A case report and DSA findings of cerebral hemorrhage caused by syphilitic vasculitis.

Syphilis is now rare and easily misdiagnosed because of the wide use of antibiotics in the clinical. We report a case of cerebral hemorrhage in a patient with hypertension who was first diagnosed as hypertensive cerebral hemorrhage. However, treponema pallidum particle agglutination and rapid plasma regain tests of cerebrospinal fluid revealed the existence of neurosyphilis. Interestingly, digital subtraction angiography (DSA) showed severe stenosis in both middle cerebral arteries and right anterior cerebral artery. The case reminded us to pay attention to syphilitic vasculitis in patients wi

Bilateral vertebral artery dissection caused by atlantoaxial dislocation.

UNLABELLED: Craniovertebral anomalies are rare causes of vertebral artery dissection. Therefore, appropriate evaluation is necessary in vertebrobasilar strokes, specially in the young. This is particularly true for patients with vertebral stroke without risk factors. RESULTS: Here we report a 24-year-old male presenting with posterior circulation infarcts. The patient had no obvious vascular risk factors. However, digital subtraction angiography showed chronic dissection of both vertebral arteries with partial recanalization. Further evaluation with computed tomogram of neck revealed atlantoa

Successful transcatheter arterial embolisation of a cystic artery pseudoaneurysm secondary to calculus cholecystitis: a case report.

Pseudoaneurysms of the cystic artery secondary to calculus cholecystitis are rare. In this report we describe a case of an elderly female who presented with abdominal pain, pyrexia, anaemia and jaundice. She had known chronic cholecystitis, but was not considered a suitable surgical candidate. Contrast enhanced computed tomography (CECT) demonstrated a probable aneurysm within the gallbladder fossa. The patient proceeded to digital subtraction angiography (DSA), which confirmed an aneurysm arising from the cystic artery and was subsequently managed with transcatheter arterial embolisation usin

Ruptured aneurysm at an anastomotic artery extending from the vertebral artery to the posterior inferior cerebellar artery: a case report.

A 51-year-old hypertensive man presented with subarachnoid haemorrhage. He had a past history of cerebellar infarction due to occlusion of the right posterior inferior cerebellar artery (PICA) 4 years earlier. Digital subtraction angiography showed a saccular aneurysm above an arterial loop extending from the vertebral artery to the distal part of the PICA, reminiscent of peripheral PICA branches. We performed aneurysmal neck clipping with excellent outcome. Aneurysms at anastomotic arteries are extremely rare and can result from increased haemodynamic stress. We report the first case of a rup

Clinical implications of cortical microvasculature in adult Moyamoya disease.

We analyzed cortical microvascular parameters using intraoperative ICG (Indocyaninegreen)-Videoangiography in 13 patients with Moyamoya disease, and carried out correlative studies by comparing them with clinical parameters obtained by digital subtraction angiography, physical examination, and regional cerebral blood flow studies. Patients with reduced cerebrovascular reserve capacity were characterized by increased microvascular surface area (MVSA). In addition, MVSA correlated positively with arterial microvascular transit time. Asymptomatic patients were characterized by increased arterial

Fibromuscular dysplasia: noninvasive evaluation of unusual case of renal and mesenteric involvement.

Fibromuscular dysplasia is the most common cause of renovascular hypertension in young patients. It primarily involves the renal and carotid arteries, and it is less common in the coronary, iliac, and visceral arteries. Digital subtraction angiography is still the best investigation to determine the location, extent, and complications of renal artery involvement. However, currently, other imaging modalities such as ultrasonography, computed tomography, and magnetic resonance imaging can reveal these findings noninvasively. We present the case of a 43-year-old woman who presented with high bloo

Gadolinium-based balloon angioplasty for pulmonary artery stenosis in an infant with a right isomerism.

We report here the first described case of utilizing gadolinium-based contrast material as the contrast agent during a catheter intervention treatment for pulmonary artery stenosis. The patient, a male infant with complex heart disease associated with a right isomerism, had a history of severe allergic reaction to iodine-containing contrast agents. A combination of digital subtraction angiography and a gadolinium contrast agent, however, provided us with good-quality images both before and after balloon angioplasty without any associated complications. This method should therefore be considere

[Stent PTA in secondary symptomatic proximal subclavian artery obstruction].

Stent-PTA of secondary symptomatic proximal subclavian artery stenosis In comparison with conventional surgical revascularisation, percutaneous transluminal angioplasty (PTA) is an alternative treatment for short stenoses or occlusions involving the origin of the subclavian artery. If there is clinical suspicion of subclavian artery obstruction (e.g. blood pressure difference in both arms), digital subtraction angiography of the aortic arch and upper limb should be performed prior to creating radial haemodialysis shunts or coronary bypass crafting involving the internal mammary artery. PTA and

Ruptured fusiform cerebral aneurysm in a neonate.

An 11-day-old male infant presented with subarachnoid and intraventricular haemorrhage caused by a ruptured intracranial aneurysm. Magnetic resonance (MR) angiogram and digital subtraction angiography (DSA) revealed a fusiform aneurysm 10 mm in diameter supplied by the proximal segment of the anterior cerebral artery (A1), with both distal segments (A2) arising from the aneurysm. The right A1 was aplastic. There have been 13 previous case reports of neonatal cerebral artery aneurysms, but only 1 of these has been fusiform. None of the earlier reports has mentioned dysplastic segments or other

[A diagnosis of venous anomaly with dextrocardia by magnetic resonance imaging and transesophageal echocardiography].

