There is no ideal operation for peptic ulcer disease and sound surgical judgement is needed in choosing an operation for a specific indication in the individual patient. Highly selective vagotomy is currently the operation of choice for uncomplicated duodenal ulcer, and Billroth I gastrectomy remains the procedure of choice for uncomplicated gastric ulcer, but a variety of procedures are used in the management of the complications of peptic ulcer disease with vagotomy and drainage being the most frequently used and valued procedure.

The authors report their experience in the treatment of hemorrhagic duodenal ulcers in 19 patients (17 men, 2 women) between 1972 and 1977. They compare the results obtained with the two methods adopted, namely gastric resection and local hemostasis of the ulcer followed by troncular or selective vagotomy, or selective proximal vagotomy and pyloroplasty. The results so analyzed speak definitely in favor of the latter procedures as opposed to gastrectomy.

Contractions of the fluid-filled RR dependent upon the activity of the myenteric plexus were observed within 2 weeks after vagotomy in sheep maintained on a liquid diet. Spontaneous but not adrenergic evoked rumination was recorded beyond the 5th week after vagotomy. The observation of ruminating-like activity during 120 to 200 min per day in chronically-vagotomized sheep indicates that the onset of rumination does not require a gastric afferent pathway.
