Guidelines for Best Practice in the Audiological Management of Adults Using Bimodal Hearing Configurations.

Clinics are treating a growing number of patients with greater amounts of residual hearing. These patients often benefit from a bimodal hearing configuration in which acoustic input from a hearing aid on 1 ear is combined with electrical stimulation from a cochlear implant on the other ear. The current guidelines aim to review the literature and provide best practice recommendations for the evaluation and treatment of individuals with bilateral sensorineural hearing loss who may benefit from bimodal hearing configurations. Specifically, the guidelines review: benefits of bimodal listening, pre

Simultaneous Translabyrinthine Tumor Removal and Cochlear Implantation in Vestibular Schwannoma Patients.

Refinement of surgical techniques has allowed hearing preservation after tumor resection to be prioritized. Moreover, restoration of hearing after tumor removal can be attempted in patients with bilateral vestibular schwannomas or those with a schwannoma in the only-hearing ear. Cochlear implantation (CI) has emerged as a proper method of acoustic rehabilitation, provided that the cochlear nerve remains intact. Studies of electrical promontory stimulation in patients after vestibular schwannoma resection have demonstrated favorable results. We describe herein two cases of hearing rehabilitatio

Mechanisms and Functional Consequences of Presynaptic Homeostatic Plasticity at Auditory Nerve Synapses.

Multiple forms of homeostasis influence synaptic function under diverse activity conditions. Both presynaptic and postsynaptic forms of homeostasis are important, but their relative impact on fidelity is unknown. To address this issue, we studied auditory nerve synapses onto bushy cells in the cochlear nucleus of mice of both sexes. These synapses undergo bidirectional presynaptic and postsynaptic homeostatic changes with increased and decreased acoustic stimulation. We found that both young and mature synapses exhibit similar activity-dependent changes in short-term depression. Experiments us…

Maladaptive neural synchrony in tinnitus: origin and restoration.

Tinnitus is the conscious perception of sound heard in the absence of physical sound sources external or internal to the body, reflected in aberrant neural synchrony of spontaneous or resting-state brain activity. Neural synchrony is generated by the nearly simultaneous firing of individual neurons, of the synchronization of membrane-potential changes in local neural groups as reflected in the local field potentials, resulting in the presence of oscillatory brain waves in the EEG. Noise-induced hearing loss, often resulting in tinnitus, causes a reorganization of the tonotopic map in auditory

On the biological plausibility of Wind Turbine Syndrome.

An emerging environmental health issue relates to potential ill-effects of wind turbine noise. There have been numerous suggestions that the low-frequency acoustic components in wind turbine signals can cause symptoms associated with vestibular system disorders, namely vertigo, nausea, and nystagmus. This constellation of symptoms has been labeled as Wind Turbine Syndrome, and has been identified in case studies of individuals living close to wind farms. This review discusses whether it is biologically plausible for the turbine noise to stimulate the vestibular parts of the inner ear and, by e

Frequency tuning of the contralateral medial olivocochlear reflex in humans.

Activation of the medial olivocochlear (MOC) efferents attenuates cochlear gain and reduces the amplitudes of mechanical, electrical, and neural cochlear outputs. The functional roles of the MOC efferents are not fully understood, especially in humans, despite postulations that they are involved in protection against acoustic trauma, facilitation of transient-sound perception, etc. Delineating the frequency tuning properties of the MOC efferents would provide critical evidence to support or refute these postulated functional roles. By utilizing spontaneous otoacoustic emissions (SOAEs), a coch

Reduced neural speech tracking in adolescents with listening difficulty.

OBJECTIVE: To investigate neural mechanisms underlying speech-in-speech listening in adolescents with listening difficulties (LiD). METHODS: Neural speech tracking (NST) was assessed using magnetoencephalography (MEG; 275 sensors) in 21 adolescents with LiD (11 females), identified by caregiver report, and 25 typically developing (TD) peers (10 females), all with normal audiograms and frequency-matched on age, sex, race, and ethnicity. Participants completed a cocktail-party task with target speech alone and with competing streams that varied in talker and spatial cues. NST was quantified usi

A biologically inspired system differentiates plants by classifying binaural audio-frequency echo sequences with error detection and resilience.

A biologically inspired system (BIS) classifies binaural audio-frequency echo sequences to recognize two plants that differ mainly in leaf size. The hierarchical BIS system is implemented with two stages. Stage 1 classifies binaural echo spectra with a pair of ear-specific trained single-layer perceptrons. Stage 2 compensates for limited classification accuracy by detecting binaural classification errors and accumulating classification votes by forming a random walk with drift that approaches one of two absorbing boundaries to differentiate the plants. The random walk provides error resilience

Pulse timing dominates binaural hearing with cochlear implants.