A 56-year-old woman was admitted with the chief complaint of dyspnea. Chest radiograph and echocardiography disclosed mirror image dextrocardia with massive pleural effusion. Magnetic resonance imaging (MRI) and transesophageal echocardiography (TEE) revealed dilated coronary sinus and persistent right-sided superior vena cava which entered the coronary sinus. These findings were compatible with those of digital subtraction angiography. Anomalies of the venous system must be recognized before surgery because adequate venous drainage is essential for extra-corporeal circulation. MRI and TEE are

[Blunt disruption of the brachiocephalic artery].

A 59-year-old female having the disrupted brachiocephalic artery (BCA) and multiple bone fractures caused by a traffic accident was transferred to our hospital in a hypovolemic shock status with a wide mediastinum on the chest radiograph. Intraarterial digital subtraction angiography was effective in confirming the bleeding site of the artery. The patient underwent an urgent operation 4 hours and 22 minutes after admission. Through median sternotomy the BCA was simply clamped, followed by a direct suture repair of the 2/3-circumferential transection on the proximal portion of BCA. The patient

Prolonged survival in a child with renovascular hypertension from abdominal-pulmonary Takayasu's arteritis.

Takayasu's disease is an inflammatory condition of the large arteries which comprises circulation to the head, extremities, and, less often, the abdominal viscera or lungs. A 10-year-old girl with renovascular hypertension secondary to Takayasu's arteritis has been followed for over 5 years. Her course is unusual in that (1) her disease is not related to tuberculosis; and (2) despite a persistently active disease course, she has been allowed to experience a fairly normal life-style through continued, extensive medical and surgical management. Vascular involvement has been documented by digital

Surgical treatment of renal artery occlusive disease: long term results.

68 patients were operated on for renal artery occlusive disease. After a mean follow-up of 61 months renovascular hypertension was cured or markedly improved in 79.2%. The results depended mostly on the duration of preexisting hypertension and on the histology of the renal artery lesion. Forty-four months after surgery, 37 reconstructions were examined by angiography. Mostly digital subtraction technique was used. Total occlusion rate was 24%. Patency rates of autogenous vein bypass grafts were similar to prosthetic grafts. It is concluded that prosthetic grafts like PTFE are a reasonable alte

The 'corkscrew' sign: an indirect MRI hint for intracranial venous hypertension.

Dural arteriovenous fistulas (DAVFs) are intracranial vascular abnormalities in which one or more meningeal arteries shunt into a venous structure, either a cortical vein or a venous sinus, causing cerebral venous hypertension and risk of haemorrhage. Imaging diagnosis and characterisation are of paramount importance to grade the haemorrhagic risk and direct management. Non-invasive vascular neuroimaging might pose a diagnostic suspicion, but invasive catheter digital subtraction angiography (DSA) is usually required. We present the case of a patient with an atypical acute cerebral haemorrhage

Separate origins of the left internal and external carotid arteries from the aorta in a patient with intracerebral hemorrhage.

Agenesis of the left common carotid artery with separate origins of the left internal and external carotid arteries from the aorta is an extremely rare anomaly. This anomaly is typically asymptomatic unless associated with other conditions. We report a case of separate origins of the left internal and external carotid arteries from the aorta in a patient with intracerebral hemorrhage. A 42-year-old man was transferred to our hospital by ambulance because of left hemiparesis. Computed tomography scan revealed right putaminal hemorrhage. Computed tomography angiography and digital subtraction an

Current status of perimesencephalic non-aneurysmal subarachnoid hemorrhage.

Perimesencephalic nonaneurysmal subarachnoid hemorrhage (PNSAH) is a distinctive disease, representing SAH centered in perimesencephalic cisterns, with negative angiography findings. In recent years, the number of patients with PNSAH has increased significantly; however, the knowledge of PNSAH is insufficient. Therefore, we performed a review of the literature from a PubMed search and recounted our understanding of PNSAH. In this review, we summarized that current high-resolution computed tomography angiography is an acceptable replacement for digital subtraction angiography to rule out aneury

Catheter-based management of a catheterization related stroke.

Ischemic stroke is a rare and serious complication of coronary angiography and percutaneous coronary intervention, which has high morbidity and mortality. To our knowledge, there is no large-scale randomized controlled trial for the management of catheter-related ischemic stroke. In this case study, we presented a 46-year-old male with peri-procedural ischemic stroke during the coronary angiography (CAG). The CAG was terminated after the stroke and the left carotid artery was selectively cannulated, and digital subtraction angiography (DSA) revealed total occlusion (Modified Thrombolysis in Ce

Asymmetric dilatation of virchow-robin space in unilateral internal carotid artery steno-occlusive disease.

OBJECTIVE: The cause of abnormal Virchow-Robin space (VRS) dilatation is still unclear. The purpose of this study was to test the hypothesis that chronic ischemia from the unilateral significant internal carotid artery (ICA) stenosis is related to asymmetric VRS dilatation. METHODS: We recruited 78 patients with severe unilateral ICA stenosis (>70%) diagnosed by magnetic resonance angiography, computed tomography angiography, or digital subtraction angiography and retrospectively reviewed 3-T brain magnetic resonance images. All VRSs on bilateral cerebral high-convexity areas were scaled into
