Cost-effectiveness comparison of response strategies to a large-scale anthrax attack on the chicago metropolitan area: impact of timing and surge capacity.

Rapid public health response to a large-scale anthrax attack would reduce overall morbidity and mortality. However, there is uncertainty abo … odeling Working Group for population-wide responses to a large-scale anthrax attack: (1) postattack antibiotic prophylaxis, (2) postattack antibiotic prophylaxis and vaccination, (3) preattack vaccination with postattack antibiotic prophylaxis, and (4) preattack vaccination with postattack antibiotic prophylaxis and vaccination. Outcomes were measured in costs, lives sa …

Unresolved issues in the prophylaxis of bacterial infections in patients with cirrhosis.

Bacterial infections are highly prevalent and a frequent cause of hospitalization and short-term mortality in patients with cirrhosis. Due to their negative impact on survival, antibiotic prophylaxis for bacterial infections in high-risk subgroups of patients with cirrhosis has been the standard of care for decades. Patients with prophylaxis indications include those at risk for a first episode of spontaneous bacterial peritonitis (SBP) due to a low ascitic fluid protein count and impaired liver and kidney function, patients with a prior episode of SBP and those with an episode of gastrointest…

Antibiotic regimen and route of administration do not alter rates of urinary tract infection after intravesical botulinum toxin injection for overactive bladder.

INTRODUCTION AND HYPOTHESIS: Guidelines vary on antibiotic prophylaxis for onabotulinumtoxinA (Botox) treatment for overactive bladder (OAB). Our primary objective was to determine whether any prophylactic regimen is more effective in preventing urinary tract infection (UTI) after Botox. The secondary objective was to identify prophylactic practice patterns among female pelvic medicine and reconstructive surgery (FPMRS) providers of different training backgrounds as well as general urologists. METHODS: This was a secondary analysis of a retrospective cohort study on urinary retention after Bo…

Intracameral cefuroxime and moxifloxacin used as endophthalmitis prophylaxis after cataract surgery: systematic review of effectiveness and cost-effectiveness.

Postoperative endophthalmitis is one of the most serious potential complications of ocular lens surgery. Its incidence can be reduced by means of antibiotic prophylaxis. Although the prophylactic use of intracameral cefuroxime has been extended, other drugs, such as moxifloxacin, have arisen as alternatives. We performed a systematic literature review on the effectiveness and efficiency of intracameral cefuroxime and moxifloxacin for the prophylaxis of postoperative endophthalmitis after cataract surgery. Several bibliographic databases were searched up to October 2010 and were updated up to J…

Levofloxacin Prophylaxis During Induction Therapy for Pediatric Acute Lymphoblastic Leukemia.

BACKGROUND: Infection is the most important cause of treatment-related morbidity and mortality in pediatric patients treated for acute lymphoblastic leukemia (ALL). Although routine in adults with leukemia, antibacterial prophylaxis is controversial in pediatrics because of insufficient evidence for its efficacy or antibiotic choice and concerns about promoting antibiotic resistance and Clostridium difficile infection. METHODS: This was a single-center, observational cohort study of patients with newly diagnosed ALL, comparing prospectively collected infection-related outcomes in patients who

[Prophylactic antibiotic therapy in urology].

Prophylactic antibiotic therapy is used increasingly frequently in urology to prevent post-operative septic complications. The use of this method is based on experimental and clinical data which define the optimal time of administration of the antibiotic and the criteria for selection of the most effective drug. Transurethral resection of the prostate is the urological operation most frequently associated with antibiotic prophylaxis. It has been the subject of a large number of randomised studies which have confirmed the efficacy and usefulness of antibiotic prophylaxis. The other urological o…

Recommendations on prophylaxis for infective endocarditis: dramatic changes over the past seven years.

Recommendations on antibiotic prophylaxis against infective endocarditis have changed dramatically since 2002. In 2002, the French were the first to make a profound change: they proposed that antibiotic prophylaxis should be optional when a medical, surgical or dental procedure that carries a risk of infective endocarditis was performed in a patient at risk but not at very high risk of infective endocarditis (group B: native valve disease, non-cyanotic congenital heart disease, obstructive hypertrophic cardiomyopathy). In 2004, the European Society of Cardiology and the British Society of Card…

The management of Ixodes scapularis bites in the upper Midwest.

Ixodes scapularis, commonly referred to as the deer tick, is the vector of Lyme disease and anaplasmosis; both illnesses are endemic to the upper Midwest. Avoidance of I scapularis bites is the primary preventative strategy for both infections. Antibiotic prophylaxis has been demonstrated to prevent Lyme disease, but similar studies have not investigated antibiotic prophylaxis for the prevention of anaplasmosis. Thus, recommendations regarding the management of I scopularis bites are focused on the prevention of Lyme disease. This paper reviews the prevailing antibiotic prophylaxis recommendat…

Periprocedural Peritonitis Prophylaxis: A Summary of the Microbiology and the Role of Systemic Antimicrobials.

BACKGROUND: Peritonitis is a leading complication of peritoneal dialysis (PD). One strategy that the International Society for Peritoneal Dialysis (ISPD) has used to help mitigate the morbidity and mortality associated with peritonitis is through prevention, including antibiotic prophylaxis utilization in high-risk situations. The aim of this study is to summarize our current understanding of postprocedural peritonitis and discuss the existing data behind periprocedural antibiotic prophylaxis, focusing primarily on PD catheter insertion, dental procedures, colonoscopies, upper endoscopies with…

[Surgical antimicrobial prophylaxis : frequent questions in the perioperative setting].

Surgical Site Infections account for 15-30% of healthcare-associated infections. Surgical antibiotic prophylaxis is one of well-documented measures to decrease the risk of infections. However, many situations threaten its proper application and thereby its effectiveness: patient already receiving antibiotics or carrying resistant germs, clinicians' misunderstanding about timing and duration of antibiotic prophylaxis or patient history of penicillin allergy. In this article we try to clarify these situations to favour good practice in perioperative antibiotic prophylaxis.

Antibiotic Utilization and Infection Among Infants with Abdominal Wall Defects.

BACKGROUND: Neonates with abdominal wall defects are at an increased infection risk because of the defect itself and prolonged neonatal intensive care unit (NICU) stays. Antibiotic prophylaxis until closure of the defect is common. However, infection risk and antibiotic use have not been well quantified in these infants. METHODS: A retrospective cohort study of infants with abdominal wall defects (gastroschisis and omphalocele) admitted to a single-center NICU from 2007 to 2018. Demographic and clinical information, including microbiologic studies, antibiotic dosing and surgical care, were co…
