[The characteristics of the pathogenesis and immunomodulator treatment in the postoperative period in pseudophakia patients].

The investigation has revealed an immunological character of the exudative reaction in postoperative period of patients with artificial crystalline lens. We noted a cellular immunodeficiency and an increase of immunoglobulins G and A in the lacrimal liquid. The administration of 5-FU (5-fluoroucile) and levamizol decreased the inflammation in the postoperative period.

Acceptability of aphakic correction. Results from Karnali eye camps in Nepal.

Of 235 aphakic patients followed for 1-10 years in Karnali, Nepal, 23% were wearing aphakic spectacles in good condition, 25% had lost or broken their spectacles, 31% were wearing scratched or repaired spectacles, 5% never received spectacles and 16% were dissatisfied, of which 84% still had good phakic vision in the fellow eye. Of 303 aphakic eyes, 89% had a visual acuity of 6/60 or better with +10 spherical correction and 7.3% (22 eyes) were blind (V/A less than 3/60); of which the major causes were pre-existing posterior segment disease unrelated to the cataract operation.

Cyclo-oxygenase inhibitors in ophthalmology.

Cyclo-oxygenase inhibitors, which are formulated as ophthalmic eyedrop preparations, have recently become commercially available for use by ophthalmologists in the United States to inhibit intraoperative miosis during cataract surgery and to prevent postoperative inflammation. In addition, they are available worldwide as ocular antiinflammatory drugs and are used in the prevention and treatment of pseudophakic and aphakic cystoid macular edema. Understanding the rationale behind the use of these agents requires an understanding of the pathophysiology of the cyclo-oxygenase inhibitors. In this

Phosphatic metabolism and corneal edema.

From three groups of patients with corneal edema, the degree of edema was clinically quantified prior to penetrating keratoplasty. Corneas from patients with failed grafts (n = 16) had the greatest edema (mean score 3.8 +/- 0.6 SE), corneas with Fuchs' endothelial dystrophy (n = 12) had the least edema (1.4 +/- 0.4), and corneas with aphakic and pseudophakic bullous keratopathy (n = 40) had intermediate edema (2.7 +/- 0.5). Phosphorus-31 nuclear magnetic resonance spectroscopy was used to quantitate five high-energy phosphates and nine low-energy phosphates and calculate 12 metabolic indices f

The correction of unilateral aphakia in children treated for orbital rhabdomyosarcoma.

The survival rate of children with localized orbital rhabdomyosarcoma is now greater than 90% 3 years after diagnosis as a result of advances in radiation and chemotherapy. Ninety percent of these children develop cataracts within 1 to 4 years after the completion of radiotherapy. The correction of aphakia in these children is complicated by the concurrent keratoconjunctivitis and dryness associated with radiotherapy. Three patients with a diagnosis of orbital rhabdomyosarcoma underwent treatment for uniocular cataract. Two of the patients were unable to use extended wear contact lenses. Both

Dyschromatopsia following cataract surgery.

The authors report on 19 individuals who experienced colour obscurations (dyschromatopsia) following cataract surgery. Although a transient blue discoloration (cyanopsia) might be expected following cataract surgery, an erythropsia (red vision) is more common. Symptoms generally begin after outdoor activity in bright sunlight, vary in duration and are recurrent. It is important to look for a history of drug use, migraine or cerebrovascular accidents.

[Permanent-wear of gas-permeable rigid lenses: a new solution for the aphakic patient].

The authors reviews his experience during a period of six years with permanent wear gas-permeable hard contact lenses. Problems with dislocation of the lens during sleep was solved by using a diameter of 11 mm. Corneal respiration was maintained due to the oxygen permeability of the new material. A new fitting technique was developed advising daily wear for 8 days post-operatively, extended wear at the 15th day, and the final lens given after 45 days. The results obtained suggests that the technique is a very real alternative to intraocular lens implantation.

Unilateral traumatic aphakia in children: role of corneal contact lenses.

Twenty-eight children below ten years of age with unilateral traumatic aphakia were fitted with hard corneal contact lenses. Visual acuity of 6/12 or better was achieved in 68% of the cases. Patients who were above seven years of age at the time of injury, and those fitted with contact lenses within eight months of trauma, were found to have better chances of recovering normal binocular functions.

Pharmacologic therapy of aphakic and pseudophakic cystoid macular edema. 1985 update.

