Post-gastrointestinal endoscopy complications in patients with obstructive sleep apnea or at high risk for sleep apnea: a systematic review and meta-analysis.

BACKGROUND: Obstructive sleep apnea (OSA) is becoming increasingly more prevalent with the rise in obesity. Complications from gastrointestinal (GI) endoscopy in this patient population have been reported in several studies, but the modest complication rates from these procedures make it difficult to come to definitive conclusions based on single studies. The objective of our study was to systematically review these studies reporting the incidence of post-procedure complications in patients with OSA undergoing endoscopy to determine whether the presence of OSA increases post-procedure complica…

[Screening of sleep apnea syndrome using daytime polysomnography--its usefulness and the characteristics of daytime sleep in the patients with sleep apnea syndrome].

Some attempts have been made to screen sleep apnea syndrome (SAS). In this study, we performed daytime polysomnography (DPSG) as a means of screening SAS. 33 patients (28 males and 5 females, a mean age of 50.7 years) who were seen with the complaints of sleep-wake disorders, snoring and breathing disturbance in their nocturnal sleep were subjects for the screening. EEG, EMG, and EOG according to standard polysomnographic technique were recorded from 3:30 (p.m.) for 60 minutes, as additional indicators, oro-nasal airflow and abdominal movement were monitored simultaneously to confirm cessation…

Predicting the Apnea-Hypopnea index for the diagnosis of obstructive sleep apnea with cardiovascular and cancer comorbidity using wearable sleep-tracking devices.

INTRODUCTION: Obstructive Sleep Apnea (OSA) is a highly prevelance sleep breathing disorder that imposes, significant public health and economic burdens through its untreated associated comorbidities. The gold standard for diagnosing OSA, polysomnography (PSG), retains several limitations because it is cumbersome to conduct in the sleep lab and requires labor-intensive efforts to annotate the measurement. Furthermore, current research focuses on the alternative of PSG on the general OSA population without specifically considering the OSA comorbid conditions, such as cardiovascular and cancer d…

Clinical and polysomnographic heterogeneity in obstructive sleep apnea: a cluster analysis of the Berlin cohort from the European Sleep Apnea Database (ESADA).

INTRODUCTION: Obstructive sleep apnea (OSA) is a clinically heterogeneous disorder. Patients with similar disease severity differ in symptoms and nocturnal oxygenation. However, OSA is still primarily classified according to AHI thresholds, which may not fully reflect the diversity of clinical presentations in OSA. In this study, we applied cluster analysis using clinical and PSG variables to explore heterogeneity in OSA. METHODS: Patients with OSA were included in this monocenter observational study. Unsupervised clustering (k-means clustering) based on clinical characteristics and polysomno…

Obstructive sleep apnea screening performance of the STOP-BANG questionnaire and a home sleep apnea test device in atrial fibrillation ablation candidates.

BACKGROUND: Obstructive sleep apnea (OSA) contributes to the onset and progression of atrial fibrillation (AF) and negatively affects AF ablation outcomes. OSA screening in AF patients is often conducted with the STOP-BANG questionnaire, although its validation is lacking. This study aims to evaluate the screening value of the STOP-BANG questionnaire and a home sleep apnea test (HSAT) device for OSA in patients referred for AF ablation. METHODS: Patients referred for their first AF ablation and without prior OSA diagnosis underwent both the STOP-BANG questionnaire and a HSAT device based on p…

Sleep Apnea Clinical Score, Berlin Questionnaire, or Epworth Sleepiness Scale: which is the best obstructive sleep apnea predictor in patients with COPD?

INTRODUCTION: The Sleep Apnea Clinical Score (SACS) and the Berlin Questionnaire (BQ) are used to predict the likelihood of obstructive sleep apnea (OSA). The Epworth Sleepiness Scale (ESS) is used to assess daytime sleepiness, a common OSA symptom. These clinical tools help prioritize individuals with the most severe illness regarding on whom polysomnography (PSG) should be performed. It is necessary to check the applicability of these tools in patients with chronic obstructive pulmonary disease (COPD). The aim of this study is to compare SACS, BQ, and ESS performance in patients with COPD. …

Cognitive behavioral therapy for insomnia is associated with reduced sleep apnea severity but not its endotype traits in those with comorbid insomnia and sleep apnea.

STUDY OBJECTIVES: Cognitive behavioral therapy for insomnia (CBT-I) improves obstructive sleep apnea (OSA) severity in comorbid insomnia and sleep apnea, though the mechanisms underlying this change are unstudied. CBT-I, which promotes sleep continuity and reduces hyperarousal, may improve OSA by raising the respiratory arousal threshold. We aimed to investigate the effect of CBT-I on OSA severity and its impact on the arousal threshold and other endotype traits. METHODS: In this single-arm trial, 25 patients with comorbid insomnia and sleep apnea (13 females and 12 males, mean age [standard …

Obstructive Sleep Apnea in a Severely Obese Child With Combined Central Sleep Apnea and Sleep-Related Hypoventilation Disorder Caused by a Medullary Tumor.

A medullary tumor is a relatively rare disease that causes severe and complicated respiratory disorders, including sleep-related breathing disorders, due to dysregulation of respiratory control. A severely obese 12-year-old male was admitted to our hospital for worsening dyspnea and received a diagnosis of type II respiratory failure. Although obstructive sleep apnea (OSA) and/or obesity hypoventilation syndrome were suspected, a polysomnogram obtained during a nap (nap study) revealed central sleep apnea (CSA) and sleep-related hypoventilation disorder (SRHD) in addition to OSA. Brain magneti…

The Value of a Well-Trained Ear: Incidental Detection of Catathrenia on Home Sleep Apnea Tests in Patients With Low Probability for Obstructive Sleep Apnea.

Catathrenia is a rare sleep-related breathing disorder marked by nocturnal groaning during prolonged exhalation. Home sleep apnea tests are growing in popularity, but no prior case reports exist documenting catathrenia detected by a home sleep apnea test. We report two cases of catathrenia incidentally detected in patients with low to intermediate risk of obstructive sleep apnea, in which diagnostic polysomnography was recommended but denied by insurers. Events were automatically scored as central apneas, leading to diagnostic uncertainty. The key to identification of catathrenia in both patie…

Methylxanthine therapy for apnea of prematurity: evaluation of treatment benefits and risks at age 5 years in the international Caffeine for Apnea of Prematurity (CAP) trial.

Methylxanthine therapy reduces the frequency of apnea and the need for mechanical ventilation. Recent research has raised concerns about the safety of methylxanthines in very preterm infants. Possible adverse effects include poor growth, worsening of hypoxic-ischemic brain damage and abnormal childhood behavior. Over 2,000 infants with birth weights 500-1,250 g have been randomized in the international placebo-controlled Caffeine for Apnea of Prematurity (CAP) trial to examine the long-term efficacy and safety of methylxanthine therapy for the management of apnea of prematurity. Additional the…

Redefining Obstructive Sleep Apnea: Treatment in the Modern Era.

BACKGROUND: Obstructive sleep apnea (OSA) is a highly prevalent and heterogeneous disorder associated with substantial cardiometabolic morbidity. Although continuous positive airway pressure (CPAP) remains first-line therapy, long-term effectiveness is frequently limited by suboptimal adherence. Advances in airway devices, surgical techniques, neuromodulation, and pharmacologic therapies have expanded the therapeutic landscape and created opportunities for individualized, mechanism-based treatment. METHODS: We conducted a selective, narrative review with structured quantitative synthesis of r…
