Anxiety disorders are the most common type of mental disorders and include such core forms as generalized anxiety disorder, panic disorder, agoraphobia, specific phobias, and social anxiety disorder. Anxiety disorders contribute significantly to the global burden of disease, and the diagnosis and treatment of these disorders is of great social and economic importance. Current approaches to the treatment of anxiety disorders involve the use of cognitive behavioral therapy, antidepressants, benzodiazepines, antipsychotics, few anticonvulsants, and some other medications.

Rational treatment of panic disorders with antidepressants rests on decisions of drug choice, dosage, and duration of treatment. In this paper, we selectively review the author's research with the standard antidepressant, imipramine, in the treatment of panic disorder with agoraphobia as it relates to practical issues. We develop general guidelines for treatment, suggest researchable ways of increasing the net effectiveness of treatment with this class of drugs, and discuss limited generalizations to the extant literature on selective serotonin reuptake inhibitors in panic disorders.

Panic disorder is a devastating condition with a higher prevalence in female patients and young adults. Panic disorders is frequently associated with major depression, agoraphobia, substance abuse, suicide attempts, and impaired quality of life. Only one half of the subjects with panic disorder seek care for their attacks, and those who do seek care tend to present to primary care and emergency department settings. If treated appropriately, panic disorder has a favorable prognosis.

The separation of social phobia from normal social anxiety (shyness) and from other psychiatric disorders (e.g. agoraphobia, atypical depression with rejection sensitivity) is as important for research and for treatment decisions as is the definition of subtypes. While empirical evidence supports the distinction of a generalized and a specific subtype of social phobia, the other boundary issues, the separation of social phobia from shyness and its distinction from the comorbidity with other psychiatric disorders, still require research.

A survey of an inner-city population of working-class and single mothers is described and the prevalence of anxiety and depression reported using two related diagnostic schemes, Bedford College caseness and DSM-III-R. This acts as an introduction to an analysis which indicates that adverse experiences in childhood and adolescence (involving parental indifference, and sexual and physical abuse) considerably raise risk of both depression and anxiety conditions (with the exception of mild agoraphobia and simple phobia) in adult life.

A 41-year-old Canadian man presented with a 1-week history of new bizarre behavior including agoraphobia. Nested PCR confirmed the presence of rabies in both nuchal snips and saliva. An initial MRI with gadolinium enhancement was normal. On day 8, repeat neuroimaging revealed new cranial nerve enhancement and slight hyperintensities of the caudate nuclei bilaterally (figure). This is consistent with cranial nerve involvement reported in animal studies1 and with a remote report of electron microscopic evidence of rabies virus in trigeminal ganglion cells reported in 2 human cases.2

In this article, we review the status of psychotherapy research at the time of the founding of the journal Psychotherapy, and trace its history over the past 50 years, spanning the career of the first author. We trace this history in the context of the development of treatments for panic disorder and agoraphobia emanating from our research program. In so doing, we discuss the early visions of Strupp and Eysenck and the realization of many of their own goals for psychotherapy research. We conclude with a view toward the future based on cumulative knowledge of psychotherapy and psychopathology a…

Psychiatric manifestation of dengue virus infections is seldom reported. Here a case is presented of a 21-year-old man who developed mainly neuropsychiatric symptoms which he caught one week after a feverish infection during his stay in India: troubles with concentration and memory, confusion, as well as depressive delusions and agoraphobia. Liquor diagnosis as well as CCT and MRI was mainly inconspicuous. The FDG-PET showed a diffuse lack of activity. IgM and IgG antibodies of dengue virus were positive. We interpreted these psychiatric symptoms as a result of a dengue virus infection.