Major advances in the pharmacologic therapy of aphakic cystoid macular edema (ACME) have not occurred since 1982. Topical nonsteroidal anti-inflammatory agents are still not commercially available for ocular therapy. Topical indomethacin remains the one agent which has been proven to be of prophylactic value for angiographic aphakic cystoid macular edema although other non-steroidal agents may also work. The therapeutic value of these compounds for established ACME remains uncertain. No prospective randomized prophylactic or therapeutic trials of either topical or systemic corticosteroids have

Vitrectomy in complicated retinal detachments.

In this paper complicated traction retinal detachments are discussed in relation to aetiology, techniques of treatment and results of treatment. Complicated traction detachments are caused by pathological vitreous membranes or bands and epiretinal membranes. These membranes are found in the following groups of eyes: aphakia were vitreous loss, penetrating trauma, chronic vitreous inflammation and failed retinal detachments. Patients with extramacular traction detachments are not necessarily treated. Patients with diabetic traction retinal detachment are not included in this series. Of 150 cons

Unilateral cataracts in children.

Our experience in the surgical treatment of unilateral cataracts in children has been presented. Over two-thirds of the patients who were visually mature at the onset of their cataract regained useful vision in the operated eye while less than 10% of the visually immature patients did so. Our findings approximate those found in the literature. While improved visual acuity is the most important objective of unilateral cataract surgery in children, other benefits such as a cleared optical axis often make such surgery desirable, even when the prospect for visual recovery is poor.

Secondary implantation.

Aphakia is a disabling condition. It must be corrected by spectacles, contact lenses, refractive keratoplasty, or insertion of a pseudophakos. In those patients who do not tolerate a contact lens or cataract spectacles, refractive keratoplasty or secondary implantation of an IOL should be considered. Keratophakia and hyperopic keratomileusis have not yet evolved to the point at which refractive keratoplasty can be considered a practical answer to aphakia. This leaves a significant population of aphakic patients who can be visually rehabilitated only be secondary implantation of an IOL. Despite

Defects of vision through aphakic spectacle lenses.

The following defects of aphakic vision will be reviewed: (1) magnification, which causes false orientation (due to excessive overall magnification of aphakic spectacles, resulting in false depth, and false projection) and distortion (due to excessive peripheral magnification of aphakic spectacles, resulting in pincushioning and concave contracture of the peripheral visual field); (2) swim; (3) image aberrations due to defects of focus through the periphery of aphakic spectacle lenses, resulting from radial astigmatism, curvature of image plane, spherical aberration, coma, and chromatic aberra

Physiology of monocular aphakia.

The refraction of the unilateral aphakic patient has been aided by recent improvements in the ophthalmometer. New techniques simplify retinoscopic assessment of the patient. Choice of the type of correction, ie, spectacle, contact lens, contact lens-spectacle combination, or intraocular lens, depends on several factors. One factor is image size difference in aphakic and phakic eyes, particularly if removal of a second cataract is not imminent and vision is good in both eyes. Simple approaches that allow approximate size correction and effective restoration of binocularity are described. The cu

[Efficacy and safety of long-term maintenance treatment with timolol ophthalmic solution in chronic open-angle glaucoma (author's transl)].

Timoptol Ophthalmic Solution (timolol maleate, MSD) is a topical beta-adrenergic receptor blokking agent which lowers elevated intraocular pressure when applied once or twice daily. It represents a major advance in topical therapy for glaucoma and elevated intraocular pressure. It is more effective in reducing elevated intraocular pressure and produces fewer side effects than pilocarpine and epinephrine. It does not alter pupil size or affect visual acuity. Timoptol maintained satisfactory reductions in intraocular pressure in long-term maintenance studies treating over 100 patients for up to

Effects of timolol and Daranide on elevated intraocular pressure after aphakic keratoplasty.

In a randomized, blind clinical study, we tested the effect of timolol and Daranide against placebo medications among patients who had intraocular pressures greater than 30 mm Hg on the first postoperative day after aphakic keratoplasty. The study failed to show any significant difference in the pressure course. We concluded that because this type of severe secondary glaucoma is extremely difficult to treat, it should be prevented by using a donor cornea larger in size than the recipient bed.

Juvenile rhegmatogenous retinal detachment.

Between Jan 1, 1966, and June 30, 1974, 179 children, birth through 16 years of age with rhegmatogenous retinal detachments, underwent scleralbuckling procedures at Wills Eye Hospital. The detachments were associated most frequently with trauma (44%), myopia (15%), aphakia (10%), and retinopathy of prematurity (8%). Successful reattachment was achieved in 80% of the cases. Vision of 20/100 or better was present in 22% preoperatively and 58% postoperatively.
