Corticosteroids for the treatment of idiopathic acute vestibular dysfunction (vestibular neuritis).

BACKGROUND: Idiopathic acute vestibular dysfunction (vestibular neuritis) is the second most common cause of peripheral vertigo after benign paroxysmal positional vertigo (BPPV) and accounts for 7% of the patients who present at outpatient clinics specialising in the treatment of dizziness. The exact aetiology of the condition is unknown and the effects of corticosteroids on the condition and its recovery are uncertain. OBJECTIVES: To assess the effectiveness of corticosteroids in the management of patients with idiopathic acute vestibular dysfunction (vestibular neuritis). SEARCH STRATEGY:

Visual gravitational vertical perception in peripheral vestibular hypofunction.

CONCLUSIONS: Rise time in the estimation of the gravitational vertical in the head tilt response (HTR) test is increased in patients with peripheral vestibular lesions and residual chronic dizziness. OBJECTIVE: Assessment of the perception of the gravitational vertical in patients with peripheral vestibular lesions through the HTR. METHODS: HTR was studied in 12 patients with peripheral vestibular lesion, 8 clinically with chronic dizziness and 4 without it; 23 normal subjects were studied as control group. Two parameters of the HTR were assessed, rise time and steady-state error to characte

Effect of vestibular neuritis on postural control using wavelets and fractal analysis.

OBJECTIVE: What is the status of postural control a few months after an attack of vestibular neuritis (VN)? Using dynamic posturography and stabilometric signal treatment with wavelets and fractal analysis, we tried to answer this question by isolating the pathological postural parameters of VN. MATERIAL AND METHOD: The study involved a group of 15 patients (GP) who suffered from VN and were compared to a group of control subjects (GC). Both groups underwent videonystagmography (VNG), dynamic posturography (PDY), and assessment using symptomatic scales (ES). RESULTS: GP and GC were comparabl

[Should vestibular neuritis be treated with corticosteroids?].

Recent studies have shown that corticosteroid treatment of patients with vestibular neuritis significantly improves recovery of peripheral vestibular function. At follow-up, the number of patients with a normal caloric response was larger among those treated with corticosteroids. However, improvement of caloric responses may not correlate directly with patient symptoms and up to now improved vertigo/dizziness due to corticosteroid treatment has not been reported. Consequently, although corticosteroid may be a treatment option, it should currently only be considered in those vestibular neuritis

What accounts for vertigo one year after neuritis vestibularis - anxiety or a dysfunctional vestibular organ?

One year after neuritis vestibularis, 29% from a sample of 75 patients still complained of vertigo. The objective of this investigation was to study why patients suffer from persisting vertigo. The alternative hypotheses were that the vertigo experienced could be explained either by a persisting vestibular dysfunction or by psychopathological changes. To elucidate this question, patients were examined with dynamic posturography, the symptom check list (SCL 90 R), the state-trait anxiety inventory (STAI), the agoraphobic cognitions questionnaire (ALQ) and the body sensations questionnaire (BSQ)

Relevance of plasma D-dimer measurement in patients with acute peripheral vertigo.

The aetiopathogenesis of acute unilateral peripheral vestibular dysfunction (APV), also known as vestibular neuritis, is still debated: the principal cause is viral infection with vascular factors second in importance. Plasmatic D-dimer, considered a plasmatic index of hypercoagulation, was measured in a group of 45 APV patients and in a group of 25 patients suffering from Ménière's disease. Measurements were taken both during the acute stage and after a four to six week period of pharmacological washout. The mean D-dimer levels were significantly higher than those measured in the controls bot

Transplantation of Human iPS Cell-derived Cerebral Cortical Neurons Promotes Fine Motor Recovery in a Female Mouse Model of Ischemic Stroke.

BACKGROUND: Stroke is a leading global health concern, with cerebral infarction accounting for 62% of cases. Despite advances in acute-phase treatments, functional impairments such as motor deficits remain prevalent. This study investigates the potential of human induced pluripotent stem cell (iPSC)-derived cerebral cortical neurons for neural regeneration and motor function recovery in a female mouse model of ischemic stroke. METHODS: Cerebral infarction was induced using the Rose Bengal photothrombosis method, followed by transplantation of iPSC-derived cortical neurons into the area adjace

Brainstem ischemic damage following occlusion of the blood vessels in the rat's posterior cerebral circulation.

