The effect of digoxin on the response to lithium therapy in mania.

Patients suffering from manic-depressive psychosis, manic type (ICD 296.0), were treated with lithium carbonate and randomly allocated to two groups, one received digoxin and the other matching placebo for 7 days. Severity of mania was rated by psychiatrists on the Manic Rating Scale and Analogue Line on days 0 and 7 and by nurses daily on the Hargreaves Rating Scale, Psychotic Rating. Fourteen patients received digoxin and lithium carbonate and 14 patients received placebo and lithium carbonate. Improvement in the placebo lithium group was significantly greater than that in the digoxin lithiu

[Dangerous side effects of lithium therapy].

A report is made on two cases of a pharmacogenic delirium as a result of combined lithium-haloperidol therapy, and on one case of lithium-induced diabetes insipidus renales with second-degree high pressure. Also, the development of a hydrocephalus internus male-resorptivus through lithium is discussed. In all patients there was a possible disposition due to old age or cerebral injury. Based on their effect on synaptic transmission the potentiating effect of lithium and haloperidol is discussed. Under the influence of Na-K stimulated ATPase and, thus, of active transport and adenylcyclase a bas

Acute Effect of Physical Exercise on Negative Affect in Borderline Personality Disorder: A Pilot Study.

BACKGROUND: Physical exercise is an evidence-based treatment to reduce symptoms and negative affect in several psychiatric disorders, including depressive, anxiety, and psychotic disorders. However, the effect of physical exercise on negative affect in patients with borderline personality disorder (BPD) has not yet been investigated. In this pilot study, we tested the safety, acceptability, and potential acute effects on negative affect of a single session of aerobic physical exercise in adults with BPD. METHOD: After completing a negative mood induction procedure, 28 adults with BPD were ran

The reliability of psychiatric diagnosis in the emergency room.

The authors compared the diagnoses made for 50 patients in an emergency room with those made during a subsequent inpatient hospitalization. They found an acceptable level of reliability for broad diagnostic categories, such as psychosis, depression, and alcoholism. The authors believe such diagnostic reliability is sufficient for emergency assessment and triage. However, the diagnosis of more specific subtypes of mental illness, such as schizophrenia and bipolar disorder, were not made reliably in the emergency room. The authors point out the risk of diagnostic labeling, and suggest that the t

The acute administration ward as a therapeutic community.

The authors evaluated six acute short-stay admission wards in a community mental health center using the sociometric Ward Atmosphere Scale. There was an attempt to run three of the six wards as therapeutic communities. No differences were found on Ward Atmosphere Scale dimensions between the therapeutic communities and other types of programs, nor did the therapeutic communities conform to an empirically derived therapeutic community program dimension profile. Possible reasons for this failure and its implications for community mental health centers are discussed.

Sleep disturbance may impact treatment outcome in bipolar disorder: A preliminary investigation in the context of a large comparative effectiveness trial.

BACKGROUND: Bipolar patients experience sleep disturbances during and between mood episodes. Yet the impact of sleep on treatment with different medications has not been fully explored. The purpose of this paper is to explore the potential impact of poor sleep at baseline on outcomes in a randomized effectiveness trial of quetiapine and lithium. METHODS: The Bipolar CHOICE study was a 6-month, parallel group, multisite randomized controlled trial. Participants with bipolar disorder (N = 482; 59% female and age 18-70 years) received quetiapine or lithium. Patients were allowed to also receive

[Homicide and major mental disorder: what are the social, clinical, and forensic differences between murderers with a major mental disorder and murderers without any mental disorder?].

OBJECTIVES: To establish the social, clinical, and forensic differences between murderers suffering from a major mental disorder and murderers without any psychiatric disorder and, in particular, to compare their respective records of psychiatric symptoms and their respective relationship with their victims. METHOD: We studied 210 forensic examinations of murderers, the offences related to the murders, and the social and clinical information collected from psychiatric court reports on persons convicted of homicide. Firstly, we identified the socio-demographic, clinical and criminological prof

Prevalence of substance abuse in a psychiatric evaluation unit.

The prevalence of substance abuse and psychiatric illness was studied in a Psychiatric Evaluations Unit. Twenty-six percent of the subjects received a psychiatric diagnosis only with no concomitant substance use disorder. Thirty-four percent were diagnosed with a substance use disorder but with no other psychiatric disorders. Thirty-nine percent of the subjects had a history of both psychiatric and substance use disorder; 62% of these substance abusers with a psychiatric illness reported using drugs (including alcohol) the week before the interview; 56% used illicit drugs while 44% used alcoho

Diagnostic categorization of psychiatric disturbance in Cushing's syndrome.

The author obtained a longitudinal psychiatric history from 30 patients with proven Cushing's syndrome. Twenty-five (83%) of the patients met strict diagnostic criteria for an episode of affective disorder during the course of their endocrine disturbance. Twenty patients met criteria for endogenous depression and eight patients also reported an episode of mania or hypomania. Schizophrenic syndromes were not evident, although two patients met criteria for a psychotic depression. Patients frequently attempted to minimize or conceal serious psychiatric disturbance, including a suicide attempt. Ty

Catatonia associated with epileptic seizures: A systematic review of case reports.