Although cochlear implants (CIs) provide valuable auditory information to more than one million profoundly deaf patients, these devices remain inadequate in conveying fine timing cues. Early deaf patients in particular struggle to use interaural time differences (ITDs) for spatial hearing and auditory scene analysis. Why CI patients experience these limitations remains controversial. One possible explanation, which we investigate here, is that the stimulation by clinical CIs is inappropriate, as it encodes temporal features of sounds only in the envelope of electrical pulse trains, not the pul

Brain-Body Coupling in Listening to Metronomic Sounds and Music.

Wearables continue to expand their ability to sample more signals from the brain and body while reducing form factors and limiting on-device computing to meet real-world comfort and power constraints. Where evolving multimodal sensing platforms can combine electrophysiological (ExG), optical, chemical, and mechanical sensors for holistic brain-body state estimation, understanding of cross-modal coupling mechanisms remains limited. Here, we collect and analyze bio-signals in a brain-body coupling experiment using simultaneous electroencephalogram (EEG), electrocardiogram (ECG), and respiratory

Gait in Older People with Cognitive Impairments During Rhythmic Auditory Cueing: A Scoping Review.

PURPOSE: Rhythmic auditory cueing (RAC) can enhance gait performance. However, little is known about its effect in people with cognitive impairment. The aim of this review was to describe, in studies of people with cognitive impairment: 1) the medical conditions and cognitive stages studied; 2) the RAC protocols and gait variables measured; and 3) whether cognitive stage or RAC protocols have differing effects on gait. MATERIALS AND METHODS: Search terms included older age, cognitive impairment, auditory cueing, and gait. Three databases (PubMed, Embase via Ovid, Web of Science) were searched

Frequency selectivity in monkey auditory nerve studied with suprathreshold multicomponent stimuli.

Data from non-human primates can help extend observations from non-primate species to humans. Here we report measurements on the auditory nerve of macaque monkeys in the context of a controversial topic important to human hearing. A range of techniques have been used to examine the claim, which is not generally accepted, that human frequency tuning is sharper than traditionally thought, and sharper than in commonly used animal models. Data from single auditory-nerve fibers occupy a pivotal position to examine this claim, but are not available for humans. A previous study reported sharper tunin

Experience-Dependent Plasticity in Nucleus Laminaris of the Barn Owl.

Interaural time differences (ITDs) are a major cue for sound localization and change with increasing head size. Since the barn owl's head width more than doubles in the month after hatching, we hypothesized that the development of their ITD detection circuit might be modified by experience. To test this, we raised owls with unilateral ear inserts that delayed and attenuated the acoustic signal, and then measured the ITD representation in the brainstem nucleus laminaris (NL) when they were adults. The ITD circuit is composed of delay line inputs to coincidence detectors, and we predicted that p

Safety evaluations for transtympanic laser stimulation of the cochlea in Mongolian gerbils (Meriones unguiculatus).

Infrared laser stimulation of the cochlea has been proposed as a possible alternative to conventional auditory prostheses. Whereas previous studies have focused primarily on the short-term effects of laser stimulation, the practical application of this technics requires an investigation into whether prolonged laser exposure can induce neural responses and safely. This study assessed the effect of laser-induced damage to the cochlea on auditory perception using Mongolian gerbils (Meriones unguiculatus) trained with a classical conditioning task. The broadband noise was presented as a conditione

Multisensory interactions of face and vocal information during perception and memory in ventrolateral prefrontal cortex.

The ventral frontal lobe is a critical node in the circuit that underlies communication, a multisensory process where sensory features of faces and vocalizations come together. The neural basis of face and vocal integration is a topic of great importance since the integration of multiple sensory signals is essential for the decisions that govern our social interactions. Investigations have shown that the macaque ventrolateral prefrontal cortex (VLPFC), a proposed homologue of the human inferior frontal gyrus, is involved in the processing, integration and remembering of audiovisual signals. The scalp time-varying network of auditory spatial attention in "cocktail-party" situations.

Sound source localization in "cocktail-party" situations is a remarkable ability of the human auditory system. However, the neural mechanisms underlying auditory spatial attention are still largely unknown. In this study, the "cocktail-party" situations are simulated through multiple sound sources and presented through head-related transfer functions and headphones. Furthermore, the scalp time-varying network of auditory spatial attention is constructed using the high-temporal resolution electroencephalogram, and its network properties are measured quantitatively using graph theory analysis. Effects of age on brainstem coding of speech glimpses in interrupted noise.

Difficulty understanding speech in fluctuating backgrounds is common among older adults. Whereas younger adults are adept at interpreting speech based on brief moments when the signal-to-noise ratio is favorable, older adults use these glimpses of speech less effectively. Age-related declines in auditory brainstem function may degrade the fidelity of speech cues in fluctuating noise for older adults, such that brief glimpses of speech interrupted by noise segments are not faithfully represented in the neural code that reaches the cortex. This hypothesis was tested using electrophysiological re