Progression of ischemic damage was investigated immunohistochemically in neural dendrites using microtubule-associated protein 2 (MAP2) as a dendritic marker in the rat's brainstem. Neuronal soma and dendrites were clearly stained by this protein but some structures such as axonal bundles, glia and endothelial cells were not visualized. When the anterior inferior cerebellar artery (AICA) was occluded unilaterally for 30 min, a wide ischemic lesion was detected in the occluded side of the brainstem and was observed as a loss of reaction to MAP2. After ischemia for 2 h, loss of reaction in the p

Temporal bone findings in a case of bilateral Menière's disease treated by parenteral streptomycin and endolymphatic shunt.

A patient with bilateral Menière's disease who had progressive hearing loss and intractable vertigo was treated at ages 60 and 62 with parenteral streptomycin to ablate vestibular function, and at age 74 by a left endolymphatic shunt procedure. He was confined to a wheelchair because of ataxia from age 75 until the time of his death at age 81. Both temporal bones show congenitally hypoplastic endolymphatic sacs and severe endolymphatic hydrops consistent with Meniere's disease. There was atrophy of the striae vasculares and loss of cochlear neurons consistent with presbycusis. A loss of hair c

Progressive pontobulbar palsy with deafness: clinical and pathological study of two cases.

Among the hereditary affections of the nervous system associated with deafness, a rare condition called "progressive pontobulbar palsy with deafness" has been described. In this slowly progressive condition, hearing loss and vestibular are-flexia are almost always the first symptoms, occurring in late childhood or early adulthood. Only 18 cases-some sporadic, several familial-have been published without a full report of pathological findings. The clinical and pathological data of two new cases-one familial, one sporadic-are described here. There are differences from other forms of bulbar paral

Does acute dysfunction of the saccular afferents affect the subjective visual horizontal in patients with vestibular neurolabyrinthitis?

CONCLUSIONS: The results of a series of studies including the present study suggest that acute dysfunction of the utricular afferents accompanied by acute dysfunction of the saccular afferents might require more time for the compensation of the otolith-ocular system than acute utricular dysfunction that was not accompanied by acute saccular dysfunction. Perhaps the inputs from the saccule also have some contribution to the subjective visual horizontal (SVH). OBJECTIVE: To clarify if acute dysfunction of the saccular afferents affects the SVH. PATIENTS AND METHODS: Twenty-six patients with ve

Neuronal apoptosis in the brainstem medulla of sudden unexpected death in infancy (SUDI), and the importance of standardized SUDI classification.

The purpose of this study was to examine the neuronal expression of apoptotic markers in the rostral medulla of a newly characterized dataset of sudden unexpected death in infancy (SUDI), and to determine the impact of diagnostic groupings on these findings and whether they pertain to the intrinsic apoptotic pathway. Immunohistochemical staining was quantified to determine the percentage of neurons positive for active caspase-9 (specific to the intrinsic apoptotic pathway), active caspase-3 (common to the intrinsic and extrinsic apoptotic pathways) and Terminal deoxynucleotidyl transferase med

Effects of cerebellar transcranial direct current stimulation on the excitability of spinal motor neurons and vestibulospinal tract in healthy individuals.

Cerebellar transcranial direct current stimulation (ctDCS) modulates cerebellar cortical excitability in a polarity-dependent manner and affects inhibitory pathways from the cerebellum. The cerebellum modulates spinal reflex excitability via the vestibulospinal tract and other pathways projecting to the spinal motor neurons; however, the effects of ctDCS on the excitability of spinal motor neurons and vestibulospinal tract remain unclear. The experiment involved 13 healthy individuals. ctDCS (sham-ctDCS, anodal-ctDCS, and cathodal-ctDCS) was applied to the cerebellar vermis at 2 mA with an int

Typical sensory organization test findings and clinical implication in acute vestibular neuritis.

OBJECTIVE: Sensory organization test (SOT) is used to evaluate postural instability. We wanted to characterize the SOT findings in patients with acute vestibular neuritis (VN). METHODS: Eighty-seven patients with VN were enrolled. The bithermal caloric and SOT were performed, and the results were compared with those from the dizziness handicap inventory (DHI). Abnormal SOT patterns were classified: severe, visual vestibular, vestibular, inconsistent, or normal patterns. The results were also analyzed by sensory analysis (somatosensory, visual, vestibular, and visual preference) and composite

The relationship between vasomotor symptoms and menopause-associated dizziness.

CONCLUSION: Vasomotor symptoms (VMS) and especially hot flushes are negatively associated with recovery from menopause-related dizziness. A further study is justified to determine whether hormone replacement therapy (HRT) can shorten this recovery time. OBJECTIVES: This study aimed to compare the rate of recovery from dizziness between patients who had VMS and those who did not. We also evaluated the effect of HRT on recovery from dizziness. METHODS: The medical records of 85 women aged between 40 and 59 years were analyzed retrospectively. The patients were asked detailed questions about th
