BACKGROUND: Clinical features of carotid artery dissection include ipsilateral local signs, contralateral ischemic stroke, or both. We observed two patients in whom these features were associated with renal infarcts.
CASE DESCRIPTIONS: A 57-year-old woman had painful Horner's syndrome caused by a right internal carotid artery dissection. On days 3 and 4 she had acute abdominal pain, first on the right side and later on the left. The computed tomographic (CT) scan showed a left renal infarct. No aortic dissection or cardiac source of embolism was found. Transesophageal echocardiography showed

In human anatomy, the vertebral artery, in its passage from the subclavian artery to the foramen magnum of the skull, enters the transverse foramina of the vertebral column generally at the level of the 6th cervical vertebra. Nevertheless, even though avoided by the vertebral artery, also the 7th cervical vertebra (C7) contains a transverse foramen. The content of this transverse foramen in C7 has been unclear, with different descriptions found in textbooks and in original literature. Here, we have revisited the content of the transverse foramen in C7 by macroscopic dissection of 32 human spec

Dissecting aneurysms of cerebral arteries are unusual causes of stroke. The carotid system is the commonest site of this pathology, the vertebral arteries are less involved and dissection of the basilar artery is rare. The authors report three cases of arterial dissection of the vertebrobasilar system, two of the vertebral arteries and one of the basilar artery. An extensive review of the literature is presented. The clinical picture of dissection of vertebrobasilar system was inespecific but pain was a prominent symptom, though had not occurred in the site of the arteries involved. The pain w

We report a patient with dissecting aneurysm who presented with a sudden severe headache without any neurological symptoms. Although brain computed tomography (CT) scan and MRI were negative, magnetic resonance angiography (MRA) showed a pseudocavity in a segment of the left vertebral artery. In addition, the dissecting wall of the left vertebral artery was clearly visualized in the original images of MRA. Our findings indicate that brain CT, MRI or cerebral angiography alone are sometimes inadequate for the diagnosis of vertebral dissecting aneurysm, and that MRA and its original images are n

BACKGROUND AND PURPOSE: Little is known about associations between spontaneous coronary artery dissection and cervical artery abnormalities. This study sought to assess the prevalence of cervical artery abnormalities among patients with spontaneous coronary artery dissection.
MATERIALS AND METHODS: A retrospective analysis was completed of patients who underwent CTA neck imaging as part of arterial assessment following the diagnosis of spontaneous coronary artery dissection. The internal carotid and vertebral arteries were evaluated for the presence of fibromuscular dysplasia, dissection and/

Recently several reports of dissecting aneurysm in the vertebrobasilar system have been published. We report the effectiveness of MRI for the detection of a dissecting aneurysm in the vertebrobasilar distribution system. A 47 year-old male [correction of female] was admitted to our hospital due to right occipitalgia and the right Wallenberg's syndrome. Computed tomography disclosed no abnormalities. Cerebral angiogram showed the pearl and string sign of the right vertebral artery, which suggested a dissecting aneurysm. MRI exhibited the infarct area at the right dorsolateral part of the medull

Penetrating facial trauma can result in vascular abnormalities that are difficult to identify on initial imaging due to artifact from retained fragments. External carotid artery pseudoaneurysms are uncommon but may carry significant morbidity if not promptly recognized. A 34-year-old female presented after a gunshot wound to the left face. Initial computed tomography angiography (CTA) demonstrated left vertebral artery V3 dissection, severe left internal carotid artery stenosis, and a comminuted mandibular fracture with retained ballistic fragments causing substantial streak artifact. Due to p

We evaluated the effectiveness of transcatheter arterial coil embolization therapy for the treatment of spontaneous hemothorax followed by aneurysm rupture in neurofibromatosis patients. Three patients were treated for massive hemothorax caused by arterial lesions associated with neurofibromatosis. Bleeding episodes were secondary to ascending cervical artery aneurysm and dissection of vertebral artery in 1 patient, and intercostal artery aneurysm with or without arteriovenous fistula in 2 patients. Patients were treated by transarterial coil embolization combined with chest drainage. In 1 pat

Vertebral arteriovenous fistula (vAVF) is an uncommon vascular disease defined as abnormal connections between the vertebral artery or its branches extracranially with nearby venous structures. This case report outlines the case of a man in his late 70s presenting with C1-C3 fractures after a mild trauma falling down a small staircase. CT angiogram (CTA) gave suspicion of vertebral artery dissection and pseudoaneurysm; however, digital subtraction angiography revealed a fracture-induced vAVF successfully treated endovascularly with coils. In conclusion, cervical fractures involving the transve…

