Open vs. arthroscopic acromioplasty: a prospective, randomized study.

Between May 1988 and May 1990, 44 patients with Stage II impingement were randomized into open and arthroscopic treatment groups. Forty-one patients were available for final follow-up in May 1991: 22 in the open group, 19 in the arthroscopic group. Comparisons of pain, function, motion, and strength were made preoperatively and at 2, 6, 12, 26, and 52 weeks postoperatively. Final analysis showed that the main benefits of arthroscopic acromioplasty were evident in the first 3 months postoperatively. Arthroscopic patients regained flexion and strength more rapidly than did open patients, had sho

Patient outcomes following arthroscopic distal clavicle excision: a prospective case series.

BACKGROUND: Distal clavicle excision for acromioclavicular joint (ACJ) pathology is currently the mainstay of surgical management in patients with symptoms refractory to nonoperative treatment. There have been few high quality studies outlining the efficacy of arthroscopic excision of the distal clavicle as a single procedure in patients with isolated disease. AIM: To characterize function and pain outcomes in patients undergoing arthroscopic distal clavicle excision by utilizing stringent inclusion criteria to isolate ACJ pathology. METHODS: Prospective data collection was undertaken with a…

Comparing the locking screw direction of three locking plates for lateral clavicle fractures: a simulation study.

BACKGROUND: The locking plate is a useful treatment for lateral clavicle fractures, however, there are limits to the fragment size that can … ensional clavicle models. Lateral fragment sizes of 10, 15, 20, 25, and 30 mm were simulated in the acromioclavicular joint, and the number of screws that could be inserted in the lateral fragment was assessed. Subsequently, the area covered by the locking screws on the inferior surface of the clavicle was measured. RESULTS: The distal clavicle plate had relatively larg …

A pilot study to evaluate micro-fragmented adipose tissue injection under ultrasound guidance for the treatment of refractory rotator cuff disease in wheelchair users with spinal cord injury.

CONTEXT/OBJECTIVES: Wheelchair users with chronic shoulder pain have few options after conservative treatments fail. This pilot study's purpose was to establish safety and treatment effects of micro-fragmented adipose tissue (MFAT) injections under ultrasound guidance for treatment of refractory shoulder pain caused by rotator cuff disease in wheelchair users with spinal cord injury (SCI) to prepare for a larger trial. DESIGN: Pilot clinical trial. SETTING: Rehabilitation hospital outpatient clinic. PARTICIPANTS: Ten wheelchair users with chronic SCI who had moderate-to-severe shoulder pain

Arthroscopic subacromial decompression: results and factors affecting outcome.

Arthroscopic subacromial decompression was performed on 114 patients with rotator cuff impingement who had not responded to nonoperative measures. None of the patients had a full-thickness cuff tear. Patients with associated instability, symptomatic acromioclavicular joint disease, or ruptures of the long head of biceps were not included. Results were determined by questioning patients about their satisfaction with the outcome of surgery and by functional assessment of the shoulder with the parameters of pain, ability to perform daily activities, and range of motion according to the Constant s…

Massive rotator cuff tears. Screening by routine radiographs.

To clarify their usefulness in screening massive rotator cuff tear, routine radiographs of the shoulder were compared in three groups: (1) shoulders with a massive rotator cuff tear (MRCT), (2) shoulders with a small full-thickness rotator cuff tear (SRCT), and (3) normal control shoulders. Two different statistical methods were employed to assess the gradation (order) of correlation in 83 shoulders (22 were MRCTs, 31 with SRCTs, and 30 controls). Superior migration of the humerus and deformity of the greater tuberosity showed by far the more significant contribution to MRCT than did other abn

Outcomes and function of conoid ligament on the basis of postoperative radiographic findings of arthroscopic stabilization for the distal clavicle fractures.

PURPOSE: Distal clavicle fractures are divided into three types according to Neer's classification. These fractures are usually treated with … ement of the proximal fragment. Although the displacement was superoposterior in all the cases, the acromioclavicular joint was maintained. The mean duration of postoperative follow-up was 2 years and 5 months. RESULT: There were no injury-related complications during the surgery and bony union was achieved within 3 months after surgery. Evaluation using 3DCT also showe …

Limb-threatening ischemia secondary to a congenital acromioclavicular remnant.

Upper extremity vascular compromise from thoracic outlet syndrome is rare and is usually the result of a "cervical rib," anterior scalene muscle abnormality, or clavicular trauma. We report a case of acute axillary artery thrombosis secondary to a congenital acromioclavicular remnant in a 40-year-old woman.

Complications after anatomic fixation and reconstruction of the coracoclavicular ligaments.

BACKGROUND: Reconstruction of the disrupted acromioclavicular (AC) joint has historically resulted in high complication rates. As a result, there has been a move toward anatomic coracoclavicular (CC) ligament fixation and reconstruction, owing to its numerous biomechanical advantages and perceived clinical advantages. PURPOSE: To report and analyze the unique complications associated with these anatomic CC ligament procedures using either cortical fixation buttons (CFBs) or tendon grafts (TGs) and to evaluate the effect that these complications have on patient outcomes. STUDY DESIGN: Case se

Whole-body MRI in patients with arthralgia or inflammatory arthritis after exposure to immune checkpoint inhibitors: a single-centre prospective imaging study.

BACKGROUND: Musculoskeletal adverse events due to immune checkpoint inhibitors are common and can present clinically as inflammatory arthritis, polymyalgia rheumatica, or arthralgia. The pathoanatomy of musculoskeletal adverse events related to immune checkpoint inhibitors remains undefined, with a paucity of available imaging data. We aimed to investigate the whole-body imaging phenotype of arthralgia and inflammatory arthritis following exposure to immune checkpoint inhibitors, to fully characterise the pattern of inflammation in these patients and subsequently inform clinical management. Fractures of the clavicle in the adult. Epidemiology and classification.

From 1988 to 1994 a consecutive series of 1000 fractures of the adult clavicle was treated in the Orthopaedic Trauma Clinic of the Royal Infirmary of Edinburgh. In males, the annual incidence was highest under 20 years of age, decreasing in each subsequent cohort until the seventh decade. In females, the incidence was more constant, but relatively frequent in teenagers and the elderly. In young patients, fractures usually resulted from road-traffic accidents or sport and most were diaphyseal. Fractures in the outer fifth were produced by simple domestic falls and were more common in the elderl

Do multiple fractures affect risk of fixation failure after surgical treatment of midshaft clavicle fracture? A retrospective cohort study.

BACKGROUND: Surgical treatment of midshaft clavicle fractures is associated with quick recovery and low risk of non-union. However, fixation failure may occur in case of severe comminution fractures. Moreover, clinical outcomes may be affected when clavicle fractures occur in combination with other injuries, particularly those involving the lower extremities, as the use of crutches or walkers may hinder the process of rehabilitation by adding strain on the acromioclavicular (AC) joint, resulting in possible fixation failure. This study aims to identify risk factors for fixation failure of mids