We examined the relationship between self-reported anxiety and physiological measures (blood pressure and heart rate) in a series of exposures to a feared situation of a single participant with panic disorder with agoraphobia. During each exposure, readings of heart rate, systolic blood pressure and diastolic blood pressure were taken every 20 s. Over 30 exposures, we found a near-linear relationship between anxiety and the three physiological measures. Implications of this result for usefulness of physiological measures in anxiety research and in the clinical treatment of anxiety disorders ar…

This article reviews the historical development of the category of panic disorder in the United States, particularly the shifting perspectives on both what causes panic and how the presence of panic should be determined. The notion that panic attacks of a panic-disorder type must be "out of the blue" and "unexpected," except in the case of triggering by a particular place (i.e., agoraphobia), is critiqued. The authors illustrate that a meaningful epidemiological determination of panic rates in other cultural groups must be preceded by a detailed ethnography that ascertains the catastrophic cog…

Pathological anxiety--painful and incapacitating--can be defined as perception of stress with cognitive distortion. The traditional unitary model of anxiety neurosis is replaced by the american model (DSM III) with "panic disorder" and "panic attacks" representing the cornerstone of the new construct, based on the evidence (by Donald Klein) of the efficacy of so-called antidepressant drugs. Behavioral and cognitive treatments are useful, mainly in anxiety with agoraphobia. "General anxiety disorder" is now considered as a "residual category". Clinical aspects, differential diagnosis, circumsta…

This paper presents the cases of two patients who suffered from panic disorder with agoraphobia and depression. One had been refractory to alprazolam and tricyclics and to behaviour therapy; she had responded to phenelzine, but due to a weight gain of 50 lbs, had discontinued treatment and she relapsed. The second patient, who also suffered from post-traumatic stress disorder, did not respond to alprazolam, imipramine or to phenelzine, but gained weight (33 lbs) on phenelzine. Both patients responded to fluoxetine 80 mg per day without concomitant weight gain.

This study investigated the relationship between the development of panic disorder and attribution style by administering the Attribution Style Questionnaire (ASQ) to 28 subjects with panic disorder with agoraphobia and 21 subjects with other anxiety disorders who had experienced a panic attack at some time. No significant differences were found between the groups suggesting that cognitive style as assessed by the ASQ may not predispose to the development of panic disorders. A factor analysis of the results indicated that the ASQ may not be appropriate for the assessment of patients with panic…

Circadian rhythm abnormalities have been demonstrated in people with depression, including a tendency toward maximal symptom severity in the morning. Although a few studies have suggested that symptoms in people with anxiety are worse later in the day, no detailed study of this observation has been reported. In 86 patients with anxiety disorders (63 with panic disorder or agoraphobia with panic attacks), anxiety symptoms tended to be more severe in the afternoon or evening than in the morning, with no abnormalities of heart rate or oral temperature. This is the first systematic demonstration o…

Current psychiatric classifications of anxiety are examined critically. In the European literature a specific cluster of symptoms is diagnostic of anxiety neurosis and their subsidiary classification is based on precipitating factors and duration of symptoms. In the American literature, exemplified by DSM-III, greater emphasis is placed on symptomatic classification and panic is given separate diagnostic status, both alone and in conjunction with agoraphobia. In both classifications generalized anxiety is at the bottom of a diagnostic hierarchy so that all other symptoms take precedence. It is…

BACKGROUND: Phobia is an extreme form of anxiety or fear which is triggered by a given situation or object or even danger associated with a situation or object for more than 6 months. There are three main forms of phobia: social phobia (social anxiety disorder), specific phobia, or agoraphobia. Agoraphobia is a form of anxiety disorder where one fears and avoids situations or places that might cause them to panic. The main purpose of this study is to investigate phobia among residents in broad training programs in Abha city and compare phobia between residents in broad training programs and ge…

BACKGROUND: Panic disorder, with or without agoraphobia (PDA or PD, respectively), is a major public health problem. After having established a PD diagnosis based on the DSM or the ICD systems, the Panic Disorder Severity Scale (PDSS) is the most widely used interview-based instrument for assessing disorder severity. There is also a self-report version of the instrument (PDSS-SR); both exist in a Swedish translation but their psychometric properties remain untested.
METHODS: We studied 221 patients with PD/PDA recruited to a randomized controlled preference trial of cognitive-behavioral and b…