Currently, there is no information about the possibility of developing clinical symptoms after whiplash in double transverse foramen subjects. Our aim was to test whether subjects with double transverse foramen have an increased risk of presenting with an acute headache, dizziness, vertebral artery dissection, and vomiting after whiplash. We recorded the absence/presence of double transverse foramen, and the absence/presence of neck pain, acute headache, dizziness, vertebral artery dissection, and vomiting in 85 patients who had suffered whiplash injuries in car rear-end impacts in road traffic…

BACKGROUND: An aberrant right subclavian artery is the most common congenital anomaly of the aortic arch and may cause symptoms due to aneurysmal dilatation, stenosis or occlusion. We present a case of subclavian-steal syndrome due to post-traumatic dissection of an aberrant right subclavian artery.
METHODS AND RESULTS: A 50 year-old man presented with dizziness and fainting episodes after exercising his right arm and a systolic blood pressure gradient of 40 mm Hg between the 2 arms. Suspecting a subclavian steal syndrome, a computed tomography angiography was requested which revealed an aber

Twenty-six patients with blunt trauma of the cervical spine, producing a subluxation from a "locked" or "perched" facet, facet destruction with evidence of instability, or a fracture involving the foramen transversarium, underwent preoperative vertebral angiography to determine the incidence of vertebral artery injury. The cervical spine injury in all the patients was deemed unstable and in need of surgical stabilization. Spinal cord injury was present in one-half of the patients studied. Vertebral artery injury was identified angiographically in 12 patients (46%). Occlusion of the vertebral a

An 85-year-old man who did not have any previous history of heart disease was admitted to our hospital with the chief complaints of disturbance of consciousness, tetraparesis, and bilateral Babinski signs. On admission, intravenous digital subtraction angiography (DSA) showed bilatral vertebral arteries without filling the basilar artery. On the second hospital day, MRI showed a septum in the basilar artery, and no infarctions were noted in the brain stem. He gradually improved with heparin therapy. On the tenth hospital day, intravenous DSA disclosed filling of the basilar artery. On the sixt

Objective This study was aimed to discuss how to control extradural venous congestion with an increased pressure in cases of arteriovenous shunt disease of the craniovertebral junction. Design The study is presented through an operative video. Results A 77-year-old patient with subarachnoid hemorrhage had a dural arteriovenous fistula located at the C1-C2 level. Left vertebral angiography showed a fistula between the left C2 radiculomeningeal muscular artery and perivertebral plexus. Furthermore, right vertebral angiography showed a ruptured aneurysm at the aberrant branch of the anterior s

The objective of this study was to identify the clinical features associated with neurological deterioration within 30 days of ischemic stroke patients with spontaneous cevicocranial dissection (SCCD) and clarify the effect on outcomes. We retrospectively identified 18 patients with SCCD (1.6%, 3 women, 52 ± 16 years old) among 1,112 patients with acute ischemic stroke within 7 days after onset. Of the 18 patients, 13 (72%) had vertebrobasilar arterial dissection. Neurological deterioration was present in 4 patients (22%), and 2 patients (11%) died. All of them became worse within 3 days after

We describe two previously unreported associations in four cases. The first two cases demonstrate an association between segmental mediolytic arteriopathy and vascular Ehlers-Danlos syndrome. The second two cases illustrate an association between vascular Ehlers-Danlos syndrome and traumatic subarachnoid hemorrhage. In case 1, there was acute subarachnoid hemorrhage and mesenteric artery dissection. In case 2, there was an acute mesenteric artery dissection with intestinal infarction. In both cases 1 and 2, segmental mediolytic arteriopathy was found in the vertebral arteries. Cases 3 and 4 we

Vertebral artery (VA) dissections are uncommon, but are increasingly recognized as a cause of posterior circulation stroke in young adults. We describe a patient with bilateral VA dissection who presented SAH. The patient was successfully treated by proximal coil occlusion of the bilateral vertebral arteries in two stages. In the treatment of SAH due to bilateral VA dissection, it is important to estimate the status of the treated vessels or contralateral vessels by follow-up angiography. Staged bilateral proximal VA occlusions should be considered in the case of recurrent VA dissection or pro
