METHODS: The study included 139 children with glaucoma: 66 at risk for glaucoma (ie, aphakia, aniridia, or uveitis) and 66 normal children. A 13-year-old boy with Marfan's syndrome presented with bilateral, acute, complete anterior dislocation of the crystalline lens. The lens substance was aspirated intracapsularly after 2 linear capsulotomies were created. The empty capsular bag was removed from the eye as a vitrectome was used to severe the adhesions between the capsule and vitreous face through 1 paracentesis and a forceps was used to pull the capsule through the other paracentesis. Aphakia was corrected by glasses and contact lenses. Two years after surgery, the best corrected visual acuity was 20/32 in both eyes.

The authors compared the results of in vivo spectrophotometry of the eye in persons with a normal lens, in patients with aphakia and after an operation of intraocular lens implantation. The author reports on the first 14 cataract extractions performed by Karl Kaspar von Siebold, a Würzburg surgeon, in 1766/67. He used Daviel's method. At that time there was no anesthesia, no sterility or even antisepsis, and no optical correction of aphakia for his patients. Four eyes were lost due to infection and another four had much poorer vision after surgery due to irregular corneal scarring. In six cases both patient and physician were satisfied with the result. Siebold's case reports exhibit a fascinating degree of self-criticism and frankness.
