Pseudogout masking other arthritides.

Of 314 patients with proved pseudogout, 9 had a separate serious disease in the same joint: 5 had septic arthritis, 2 rheumatoid arthritis, 1 psoriatic arthritis, and 1 hypertrophic pulmonary osteoarthropathy. In every instance but one, the episode of crystal synovitis appeared to be provoked by the new inflammatory process, disturbing previously asymptomatic chondrocalcinosis. Pseudogout attacks, although generally idiopathic solo events, can mask septic and chronic inflammatory arthritis.

Rheumatoid arthritis complicated by pachy- and leptomeningeal rheumatoid nodule-like granulomas and systemic vasculitis.

Rheumatoid nodule is a frequent and characteristic extra-articular manifestation of rheumatoid arthritis (RA). Its involvement of central nervous system is a rare occurrence with only a few reported cases. A 78-year-old man with severe arthritis showing the formation of rheumatoid nodule-like granulomas in the dura and subarachnoid space along with the spleen is presented. The characteristic morphological finding of the granulomas was the presence of neutrophils and the absence of definite fibrinoid necrosis, which differed from the typical features of rheumatoid nodules previously described.

Joint needle aspiration compared to tissue samples in septic arthritis of the native knee.

Culture of the causative infectious agent is the only definitive method of diagnosing septic arthritis and can be identified by culture of synovial fluid (SF) or by tissue cultures (TC) obtained at surgery. The aim of this study is to compare the cultures of joint needle aspiration (JNA) with tissue cultures obtained at surgery. 52 patients treated for a suspicion of a septic arthritis of the native knee were retrospectively reviewed. In 84% tissue cultures were equal to joint needle aspiration. Median Gächter classification was 1.0 (range 1-3). 60% of the patients underwent multiple procedure

Major involvement of bacterial components in rheumatoid arthritis and its accompanying oxidative stress, systemic inflammation and hypercoagulability.

We review the evidence that infectious agents, including those that become dormant within the host, have a major role to play in much of the etiology of rheumatoid arthritis and the inflammation that is its hallmark. This occurs in particular because they can produce cross-reactive (auto-)antigens, as well as potent inflammagens such as lipopolysaccharide that can themselves catalyze further inflammagenesis, including via β-amyloid formation. A series of observables coexist in many chronic, inflammatory diseases as well as rheumatoid arthritis. They include iron dysregulation, hypercoagulabili

Arthropathies and inflammatory conditions of the elbow.

MR imaging of arthropathies and inflammatory conditions affecting the elbow are presented. Noninfectious conditions discussed include osteoarthritis, disorders characterized by synovial proliferation, pigmented villo- nodular synovitis, synovial osteochondromatosis, crystal deposition disorders, and neuropathic osteoarthropathy. Infectious conditions discussed include septic olecranon bursitis, septic arthritis, osteomyelitis, and pyomyositis. Clinical aspects of these entities are discussed, including utility of MR imaging for diagnosis and clinical management.

Dalbavancin for the treatment of bone and joint infections: A meta-analysis.

BACKGROUND: Bone and joint infections are challenging infectious diseases to treat and require prolonged antimicrobial treatment. Dalbavancin demonstrated promising pharmacokinetic/pharmacodynamic properties for the treatment of these infections. The objective of this meta-analysis is to compare the effectiveness of dalbavancin to standard of care (SOC) for the treatment of bone and joint infections. METHODS: Two independent authors performed a comprehensive search through the major databases up to September 2023. Interventional and observational studies that compared the clinical success of

[Bacterial arthritis with E. coli in an elderly patient].

Infectious arthritis is mostly caused by hematogenous spread of Gram-positive bacteria, which often infects a previously damaged joint. During the past 20 years there has been a notable increase in joint infections caused Gram-negative bacteria. They develop mostly in patients with systemic diseases, such as malignancy, cirrhosis or HIV infection, which cause an immune deficient state. We present an 84-year old man admitted because of fever and a diagnosis of pneumonia. During hospitalization he complained of pain in his right knee. On physical examination there was evidence of local inflammat

Infectious but nonpathogenic isolate of Borrelia burgdorferi.

We document for the first time an infectious but nonarthritogenic variant of Borrelia burgdorferi. Strain 25015, previously isolated from an Ixodes dammini larva collected in upstate New York, was infectious but failed to produce arthritis or carditis in laboratory rats and mice. By contrast, pathogenic strain N40 invariably caused arthritis. This nonarthritogenic variant, with proteins with molecular weights different from those of the standard B31 strain, was frequently isolated from normal joint tissues of experimentally infected rats. Outer surface proteins A and B of strain 25015 have mol

Postpartum septic sacroiliitis misdiagnosed as sciatic neuropathy.

Early diagnosis of septic sacroiliitis is difficult because symptoms are nonspecific, especially during pregnancy and the postpartum period. We describe a female patient with left buttock pain radiating down the thigh after an uncomplicated induction delivery. She was afebrile and had no apparent abnormality on pelvic x-ray or computed tomography scan. A sensory deficit in the lateral portion of her left lower limb was found, and electromyography showed neurogenic abnormalities in the left lower limb. She was initially misdiagnosed as sciatic neuropathy. As her symptoms worsened, septic sacroi

Septic Arthritis of the Acromioclavicular Joint: A Case Report.

UNLABELLED: Septic arthritis of native joints is uncommon, but the condition can be threatening to life and limb if left untreated.1 Septic arthritis of the acromioclavicular (AC) joint of the shoulder is particularly rare and has only appeared sparsely in medical literature, mainly through individual case reports. Early recognition and treatment of the condition is vital, but diagnosis of septic AC arthritis can be difficult due to its presentation with vague symptoms and nonspecific laboratory findings. This case report describes the care of a patient with poorly managed diabetes who present

Misdiagnosis of late-onset Lyme arthritis by inappropriate use of Borrelia burgdorferi immunoblot testing with synovial fluid.

The primary objective of this study was to determine whether patients with putative late-onset Lyme arthritis based upon synovial fluid Borrelia burgdorferi IgM and IgG immunoblot testing offered by commercial laboratories satisfied conventional criteria for the diagnosis of Lyme arthritis. Secondary objectives included assessing the prior duration and responsiveness of associated antibiotic therapy. We conducted a retrospective analysis of 11 patients referred to an academic medical center infectious disease clinic during the years 2007 to 2009 with a diagnosis of Lyme disease based upon prev

[Current concepts on the physiopathology of adult-onset Still's disease].

PURPOSE: Adult-onset Still's disease (AOSD) is a rare systemic inflammatory disorder of unknown origin. It is characterized by hectic fever, evanescent rash, polyarthralgias or polyarthritis, sore throat, hepatosplenomegaly, lymphadenopathy, polynuclear leukocytosis, liver cytolysis, and high serum level of ferritin with low glycosylated fraction. CURRENT KNOWLEDGE AND KEY POINTS: An increased serum level of ferritin, IL-8, IL-6, IL-18 and TNF-alpha indicates that macrophages are highly activated in AOSD. Interleukin 18 (IL-18) seems to be a key cytokine in the pathogenesis of AOSD. Serum IL-
