As has been previously discussed, the incidence and resultant associated disabilities of ankle sprains have been well documented in the literature. Eighty-one reconstructions of the lateral ligaments of the ankle were carried out at our clinics between 1967 and 1983. Three methods were used and 53 patients were followed up. The only two poor results followed infection after dural reconstruction. Peroneus brevis reconstruction, dural reconstruction and ligament advancement all had good results in over 80% of the patients. There was restriction of supination and an increase of osteoarthritis after the peroneus brevis reconstruction operations. Statistical analysis did not show that any one of these procedures was better than any other.

We report the results of treatment in eight patients who had posteromedial dislocation of the ankle without fracture. The ages of seven of the eight patients ranged from twenty-three to thirty-six years. Four patients had increased generalized ligamentous laxity. The management of ligamentous injuries to the ankle is controversial. Neither the methods for classification and diagnosis, or the procedures for treatment are clear cut. It is difficult to recognize tendon pathology in the ankle area when there is no open wound. This manuscript was written to indicate subtle characteristics associated with tendon trauma around the ankle. Failure to recognize these problems will result in improper treatment or lack of treatment and the results may be disastrous to the patient. Diagnostic aids, such as radiographs, ultrasound, or tenography, may be valuable; however, a careful physical examination should make it possible to diagnose correctly and to treat tendon pathology surrounding the ankle.

To study the incidence of fibulocollateral ligament ankle sprains in the young male athlete, a survey of 84 varsity basketball players was done. Seventy percent of the players had a history of an ankle sprain. Eighty percent of those with a positive history had multiple sprains. When combined with skillful pharmacologic management, applying the full stock of biomechanical principles available to the rheumatoid foot and ankle is very effective in avoiding the need for surgery and slowing or preventing deformity. The general principles of arthritis rehabilitation also help restore function. When good medical management, joint protection, exercise, and energy conservation techniques are combined with psychosocial vocational therapy as needed, the crippling loss of function so often seen in the past can be largely avoided.

This paper discusses some of the problems encountered in the diagnosis and treatment of 371 ankle fractures treated at the University of Kansas Medical Center over a 14-year period, 1965-1979. Seven of these injuries required major reconstructive procedures after failure of initial treatment and 11 fractures required eventual ankle arthrodesis. Twenty-two cases of multiple hereditary exostoses revealed pathological changes in two areas: the first group caused by epiphyseal disturbances, and the second group due to mechanical problems created by the exostoses. The epiphyseal disturbances resulted in ankle valgus, shortened metatarsals, and angular deformities of the necks of the metatarsals. The exostoses produced local tenderness, synostosis of the medical subtalar facet resulting is loss of subtalar motion, and asymptomatic synostosis of the lower tibial fibular syndesmosis.

An experimental study in 39 ankles of the mechanisms of lateral ankle sprains was carried out using a simple qualitative apparatus. Seventy-one cases of ankle fractures in children were studied. An analysis of the fracture lines involving the distal tibial and fibular epiphyses were done.