The association of catatonia with epileptic seizures is a rare phenomenon that is poorly understood and needs more clinical research. This systematic review included all published case reports of patients with catatonia meeting ICD-11 criteria associated with epileptic seizures, published until December 2021 in PubMed. Case reports were synthesized and results were expressed as percentages. In total, 42 articles with 52 case reports were included. Most patients were adults with a dispersed age (mean age 44.9 ± 19.3), slightly more males (59.6 %), with psychiatric history (76.9 %) of affective

Pineal morphology of the clinical high-risk state for psychosis and different psychotic disorders.

BACKGROUND: Pineal volume reductions have been reported in schizophrenia and clinical high-risk states for the development of psychosis, supporting the role of melatonin dysregulation in the pathophysiology of psychosis. However, it remains unclear whether pineal volume is associated with the later onset of psychosis in individuals at clinical high-risk (CHR) of psychosis or if pineal atrophy is specific to schizophrenia among different psychotic disorders. METHODS: This magnetic resonance imaging study examined the volume of and cyst prevalence in the pineal gland in 135 individuals at CHR o

COVID-19-associated psychosis in Indian settings: A retrospective descriptive study.

BACKGROUND: The coronavirus-19 (COVID-19) pandemic has affected millions of people across the world since early 2020. Besides the large number of case fatalities, this virus has produced significant health-related sequelae involving multiple systems of the body. As with previous coronavirus infections, this was also found to be associated with various neuropsychiatric symptoms. Psychosis has been uncommon, and the few reported cases across the world have forwarded association with either raised inflammatory markers or the consequences of social isolation. MATERIALS AND METHODS: This is a retr

Analysis of genetic association and epistasis interactions between circadian clock genes and symptom dimensions of bipolar affective disorder.

Bipolar affective disorder (BD) is a severe psychiatric disorder characterized by periodic changes in mood from depression to mania. Disruptions of biological rhythms increase risk of mood disorders. Because clinical representation of disease is heterogeneous, homogenous sets of patients are suggested to use in the association analyses. In our study, we aimed to apply previously computed structure of bipolar disorder symptom dimension for analyses of genetic association. We based quantitative trait on: main depression, sleep disturbances, appetite disturbances, excitement and psychotic dimensi

Gender differences in incidence and age at onset of mania and bipolar disorder over a 35-year period in Camberwell, England.

OBJECTIVE: Despite clear gender differences in the symptoms and course of bipolar affective disorder, studies investigating age at onset by gender have yielded inconsistent results. The authors investigated gender differences in age at onset and incidence of first-episode mania and bipolar disorder in an epidemiological catchment area in southeast London over a 35-year period. METHOD: All adult cases of first-episode psychosis, mania, or hypomania presenting to services in Camberwell, southeast London (1965-1999), were identified. Computerized diagnoses for these cases were generated by using

Outcomes for migrants with a first episode of psychosis: A systematic review.

Migrants are at a greater risk of developing a psychotic disorder, but knowledge about their outcomes has not yet been collated. We reviewed the literature concerning outcomes (symptomatic, functional, morbidity and mortality and service utilisation) for migrants with first-episode psychosis. Medline, EMBASE and PsycINFO were systematically searched for studies whose population included first- or second-generation migrants with first-episode psychosis, where outcome measures were examined after a minimum follow-up period of 6 months. Fourteen of the 1554 articles identified in the search fulfi

Epidemiology of first-episode psychosis: illustrating the challenges across diagnostic boundaries through the Cavan-Monaghan study at 8 years.

The epidemiology of first-episode psychosis is poorly understood because of the paucity of systematic studies, yet it constitutes the fundamental basis for understanding the disorder and the foundations on which clinical, biological, therapeutic, and long-term outcome studies are built. A particular need is to clarify the diagnostic breadth of first-episode psychosis and, on this basis, to undertake systematic comparisons across representative populations of the psychoses, to include comparisons with first-episode mania. Considered here is the new generation of prospective studies that may be

Psychiatric disorders and pruritus.

The skin and psyche are intimately related with various skin diseases caused by or resulting in psychiatric disturbances. Pruritus is a commonly reported symptom in psychiatric patients, and likewise psychiatric co-morbidities, including anxiety and depression, are frequently seen in chronic pruritus patients. Primary psychodermatologic conditions, such as somatic symptom disorder, dermatitis artefacta, obsessive-compulsive and related disorders (excoriation disorder and prurigo nodularis), delusional infestation, and substance use disorder, can all induce significant pruritus in patients, sev

Sociodemographic and clinical characteristics of suicide methods in Denmark: A nationwide registry-based study of suicides during 2010-2020.

BACKGROUND: Suicide mortality is influenced by the lethality of the method used, making it essential for effective suicide prevention. This nationwide study examined whether suicide methods differ by sociodemographic and clinical characteristics among individuals who died by suicide in Denmark. METHODS: We conducted a retrospective register-based cohort study including all suicide decedents in Denmark between 2010 and 2020 (n = 6257). Data were obtained from Danish national health registers. Multinomial logistic regression examined associations between individual characteristics and suicide m

Diagnostic Stability of Acute and Transient Psychotic Disorders at a Tertiary Care Center: A Retrospective Record-Based Study.

Background Acute and transient psychotic disorders (ATPD) have been a diagnostic enigma due to their fleeting nature. While classified in various systems, discrepancies continue between the WHO's International Classification of Diseases, 10th Edition (ICD-10), International Classification of Diseases, 11th Edition (ICD-11), and Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition (DSM-5). Research on ATPD, especially in developing countries like India, is scarce, leading to uncertainty about their prevalence and diagnostic stability. Aim and objective This study aims to investi
